BACKGROUND: It is commonly believed in medicine that using heat will increase the distensability and flexibility of soft tissue. If true, increased flexibility would be a positive factor to reduce injuries in sports. However, cold should have the opposite effect and is often used to treat sports injuries. This study was accomplished to quantify the effect of heat and cold on the force needed to flex the knee and laxness of the anterior and posterior cruciate ligaments.
MATERIAL AND METHODS: The present study examined 20 male and female subjects to determine if heat would increase extensibilit

The medical records of 186 patients seeking treatment for landmine injuries in the authors' region between 1993 and 2001 were evaluated. Of these patients 13 died of accompanying complications. Ten (10) patients with general body trauma and upper limb trauma were excluded from the study. Of 163 patients with lower-limb injuries included in the study, 21 with traumatic amputation underwent surgical amputation at different levels. Patients without traumatic amputation were divided into 2 groups. There were 41 patients (29%) in Group I who were treated by limb salvage procedures. Treatments used

The aetiology of chronic functional lateral ankle instability is fairly well understood. Pathophysiological factors such as mechanical instability, proprioceptive deficit and peroneal muscle weakness have been demonstrated. Subtalar instability has been in focus during the last years as one of the possible factors behind chronic functional instability of the foot. The exact aetiology and the true incidence of subtalar ligament injuries remain unknown. Most subtalar ligamentous injuries probably occur in combination with injuries of the talo-tibial articulation. Subtalar instability can have th

Altered semitendinosus (ST) morphology and distal tendon insertion following anterior cruciate ligament reconstruction (ACLR) may reduce knee flexion torque generating capacity of the hamstrings via impaired ST force generation and/or moment arm. This study used a computational musculoskeletal model to simulate mechanical consequences of tendon harvest for ACLR on ST function by modeling changes in ST muscle tendon insertion point, moment arm, and torque generating capacity across a physiological range of motion. Simulated ST function was then compared between ACLR and uninjured contralateral

BACKGROUND: Although previous literature has described the relevant anatomy for an open anterior Bankart approach of the shoulder, there is little known regarding the anatomic relationship changes in the neurovascular structures after an open Latarjet procedure.
PURPOSE: To define the neurovascular anatomy of the native shoulder in relation to the coracoid and to define the anatomy after the Latarjet procedure in relation to the glenoid to determine distances to these neurovascular structures with and without neurolysis of the musculocutaneous nerve (MCN) from the conjoint tendon.

Tears of the superior labrum involving the biceps anchor are a common entity, especially in athletes, and may highly impair shoulder function. If conservative treatment fails, successful arthroscopic repair of symptomatic SLAP lesions has been described in the literature particularly for young athletes. However, the results in throwing athletes are less successful with a significant amount of patients who will not regain their pre-injury level of performance. The clinical results of SLAP repairs in middle-aged and older patients are mixed, with worse results and higher revision rates as compar

The long head of the biceps (LHB) is commonly implicated in shoulder pathology due to its anatomic course and intimacy with the rotator cuff and superior labrum of the glenoid. Treatment of tendinosis of the LHB may be required secondary to partial thickness tears, instability/subluxation, associated rotator cuff tears, or SLAP (superior labrum, anterior to posterior) lesions. Treatment options include open or arthroscopic techniques for tenodesis vs tenotomy. Controversy exists in the orthopedic literature regarding the preferred procedure. The all-arthroscopic biceps tenodesis technique is a

PURPOSE: There is no consensus in the literature regarding the optimal surgical treatment of symptomatic partial rotator cuff tears. We attempted to determine the optimal surgical treatment for partial articular-sided rotator cuff tears through a systematic review of appropriate studies.
METHODS: Medline®, PubMed, Ovid, and the cochrane register of controlled trials were searched for all studies published between January 1991 to March 2010 that used the key words "shoulder", "partial rotator cuff tear", "PASTA", "articularsided rotator cuff tear", "incomplete rotator cuff tear", "arthroscopic

A new skin or fascial flap is described, based on the dorsal ulnar artery. In hundred fresh cadaveric forearms this vessel was constant with a diameter of 1 to 1.3 mm, passing dorsally from the ulnar artery deep to the flexor carpi ulnaris muscle. It supplies the skin and the fascia in the distal two thirds of the ulnar side of the forearm (length from 9 to 20 cm; width 1.5 to 10 cm). A flap based on this vessel can cover defects of the dorsal and palmar aspects of the hand and wrist, the thenar and the hypothenar eminence. It can be used as a fascial flap to avoid fibrosis around the median n

Wrist instabilities are frequently underdiagnosed and undertreated because they are complex and difficult to understand. A study of the anatomy of the wrist and the pathomechanics of these injuries leads to a better understanding of these problems and to a more specific diagnosis. Treatment involves stabilization of the unstable segment by ligament repair in the acute injury and by limited intercarpal arthrodesis in the chronic case.

Accurate diagnosis in ligament and ligament-bone injuries of the foot and ankle demands a knowledge of the anatomy and mechanics of the foot and ankle, a good history of the mechanism of injury, clinical examination including fingertip palpation and assessment of stability, and adequate x-ray studies, perferably in the deformed position, with stress films and arthrograms in selected cases. By making an accurate diagnosis, early proper treatment is instituted, the athlete is restored to health sooner, and, almost as important, the athlete and the coach get a more realistic prognosis regarding t

A 5-year-old Poodle presented with a plantigrade stance three weeks after traumatic gastrocnemius tendon rupture. Surgical exploration confirmed a complete rupture; following debridement and tenorrhaphy, a 20 mm defect remained. This gap was bridged using a double-layer ultra-high-molecular-weight polyethylene (UHMWPE) suture tape and augmented with flexor digitorum lateralis (FDL) transposition. At 2 months, ultrasonography confirmed maintained tendon continuity without foreign body reaction. By 3 months, posture normalized and gait analysis showed near-symmetric hindlimb loading. No major co

OBJECTIVE: This study aimed to evaluate surgical management of superficial digital flexor tendon (SDFT) luxation by modifying the morphology of the calcaneal tuberosity to (1) reduce the risk of recurrent luxation and (2) challenge the need for tarsal joint immobilization postoperatively.
STUDY DESIGN: Prospective observational study.
SAMPLE POPULATION: A total of 11 surgeries in nine client-owned dogs.
METHODS: Dogs with SDFT luxation and no concurrent disease underwent modified block recession calcaneoplasty without joint immobilization. Follow-up at 6 weeks and 12 months included pressur

BACKGROUND: Gluteal tendon tears (GTT) are associated with significant morbidity, similar to end stage, hip osteoarthritis. To date, there is unclear evidence as to what best management for GTT constitutes, and what are the subsequent outcomes for the different management approaches (surgical, non-surgical). The purpose of this systematic review is to document treatments, synthesise evidence, and assess the quality of available evidence regarding the treatments and outcomes associated with symptomatic GTT. METHODS: A systematic review was conducted according to PRISMA (Preferred Reporting Item

Upper limb spasticity is a frequent and disabling consequence of cervical spinal cord injury. In tetraplegic patients, spasticity may either support residual function or severely compromise surgical reconstruction, hygiene, comfort, and independence. Over the past decade, advances in neurophysiology, diagnostic tools, and surgical techniques have substantially modified the management of spastic upper limbs in tetraplegia. This review synthesizes recent advances with a particular focus on surgical decision-making, selective neurotomy, and combined tendon procedures, and highlights their impact

Hip abductor pathology exists on a spectrum, potentially involving symptoms of pain, weakness, loss of active motion, and disordered movement. Imaging findings may include tendinosis, detachment of a portion of the footprint, fluid imbibition with undersurface tearing at the footprint, or full-thickness detachment of 1 or both tendons with or without evidence of fatty infiltration or atrophy of the associated muscle bellies. Yet, sometimes there are no imaging findings at all. Endoscopic repair of hip abductor tendon tears is growing in popularity and interest. Recent research suggests that wi

There is a growing need for nonarthroplasty treatment options for irreparable rotator cuff tears. Options include superior capsular reconstruction (SCR), tendon transfers, subacromial balloon spacer, bridge grafting, biological tuberoplasty, and partial rotator cuff repair with or without augmentation. In our experience, repair with marginal convergence techniques is superior to SCR; if there is enough tissue, repair is the preferred strategy. In an effort to improve outcomes after repair, there has been increased interest in the use of interposition or bridging graft (BG) techniques, in which
