A 29-year-old Haitian man had a sore throat with a marked tonsillar exudate followed by the onset of increasingly blurred vision with swallowing difficulties and, then, a diffuse acute demyelinating neuropathy. Cerebrospinal fluid showed moderate pleiocytosis and elevated protein content. Serological testing for diphtheria, but not for tetanus and poliomyelitis, was markedly positive. Other tests including HIV1 and 2, HTLV1 and Lyme disease were negative. Diphtheritic neuropathy should be considered in the differential diagnosis of Guillain-Barré syndrome in nonvaccinated patients.

Twenty patients aged 56-92 years who underwent surgical correction of pharyngeal pouches at Queen Alexandra Hospital, Portsmouth were asked to fill in a detailed questionnaire. They were then interviewed and barium swallow studies performed from 3 months to 4 years post-operatively. Nineteen patients in the sample were found to have reflux and 20 had dysmotility. These findings suggest that pharyngeal pouches are not a purely localized inco-ordination of the cricopharyngeal sphincter but are associated with a generalized oesophageal muscle dysfunction.

The authors present an account of their experience with retrosternal oesophagocoloplasty as a palliative by pas in inoperable carcinoma of the oesophagus. They differentiate secondary and primary indications. So far they used this operation in 27 patients. The total lethality is 33%, the mean survival period is 8 months. Some patients with the unilateral paresis of the left recurrent nerve have laryngoesophageal dysfunction with the danger of aspiration during swallowing. The shape, position and function of the paretic vocal cord can be favourable influenced by the administration of hydrophile

17% of postoperative ICU patients develop postoperative complications. In 84% of cases, complications are related to surgery, in 16% they are not. The main postoperative surgical complications are abdominal sepsis (65%), postoperative bleeding (25%), and bowel obstruction (7%). Therefore, the primary objective of postoperative investigations must be to check the sites of operation. Bedside procedures are examination of drainage fluid, sonography, endoscopy, and X-ray (gastrografin swallow). The diagnostic value of these procedures is 80%. Other diagnostic procedures include CT, angiography, an

A 28-year-old man presented with lethargy, solmulence, and polyuria following near-drowning in a vessel of an offshore oil rig. Laboratory evaluation demonstrated severe hypercalcemia that responded to saline diuresis and nasogastric suctioning. Calcium salts are used frequently in the drilling and completion of oil wells, and it is presumed that this patient's hypercalcemia represented acute intoxication from swallowed and aspirated fluid. This case highlights the need to consider the potential constituents of the drowning fluid in victims of near-drowning, particularly if unexplained clinica

A de novo interstitial deletion of chromosome 13 (46,XY,del(13)(pter----q14.3::q22.3----qter] is described in a 22 year old man with severe mental retardation, poor language development, low set ears, hypertelorism, broad nasal bridge, short hands and fingers, and a history of swallowing disorder in childhood with subsequent dyspepsia. The dysmorphic features did not become evident until later childhood, supporting the view that karyotyping should be performed routinely in all children with developmental delay.

Seventy patients with bilateral strokes underwent neurologic and videofluoroscopic barium swallowing examinations; 34 (48.6%) aspirated. Patients with aspiration were more likely to have posterior circulation strokes, abnormal cough, abnormal gag, and dysphonia. However, patients likely to aspirate can be identified best by the presence of an abnormal voluntary cough, an abnormal gag reflex, or both. The prediction of patients at risk for aspiration was not improved by additional clinical information (ie, presence of dysphonia or bilateral neurologic signs).

The case is reported of a 23-year-old man, working in agriculture, who was exposed to the silage additive ammonium tetraformiate, a strong acid. While pumping the fluid some of the substance entered his mouth and was swallowed. He vomited and developed transient dyspnoea. On admission he appeared acidotic, with haemolysis and haemoglobinuria. Lesions on the soft palate and in the oesophagus were observed, and a pulmonary infiltrate developed. Information on the substance was initially not available. Labeling appeared incomplete and misleading. Optimal treatment was delayed.

Total laryngopharyngectomy results in a considerable degree of morbidity. Although many advances have been made over the past 10 years, the restoration of speech remains an unsolved problem. Herein, we present three patients who underwent both neoglottic and neopharyngeal reconstruction using a free vascularized segment of jejunum. All three patients were able to swallow and two were able to speak. We believe that this method of reconstruction restores speech after total laryngopharyngectomy.

Suppurative thyroiditis due to branchial pouch sinus is a rare disease, affecting children and young adults. It presents as an abscess in the lower lateral neck, usually on the left side. Suppurative thyroiditis can be suggested by ultrasound, radionuclide scintigraphy or CT scan, but the diagnosis can only be established by barium swallow showing the sinus from the apex of the pyriform sinus to the perithyroid region. Complete surgical excision of the sinus is essential. Two patients are presented and the role of the various imaging modalities in the diagnosis is discussed.

This study was conducted to examine the pharmacokinetics of doxycycline in nine healthy volunteers who had fasted overnight. Single-dose cross-over design was used. Each subject received orally 0.2 g doxycycline. The subjects swallowed the antibiotic capsules with 300 ml of water or 300 ml of fresh milk. Simultaneous ingestion of milk diminished the peak plasma concentration by 24% and the absorption by 9-53% (mean 30%). It is concluded that, like the other tetracyclines, doxycycline should not be administered together with milk.

A patient with treated pulmonary tuberculosis and a thoracic aortic aneurysm was seen with a one-month history of dysphagia. Barium swallow revealed a mass in the lower esophagus and extravasation of contrast material into the mediastinum. Endoscopy and biopsy specimens showed acid-fast organisms. The patient was treated with antituberculous drug therapy but bled massively from the gastrointestinal tract and died. Autopsy revealed an aortoesophageal fistula at the level of the thoracic aneurysm. Histopathological study confirmed that this rare tuberculous lesion of the esophagus caused the fis

We report on the unexpected death at almost 8 years of a boy with the G syndrome who had successfully survived many prior life-threatening complications of the condition. The patient had a characteristic facial appearance, hypertelorism, second-degree hypospadias, stridor and cough on feeding with aspiration of barium, and uncoordinated esophageal swallowing mechanism, but no obvious laryngotracheobranchial defect on endoscopy (or coroner's autopsy). He had a prior cardiac arrest and had outgrown need for a tracheostomy. Aspiration is presumed to be the cause of death.

During the period between 1965 and 1983, 101 pharyngolaryngoesophagectomies with primary pharyngogastric anastomosis were carried out for primary hypopharyngeal cancer at The Royal National Throat, Nose and Ear Hospital, London, England. The Hospital mortality rate was 11%; postoperative complications such as pleural effusion and regurgitation of bile occurred in all patients. Successful swallowing occurred in all patients before they left the hospital--usually within 2 wk of surgery. An actuarial survival rate of 58% was obtained for the entire group.

Disorder of deglutition is one of the severest signs in postoperative or posttraumatic paresis of the caudal cranial nerves. The main problem in these cases is aspiration. It is a serious disorder with potentially life-threatening pulmonary sequelae. In cases with unilateral paresis of the vagus and glossopharyngeal nerve we can consider the problem of surgical rehabilitation of swallowing as to be solved very well, as described in the present paper. However, in cases with bilateral vagus paresis we are faced with many more problems especially in the high-grade aspiration syndrome. The surgica

The best approach to evaluation of a mediastinal mass begins with a history and a physical examination that are oriented toward discovery of evidence of compression or obstruction to any of the mediastinal organs. Currently, the most efficient radiographic approach, after the plain chest roentgenogram, is computed tomography of the thorax, supplemented as needed by selected plain tomograms or a barium swallow examination. Unless the mass is definitely a benign lesion of no consequence, such as a pericardial fat pad, thoracotomy by an experienced thoracic surgeon is usually necessary to make a

Reports of Visceral larva migrans (VLM) are not frequent in Brazil, but the prevalence of this disease may be more common due to poor environmental conditions to which most of the children are exposed. The most common infectious agent is Toxocara canis, a parasite of the intestinal tract of dogs. Children get infected eating contaminated material with feces of dogs. The ova are swallowed and reach the intestinal lumen of the children. Systemic migration occurs by lymphatic or portal circulation to several organs, most frequently, to the liver and lungs, causing hepatosplenomegaly and/or bronch

Myotonic dystrophy in the neonate is commonly accompanied by facial diplegia, generalized muscular hypotonia, talipes equinovarus, and muscular respiratory failure. The gastrointestinal manifestations of this disease include poor sucking, choking, regurgitation, aspiration, and swallowing difficulties. Gastroparesis can be a major contributor to the feeding difficulties experienced by these infants. We report on an infant with congenital myotonic dystrophy in whom a severe gastric motility problem was alleviated by metoclopramide therapy. This smooth muscle manifestation may be an important an

Congenital vascular anomalies of the great vessels causing marked tracheoesophageal compression are rare. Notable symptoms usually manifest in the neonatal period and include stridor, repeated cyanotic attacks, recurrent pneumonia, and dysphagia. Although the diagnosis can often be made from barium swallow studies, the preoperative examination should nevertheless include an endoscopy. This is essential if the degree of tracheomalacia is to be adequately assessed and other tracheobronchial abnormalities are to be identified. Possible airway problems can then be anticipated, thus minimizing post

A forty year-old Japanese woman complained of slight hematemesis and severe pain on swallowing. Fluoroscopic examination showed typical esophagograms of "double barreled esophagus" or "mucosal stripe". An esophageal ulceration, probably where the dissection re-entered the usual lumen, was endoscopically evident at the lower end of the mucosal bulging. A large, shallow ulcer of unknown cause was also identified in the just anal area from the esophago-gastric junction. She recovered with conservative treatment of fasting, cimetidine and sodium alginate.
