Safety of antiadhesion barriers in hand surgery.

INTRODUCTION: Adhesions are a major cause of complications and need for repeat surgery in the hand and wrist. They are indication for tenolysis and cause prolonged need for therapy and inability to regain full hand motion. Minimizing adhesions in the hand and wrist would greatly improve surgery, by decreasing cost as well as improved outcome and postoperative function. Antiadhesion barriers, products consisting of hyaluronic acid and methylcellulose were originally advocated for its use in the peritoneal cavity for prevention of adhesions-initially by gynecologist, then general, trauma, and co

Incisional hernia in patients at risk: can it be prevented?

INTRODUCTION: Incisional hernia is a long-term complication of laparotomy. Its exact frequency varies according to different authors, but is always around 10-15%. There are patients who present with systemic associated diseases [chronic obstructive pulmonary disease (COPD), obesity, severe cardiopathies, immunodeficiencies, etc.] that favour or increase the risk of appearance of an incisional hernia. The aim of the present study was to assess whether the prophylactic placement of a polypropylene mesh in patients at risk can reduce or avoid the appearance of an incisional hernia. MATERIALS AND

Intestinal anastomosis by use of the biofragmentable anastomotic ring: is it safe and efficacious in emergency operations as well?

PURPOSE: Although sutureless anastomosis by use of the biofragmentable anastomotic ring is now accepted as an alternative to conventional manual sutured or stapled methods in elective enterocolic surgery, its applicability to emergency enterocolic surgery has not yet been established. The aim of this prospective study was to determine whether the biofragmentable anastomotic ring anastomosis in emergency enterocolic surgery could be performed as safely as in elective surgery or as emergency handsewn anastomosis. METHODS: To evaluate the safety and efficacy of sutureless bowel anastomosis by us

Pathologic complete response after first-line tislelizumab, cetuximab, and dabrafenib in microsatellite-stable BRAF V600E-mutant metastatic colorectal cancer: a case report.

BRAF V600E-mutant metastatic colorectal cancer (mCRC) is characterized by aggressive disease behavior and limited responsiveness to conventional systemic therapy. Although BRAF/EGFR-targeted therapies have improved clinical outcomes, durable complete remission remains uncommon, particularly in microsatellite-stable (MSS) disease. We report the case of a 26-year-old woman with MSS, BRAF V600E-mutant sigmoid colon adenocarcinoma and synchronous liver and omental metastases (cT4aN2M1c, stage IVC). Pretreatment biopsy demonstrated high programmed death-ligand 1 (PD-L1) expression (combined positiv

Gut microbiome in surgical oncology: mechanisms, perioperative outcomes, and therapeutic opportunities.

BACKGROUND: The gut microbiome is a fundamental determinant of gastrointestinal physiology. It is essential in maintaining host homeostasis … fits, as demonstrated by perioperative synbiotics reducing infectious complications by up to 45% in colorectal surgery and 64% in major liver surgery. Furthermore, preoperative oral care reduces post-oesophagectomy pneumonia by up to 50%, while Helicobacter pylori eradication halves metachronous gastric cancer risk. However, a detrimental 'antibiotic paradox' exists …

The oncovascular surgeon in modern surgical oncology: A literature review.

BACKGROUND: Involvement of major vascular structures by malignant tumors has historically been considered a contraindication to curative surgical resection. Advances in vascular reconstruction, perioperative management, and multimodal oncologic therapy have challenged this paradigm. Oncovascular surgery has emerged as a specialized field integrating radical oncologic resection with arterial and/or venous reconstruction, thereby expanding surgical resectability. Despite increasing evidence, the role of the oncovascular surgeon remains variably defined. METHODS: A narrative literature review wa

The association between radiographic osteosarcopenia and overall mortality among older patients undergoing colorectal cancer resection.

BACKGROUND: Osteosarcopenia based on computed tomography (CT) is gaining attention as a predictor of adverse outcomes in older adults with cancer. AIMS: We examined whether preoperative radiographic osteosarcopenia was associated with overall mortality (OM) among older patients with colorectal cancer (CRC). METHODS: Data of older patients (aged ≥ 60 years) who had undergone CRC surgery between October 2018 and February 2024 were retrospectively analyzed. A preoperative abdominal CT scan was used to assess skeletal muscle index (SMI) (cm2/m2) and vertebral bone density (VBD) in Hounsfield units

Identification of a Novel AT1R-Targeted Near-Infrared Fluorescent Probe for Visualization and Imaging-Guided Surgery in Pancreatic Cancer.

Despite an evolving therapeutic landscape, pancreatic ductal adenocarcinoma (PDAC) remains associated with a poor prognosis and high mortality rates, primarily owing to its aggressive nature and high recurrence rates. Near-infrared fluorescence (NIRF)-guided surgery is a gold standard curative treatment for PDAC; however, its effectiveness is significantly limited by the scarcity of available PDAC-targeted NIRF probes. In this study, we developed a novel NIRF molecular probe, MPA-GGG-Ang-df, by conjugating the dye MPA and evaluated its targeting ability toward the angiotensin II type 1 recepto

Comparative Outcomes of Loop Ileostomy and Loop Transverse Colostomy Following Curative Resection for Rectal Cancer.

INTRODUCTION: Temporary diversion of the fecal stream with a loop stoma is commonly performed to safeguard against an anastomotic failure and cause further complications. Commonly used stomas include loop ileostomy (LI) and loop transverse colostomy (LTC). Both of these techniques have their own complication profiles. This study highlights the comparison of patient outcomes and quality of life in these two stoma groups following curative resection for rectal cancer. METHODS: This randomized comparative study was carried out in the Colorectal Surgery Unit of Alexandria University Hospitals, Al…

Distinct Risk Profiles for Intraoperative Blood Loss and Perioperative Transfusion in Colorectal Cancer: A Comparative Predictive Modelling Study.

BACKGROUND: Significant intraoperative blood loss (SIBL) and perioperative transfusion (PTF) are major concerns in colorectal cancer (CRC) surgery, yet they are often considered sequential events despite potentially distinct mechanisms. METHODS: We retrospectively analysed 706 patients who underwent curative resection for non-metastatic CRC. Logistic LASSO regression was used to identify predictors for SIBL (> 200 mL) and PTF. Model performance was evaluated using area under the curve (AUC), calibration metrics, and 1000 bootstrap validation. RESULTS: SIBL occurred in 18.1% of patients, and

Recurrence Patterns of Resected Ampullary Adenocarcinoma and Implications for Multimodal Therapy: A Hidden Genome-Derived Analysis from the Ampullary Cancer Collaborative.

BACKGROUND: Ampullary adenocarcinoma is characterized by significant clinical heterogeneity that may preclude rational application of adjuvant therapies. The Hidden Genome Classifier defines distinct prognostic subtypes of ampullary adenocarcinoma. The impact of genomic subtype on recurrence patterns is unknown and may provide novel biomarkers to optimize treatment selection. METHODS: The previously validated Hidden Genome Classifier was applied to an international, six-institution cohort of 218 patients with resected ampullary adenocarcinoma. Genomic classification was correlated with patter

Enhanced Recovery After Liver Surgery: Does Compliance Impact Survival?

BACKGROUND: Enhanced recovery programs (ERPs) improve short-term outcomes after liver surgery (LS), but their impact on survival remains uncertain. This study evaluated the association between ERP compliance and survival in a large multicenter cohort. METHODS: This prospective European multicenter cohort included adults undergoing elective LS between 2016 and 2024 in 10 centers implementing Enhanced Recovery After Surgery (ERAS)-consistent ERPs. Compliance was defined as the proportion of 21 fulfilled items with high compliance (≥70%). A 1:1 propensity score-matching was performed to balance

Colorectal cancer survival in patients without curative measures: a retrospective cohort study.

BACKGROUND: A substantial proportion of colorectal cancer (CRC) patients are managed without curative intent due to frailty, comorbidity, or patient choice. Outcomes in this heterogeneous population remain incompletely characterised. We evaluated survival and treatment patterns in patients managed non-curatively, including exploratory analyses by tumour sidedness and metastatic status. METHODS: We conducted a retrospective cohort study of patients diagnosed with CRC at King's College Hospital, London, between 2013 and 2022. Patients with histologically or radiologically confirmed CRC discusse

Prognostic importance of tumour deposits with and without concurrent lymph node metastases in patients with stage III colon cancer.

BACKGROUND: Tumour deposits (TD) are associated with poor prognosis in patients with colon cancer. However, the current staging system only acknowledges TD in patients without lymph node metastases (LNM). This study investigated the prognostic impact of TD with or without LNM in patients with stage III colon cancer. METHODS: This was a national retrospective cohort study using the Danish Colorectal Cancer Group database, including patients with stage III colon cancer who underwent curative surgery between 2017 and 2022. Patients were stratified into 3 groups: TD alone; LNM alone; and TD and L

Identification of predictors for lymph node metastasis in T2 colorectal cancer: retrospective cohort study from a high-volume hospital.

BACKGROUND: Colorectal cancer (CRC) is the most prevalent malignant tumor of the digestive system globally, ranking third in incidence and second in mortality. In previous studies, the rate of lymph node metastasis (LNM) in T2 CRC ranged from 18.0 to 28.0%. We aim to identify T2 CRC patients without LNM and thereby mitigate the complications and potential impact on the quality of life associated with surgery. METHODS: In this retrospective study, 787 cases with T2 CRC were selected. The preoperative and postoperative clinicopathological features were retrospectively studied. Univariate analys
