Relative contribution of envelope and fine structure to the subcortical encoding of noise-degraded speech.

Brainstem frequency-following responses (FFR) were elicited to the speech token /ama/ in noise containing only envelope (ENV) or fine structure (TFS) cues to assess the relative contribution of these temporal features to the neural encoding of degraded speech. Successive cue removal weakened FFRs with noise having the most deleterious effect on TFS coding. Neuro-acoustic and response-to-response correlations revealed speech-FFRs are dominated by stimulus ENV for clean speech, with TFS making a stronger contribution in moderate noise levels. Results suggest that the relative weighting of tempor

Minimally Invasive Endoscopic Supracerebellar-Infratentorial Surgery of the Pineal Region: Anatomical Comparison of Four Variant Approaches.

OBJECTIVE: The endoscopic supracerebellar-infratentorial (SCIT) approach is a viable method to access pathology of the posterior incisura, but a narrow working space and frequent instrument conflict can potentially limit its surgical efficacy. Until now, no rigorous studies were available comparing surgical freedom and angle of attack for four previously well-described approaches to pineal region targets. METHODS: Four formalin-fixed cadaver heads were dissected bilaterally (eight sides). A midline approach and three progressively lateral approaches to the pineal region were performed (parame

Painful tonic spasms and brainstem involvement in a patient with neuromyelitis optica spectrum disorder.

Neuromyelitis optica (NMO) is an inflammatory-demyelinating disease of the central nervous system classically characterized by optic neuritis and severe myelitis. New diagnostic criteria defined neuromyelitis optica spectrum disorder as limited forms of NMO or diverse neurologic presentations in the presence of specific antiaquaporin-4 antibodies. We report the case of a 57-year-old woman admitted in our department for recurrent attacks of optic neuritis, tetraparesis with severe painful tonic spasms of the left limbs and brainstem involvement. Painful tonic spasms have been described as movem

Regeneration of neuronal cells following cerebral injury.

Stem cells possess a definitive role in neuronal rejuvenation following a cerebral injury. Whether endogenous, from the neurogenic niches of the subventricular zone and subgranular zone, or recruited from the bone marrow through peripheral circulation, accumulating evidence demonstrates that stem cells ameliorate the consequences of cerebrovascular events, particularly cerebral ischemia. In this chapter, we review milestone studies implicating the role of stem cells in response to disease. Furthermore, we outline specific mechanisms of action along with their clinical potential as therapeutic

[Regenerating neurons by using endogenous neuronal progenitor cells].

Currently, there is no effective treatment for the neuronal loss caused by neurodegenerative diseases, such as amyotrophic lateral sclerosis (ALS) or ischemic stroke. However, recent studies have shown that endogenous neural progenitor cells continuously generate new neurons in the subventricular zone (SVZ) of the adult mammalian brain. Some of these new neurons migrate to the injured site and differentiate into mature neurons. Such new neurons may be able to replace degenerated neurons and improve or repair neurological deficits. To establish a neuroregenerative therapy using this endogenous

Exceptions to the Valsalva doctrine.

In 1707, Valsalva reported a patient with unilateral cerebral hemorrhage associated with weakness on the contralateral side of the body, and Morgagni in De Sedibus subsequently referred to this crossed relationship of brain to body as the Valsalva doctrine. Since then, decussation of corticospinal tracts and many other neural pathways within the CNS has become so deeply embedded in neurologic thinking that exceptions may not be considered and therefore may be overlooked. Several uncommon clinical events that result in paralysis ipsilateral to brain injury highlight nuances of normal human neur

Eye movement abnormalities in multiple sclerosis.

Patients with multiple sclerosis commonly describe visual symptoms that result from several eye movement abnormalities that occur from disruption of critical pathways in the brainstem, cerebellum, and cerebral hemispheres. These abnormalities include internuclear ophthalmoplegia, ocular motor palsy, ocular misalignment, pathologic nystagmus, impaired saccades, saccadic intrusions, and impaired pursuit. Detailed knowledge of these problems and their neuroanatomic localization will aid the physician by guiding diagnosis and therapeutic decision making.

Mixed germ cell tumor of the midbrain. Case Report.

This 14-year-old boy presented with left hemiparesis, gait disturbance, and multiple cranial nerve palsies. Magnetic resonance imaging revealed a multicystic mass with hemorrhagic fluid-fluid levels in the right midbrain, suggesting the presence of a cavernous malformation. Diffusion tensor imaging showed the mass to be close to the right corticospinal tract and ipsilateral medial lemniscus. Subtotal removal of the mass was performed via a right subtemporal approach. The histopathological diagnosis was of a mixed germ cell tumor (GCT) comprising mature teratoma and germinoma cells with syncyti

Brainstem ganglioglioma.

Gangliogliomas are usually benign slow-growing neoplasms, seen mainly in the first 3 decades of life and are prevalently located supratentorial, mostly in the temporal and frontal lobe. The authors present a rare case of a brainstem ganglioglioma in an 11-year-old boy who was referred to their hospital complaining of episodes of blurry vision, loss of memory, gait disturbances, and morning headache with vomiting, lasting for over a month. Computed tomography and magnetic resonance imaging scans revealed a mass on the dorsal surface of the brainstem, compressing the brainstem and producing seco

Treatment for Fisher syndrome, Bickerstaff's brainstem encephalitis and related disorders.

BACKGROUND: Fisher syndrome is one of the regional variants of Guillain-Barré syndrome, characterised by impairment of eye movements (ophthalmoplegia), incoordination (ataxia) and loss of tendon reflexes (areflexia). It can occur in more limited forms, and may overlap with Guillain-Barré syndrome. A further variant is associated with upper motor neuron signs and disturbance of consciousness (Bickerstaff's brainstem encephalitis). All of these variants are associated with anti-GQ1b IgG antibodies. Intravenous immunoglobulin (IVIg) and plasma exchange are often used as treatments in this patient

Spinal and supraspinal postural networks.

Different species maintain a particular body orientation in space (upright in humans, dorsal-side-up in quadrupeds, fish and lamprey) due to the activity of a closed-loop postural control system. We will discuss operation of spinal and supraspinal postural networks studied in a lower vertebrate (lamprey) and in two mammals (rabbit and cat). In the lamprey, the postural control system is driven by vestibular input. The key role in the postural network belongs to the reticulospinal (RS) neurons. Due to vestibular input, deviation from the stabilized body orientation in any (roll, pitch, yaw) pla

Neonatal auditory function and depressed Apgar score: correlation of brainstem auditory response with Apgar score.

AIM: To examine the relationship between neonatal auditory function and Apgar score in term infants with depressed Apgar scores. METHODS: Brainstem auditory evoked response (BAER) was recorded on day 3 after birth in term infants who had Apgar scores < or = 7 at 1 and/or 5 min. Half of the infants also had Apgar scores at 10 min, with 17 < or = 7. RESULTS: No BAER variables correlated significantly with 1-min Apgar score. However, wave III and V latencies, and I-V, I-III and III-V intervals correlated significantly and negatively with 5-min Apgar score (p < 0.05-0.01). These BAER variables w

Locked-in syndrome due to metastatic pontomedullary tumor--case report.

A 21-year-old man presented with an extremely rare case of locked-in syndrome caused by a metastatic brainstem tumor manifesting as quadriplegia, lower cranial nerve pareses, and irregular respiration. Cranial magnetic resonance imaging revealed a large pontomedullary tumor. An emergency operation was performed via a posterior fossa approach and the tumor was grossly totally removed. The histological diagnosis was malignant melanoma. The clinical status of the patient remained unchanged and he died on postoperative day 34 of diffuse bronchopneumonia. Locked-in syndrome is characterized by quad
