Transcranial direct current stimulation and cognitive training in the treatment of cognitive deficit in schizophrenia: a randomized controlled trial.

BACKGROUND: Cognitive impairment is a core feature of schizophrenia spectrum disorders and a major contributor to functional impairment in patients with the disorder. Given the limited effects of pharmacological treatments on cognition, there is an increasing interest in the use of alternative therapeutic approaches, including cognitive training and non-invasive brain stimulation. Our study aimed to evaluate the effectiveness of the combined treatment of tDCS and cognitive training on improving cognitive functions in patients with schizophrenia. METHODS: This one-week, randomized, double-blind

Pathogenic variants associated with speech/cognitive delay and seizures affect genes with expression biases in excitatory neurons and microglia in developing human cortex.

BACKGROUND & OBJECTIVE: Congenital brain malformations and neurodevelopmental disorders (NDDs) are common pediatric neurological disorders and result in chronic disability. With the expansion of genetic testing, new etiologies for NDDs are continually uncovered, with as many as one third attributable to single-gene pathogenic variants. While our ability to identify pathogenic variants has continually improved, we have little understanding of the underlying cellular pathophysiology in the nervous system that results from these variants. We therefore integrated phenotypic information from subjec

Social phobias: behavioural and pharmacotherapy.

Five to 10% of the general population suffers from symptoms of an anxiety disorder. Social phobias, while less common than panic disorder, agoraphobia or simple phobias, are just as debilitating. Patients present with somatic, behavioural, mood and cognitive disturbances, of which unrecognized social isolation, depression, loss of employment, and drug and alcohol abuse can be the result. A symptomatic approach can be implemented through the use of education, insight, support, behaviour therapy, cognitive therapy, and pharmacological intervention.

Cognitive enhancement through transcranial infrared laser stimulation: Insights from our research.

Photobiomodulation (PBM), particularly transcranial infrared laser stimulation (TILS), has emerged as a promising non-invasive method to enhance cognitive functions in healthy individuals and those with neuropsychiatric or neurodevelopmental disorders. Conventional pharmacotherapy often fails to improve cognition effectively and can produce adverse effects. In contrast, TILS of the prefrontal cortex modulates brain energy metabolism through mechanisms like mitochondrial upregulation, improved cerebral oxygenation, and enhanced neuroplasticity. This chapter synthesizes findings and insights fro

The Contribution of Anxiety Sensitivity to Obsessive-Compulsive and Anxiety Symptoms in a Naturalistic Treatment Setting.

Anxiety sensitivity, a fear of arousal-related sensations, is prevalent in a number of disorders. We examined the relationship between internalizing symptoms and the anxiety sensitivity components of physical, social, and cognitive concerns in a sample of 165 patients seeking treatment in a clinic specializing in CBT for anxiety-related disorders. Social anxiety symptoms were associated with greater social concerns. Cognitive concerns were characteristic of both depression and generalized anxiety symptoms, suggesting these two classes of symptoms may share AS symptomatology. Physical concerns

Impulsiveness in obsessive-compulsive disorder: results from a family study.

OBJECTIVE: Although obsessive-compulsive disorder (OCD) is usually conceptualized as an anxiety disorder some studies suggested it to be a deficit of impulse control. The purpose of this study was to assess impulsiveness in OCD families and compare it to control families. METHOD: Seventy cases and their 139 relatives were compared with 70 controls and their 134 relatives from a German family study on OCD (German Epidemiologic Network for OCD Studies). All subjects were interviewed and diagnosed according DSM-IV criteria and were administered the Barratt Impulsiveness Scale (BIS) and PADUA-Inv

Cognitive impairment in the lateralized phenotype of corticobasal degeneration.

Corticobasal degeneration (CBD) is a rare disorder, which normally includes a combination of neurobehavioural features, movement disorders and other manifestations. It is now recognized that CBD patients usually present with two phenotypes: the lateralized phenotype and the dementia phenotype. The aim of our work was to determine the nature and the progression of cognitive and behavioural impairment in 10 lateralized CBD patients. In our patients, the most salient aspects of cognitive impairment were: an evident alteration of rapid alternating operative strategies, associated with the evident

Response inhibition deficits in obsessive-compulsive disorder.

Difficulty inhibiting irrelevant information may play a central role in the aetiology of obsessive-compulsive disorder (OCD). The aim of the present study was to determine whether OCD subjects (n=20) exhibit deficits in behavioural and cognitive inhibition compared with a clinical control group diagnosed with panic disorder (n=20). All subjects were administered a Go/Nogo task (a measure of behavioural inhibition) and a Stroop test (a measure of cognitive inhibition). OCD subjects made more commission errors on the Go/Nogo task, and they made more errors and displayed longer reaction times on

Markedly restricted life space due to long-term persistence of cerebellar cognitive affective syndrome following cerebellar hemorrhage: a case report.

BACKGROUND: The long-term effects of Cerebellar Cognitive Affective Syndrome (CCAS) after cerebellar stroke on daily life remain unclear. Life space, evaluated using Life Space Assessment (LSA), reflects mobility, social participation, and independence, providing real-world insight into CCAS. CASE DESCRIPTION: We report the case of a 68-year-old Japanese woman with normal premorbid cognition and independence who underwent emergent craniotomy for a right cerebellar hemorrhage involving lobules VII - VIII and the dentate nucleus. Initially, the patient exhibited severe ataxia (Scale for the Ass

Cognitive, behavioral and psychiatric symptoms in patients with spinal cord injury: a scoping review.

Spinal Cord Injury (SCI) is a condition where the spinal cord is damaged and experiences partial or complete loss of motor and/or sensory function, which is typically less than normal. After SCI, patients may exhibit more severe psychiatric symptoms and experience cognitive impairments, including reduced speed and attention processing capacity, as well as difficulties with executive function and episodic memory retention. Among the behavioral and psychiatric symptoms, depression, anxiety, substance use disorder, and posttraumatic stress disorder are the most common. This review aims to investi

Long-term epigenetic changes in offspring mice exposed to alcohol during gestation and lactation.

BACKGROUND: Alcohol exposure impairs brain development and leads to a range of behavioural and cognitive dysfunctions, termed as foetal alcohol spectrum disorders. Although different mechanisms have been proposed to participate in foetal alcohol spectrum disorders, the molecular insights of such effects are still uncertain. Using a mouse model of foetal alcohol spectrum disorder, we have previously shown that maternal binge-like alcohol drinking causes persistent effects on motor, cognitive and emotional-related behaviours associated with neuroimmune dysfunctions. AIMS: In this study, we soug

Cerebellar disorders: clinical/radiologic findings and modern imaging tools.

Cerebellar disorders, also called cerebellar ataxias, comprise a large group of sporadic and genetic diseases. Their core clinical features include impaired control of coordination and gait, as well as cognitive/behavioral deficits usually not detectable by a standard neurologic examination and therefore often overlooked. Two forms of cognitive/behavioral syndromes are now well identified: (1) the cerebellar cognitive affective syndrome, which combines an impairment of executive functions, including planning and working memory, deficits in visuospatial skills, linguistic deficiencies such as a

Abnormal activation of the occipital lobes during emotion picture processing in major depressive disorder patients.

A large number of studies have demonstrated that depression patients have cognitive dysfunction. With recently developed brain functional imaging, studies have focused on changes in brain function to investigate cognitive changes. However, there is still controversy regarding abnormalities in brain functions or correlation between cognitive impairment and brain function changes. Thus, it is important to design an emotion-related task for research into brain function changes. We selected positive, neutral, and negative pictures from the International Affective Picture System. Patients with majo

A cross-sectional study on clinical correlates of anxiety disorders in 613 community living older adults in Hong Kong.

OBJECTIVES: Previous studies have reported that the prevalence of late life anxiety disorders varies between 3.2% and 15.3%. Despite its high prevalence, anxiety disorders are still under-recognized in old age. This study evaluated the association among health, psychosocial factors, lifestyle choices, cognitive function and anxiety disorders in the older population in Hong Kong. METHODS: Six hundred and thirteen volunteer participants aged 60 years or above were interviewed for their socio-demographic background, physical and mental health statuses, social network, cognitive function and thei

Response to antidepressant medications in late-life depression across the spectrum of cognitive functioning.

OBJECTIVE: Late-life depression frequently co-occurs with cognitive impairment. To inform clinical management of these conditions, we examined the hypotheses that, relative to cognitively normal elders meeting DSM-IV criteria for major depressive disorder, those with cognitive impairment would require greater intensity of pharmacotherapy to reach criteria for antidepressant response and would take longer to respond. METHOD: Using data from the MTLD-3 study, we conducted a series of secondary analyses examining the implications of cognitive impairment for short-term, open-trial pharmacotherapy

Copy Number Variant Architecture of Child Psychopathology and Cognitive Development in the ABCD Study.

OBJECTIVE: Late childhood is a crucial period for individuals with psychiatric disorders. While common single-nucleotide polymorphisms explain a large proportion of inherited risk, structural variations including copy number variants (CNVs) play a significant role in the genetic architecture of neurodevelopmental disorders. The relevance of CNVs to child psychopathology and cognitive function in the general population remains underexplored. The authors conducted a comprehensive exploration of the CNV architecture underlying dimensions of psychopathology and cognitive phenotypes within the Adol

Psychiatric-onset prodromal dementia with lewy bodies: a comparative analysis of two cases.

Psychiatric-onset DLB refers to a prodromal presentation of DLB in which late-life psychiatric symptoms precede the characteristic cognitive and neurological features of DLB, often leading to an initial diagnosis of a primary psychiatric disorder. We report two women with psychiatric-onset DLB presentations who were followed longitudinally at a single institution. Case 1 presented with late-onset severe depression and anxiety, together with clinically suspected rapid eye movement sleep behavior disorder (RBD). Cognitive function was preserved, and dopamine transporter scintigraphy showed reduc
