The use of a central one-third patellar tendon as an autograft for surgical reconstruction of a damaged cruciate ligament is common. Few complications of its use have been reported. To compare the functional results of early controlled mobilisation and static immobilisation following repair of extensor tendons, we conducted a comparative study between two centres. In one, a consecutive series of tenorrhaphy patients was treated post-operatively by the dynamic splinting technique. In the other, a consecutive group was treated by static splinting. Review of the notes of 46 extensor tendon repairs in 21 patients treated by post-operative dynamic traction without an M.P. flexion block, no tendon ruptures or extensor lag and only one digit without full flexion after a mean follow-up of seven weeks. Re-examination of 26 treated repairs in nine patients for this study demonstrated a mean T.A.M. of 259 degrees at an average 14 months follow-up. No bow-stringing occurred because the extensor retinaculum was not excised, and no tenolyses were necessary.

Osgood-Schlatter disease is a common condition which affects many adolescents. It is generally a self-limiting process with a finite clinical course and late sequelae are notably uncommon. In this paper we present a unique case of bilateral fractures through "giant" patellar tendon ossicles in an adult with a history of Osgood-Schlatter disease.

A previously undescribed accessory flexor digitorum superficialis muscle-tendon unit was discovered on surgical exploration of the wrist in an 18-year-old girl with symptoms of distal ulnar neuropathy and a volar soft-tissue mass. Excision of the muscle-tendon unit resulted in complete resolution of symptoms. The preoperative diagnosis of an accessory muscle is difficult, being commonly mistaken for synovitis, ganglia, or tumor.

Various chick embryo tissues were incubated in vitro with a range of Zn2+ concentrations, and inhibition of the hydroxylations of collagen proline and lysine residues was studied in the intact tissues. At an constant inhibition level of proline hydroxylation, lysine hydroxylation proved to be inhibited more in the skin and bone than in the tendon and cartilage. The ratios of lysyl hydroxylase to prolyl hydroxylase activity were also distinctly lower in the former than in the latter. The paper reports on the basis of long-term observations on an old surgical method which used to be practised in infants before arthrolysis around the anklebone (pantalar arthrolysis) was employed in the treatment of insufficient lifting of the outer edge of the foot in clubfoot recidivation. This old surgical method consisted of translocating the tendon of the m. peroneus brevis (m. fibularis brevis) before the outer ankle bone. The method is described, and its mode of action is discussed. Results obtained with 14 operations of this kind, are presented.

Reconstruction of the collateral ligaments of the metacarpophalangeal joint of the thumb was performed for fourteen ruptures of the ulnar and two ruptures of the radial collateral ligament, one to eighteen months after injury. The extensor pollicis brevis tendon was used to reinforce the resutured ligament. Symptoms were relieved in all cases after follow-ups ranging from one to seven years, even in two thumbs with mild degenerative arthritis of the metacarpophalangeal joint.

Tourniquet paralysis, arterial spasm, aneurysm formation, and the post-ischemic hand syndrome are all complications of penumatic tourniquet use in surgery of the extremities.
