A case is described of the rupture of a choledochal cyst in a 21-year-old man involved in a motor vehicle accident. At laparotomy, a haemoperitoneum was traced to a large haematoma in the lesser omentum which extended into the mesocolon and was found to be tinged with bile. Eventually it became apparent that the primary pathological condition was a large choledochal cyst into which opened the gallbladder. This cyst had split longitudinally down its anterior surface from the porta hepatis to the duodenum. Bile leakage was controlled by inserting a Foley catheter into the common hepatic duct, an

Scrapie, kuru, and Creutzfeldt-Jakob disease are characterized by a similar spongiform pathology, prolonged incubation periods, and an agent with unique physical, chemical, and biological properties. Swiss mice were inoculated with the scrapie agent and sacrificed three to five months later for light and electron microscopy. At three months, small vacuoles were seen within the neuropil of the cerebral cortex and basal ganglia. By the fifth month these vacuoles had increased in number and size and were accompanied by moderate astrocytic proliferation. The brainstem, cerebellum, and spinal cord

Intraductal papillary neoplasm of the bile duct (IPNB) is an uncommon biliary tumor with recognized malignant potential and a broad clinicopathological spectrum. Its diagnosis remains challenging, particularly in extrahepatic disease, due to limitations of cross-sectional imaging. We report a consecutive series of five patients diagnosed with IPNB in a tertiary hepatobiliary center, highlighting their clinical, radiologic, endoscopic, and pathological features. Most patients presented with obstructive jaundice or cholangitis, and lesions were predominantly located in the distal bile duct. Cros

Brain metastases constitute the most prevalent intracranial malignancies. Despite recent therapeutic advances, patient prognoses remain poor, largely due to significant treatment resistance and a highly restrictive tumor microenvironment. The rediscovery of meningeal lymphatic vessels (MLVs) has provided new insights into how the brain interacts with the immune system. MLVs are responsible for cerebrospinal fluid drainage, waste clearance, and immune cell trafficking, and their dysfunction is linked to many neurological diseases. In brain metastases, MLVs play a dynamic, dual role based on the

The triticeal cartilage is a small cartilage found within the thyrohyoid membrane present in approximately 33%-53% of individuals. Rarely, calcification/ossification of this cartilage can occur, leading to symptoms of dysphagia, odynophagia, or globus sensation. Despite its clinical relevance, symptomatic triticeal cartilage calcification remains poorly characterized in the literature, with only isolated case reports and no established diagnostic or management framework. We present a case of a 44-year-old woman who presented with seven months of atypical throat and neck pain and globus sensati

Appendiceal mucocele is a rare condition characterized by cystic distension of the appendix due to mucin accumulation, presenting a notable diagnostic dilemma that frequently mimics adnexal or ovarian masses in female patients. We describe a 25-year-old female who presented with right iliac fossa pain and was previously managed conservatively for an assumed appendiceal abscess, while concurrently undergoing active follow-up for a known ovarian cyst. Initial ultrasonography revealed a suspicious left adnexal mass, but subsequent computed tomography imaging demonstrated a fluid-filled, dilated a

Intussusception in adults is an uncommon cause of intestinal obstruction and is usually associated with an underlying structural lesion, unlike pediatric cases. In most adult patients, a pathological lead point is identified, frequently neoplastic in origin, making surgical management the standard treatment. We present the case of a 66-year-old man with a history of type 2 diabetes mellitus who presented with colicky abdominal pain, vomiting, and hematochezia. Physical examination revealed abdominal distension, tympany, diffuse tenderness, rebound tenderness, and hyperactive bowel sounds. Comp

Pathological cardiac hypertrophy is a critical factor leading to cardiomyopathy and ultimately heart failure. While several signaling pathways controlling cardiac hypertrophy have been identified, the molecular mechanisms underlying their precise regulation remain incompletely understood. Strip1, a structural component of STRIPAK complexes, has been implicated in various cellular functions; however, its role in cardiomyocytes is uncharacterized. Here we identify Strip1 as a potent anti-hypertrophic factor, controlling cell size and the hypertrophic gene program in neonatal rat ventricular card

Myopia is a growing public health problem expected to affect half the global population by 2050, making appropriate management of the condition necessary. Therefore, the purpose of this study was to investigate the knowledge and practices of South African optometrists regarding childhood myopia management. A quantitative, cross-sectional, descriptive design with a survey method was employed. An online questionnaire was distributed to practicing optometrists and included questions related to demographic information, knowledge of myopia and associated ocular pathologies, diagnostic work-up and m

Early pancreatic ductal adenocarcinoma (PDAC), including carcinoma in situ (CIS)/high-grade pancreatic intraepithelial neoplasia (PanIN) and very small invasive tumors, is increasingly recognized, but the significance of pancreatic parenchymal atrophy and delayed enhancement remains unsettled. This narrative review summarizes their pathological basis, technical determinants, observer variability, differential diagnoses, and evidence limitations across computed tomography (CT), magnetic resonance imaging (MRI)/magnetic resonance cholangiopancreatography (MRCP), endoscopic ultrasonography (EUS),

INTRODUCTION AND IMPORTANCE: The synchronous occurrence of multifocal pancreatic neuroendocrine tumors (NETs) with an intraductal papillary mucinous neoplasm (IPMN) is exceedingly rare, with limited cases documented. The biological relationship between these dual pathologies remains poorly understood.
CASE PRESENTATION: A 36-year-old man presented with epigastric pain. Imaging revealed a segmental cystic dilatation of the main pancreatic duct, consistent with an IPMN, but no discrete mass. Following left pancreatectomy, histopathological examination identified a low-grade pancreatobiliary-typ

BACKGROUND: Thoracic aortic aneurysm is an increasingly recognized condition that is sometimes diagnosed incidentally on imaging examinations performed to evaluate other unrelated conditions. Symptomatic presentations of thoracic aortic aneurysms may be due to mass effect on the airway, esophagus, recurrent laryngeal nerve and the thoracic spine. Alternatively, they may present with the dreaded complication of aortic dissection or rupture.Occasionally abnormalities of the aortic contour or size can be detected on routine chest x-ray. However, it is difficult to confidently diagnose thoracic ao

BACKGROUND AND OBJECTIVE: Gallbladder diseases present a critical challenge and can cause serious complications, if not diagnosed and treated promptly. Diseases including gallstone, inflammation, and other abnormalities may lead to a number of significant consequences, such as bile duct obstructions, chronic pain, infections, and in severe cases, life-threatening sepsis or gallbladder cancer.
METHODS: This study presents a novel deep learning-based diagnostic model using an attention-guided residual convolutional neural network to classify nine distinct gallbladder diseases, including gallsto

Dilated cardiomyopathy (DCM) is a group of heart muscle diseases that often lead to heart failure, with more than 50 causative genes have being linked to DCM. The heterogenous nature of the inherited DCMs suggest the need of precision medicine. Consistent with this emerging concept, transcriptome studies in human patients with DCM indicated distinct molecular signature for DCMs of different genetic etiology. To facilitate this line of research, we reviewed the status of transcriptome studies of inherited DCMs by focusing on three predominant DCM causative genes, TTN, LMNA, and BAG3. Besides st

Aortic aneurysms represent a very common pathology that can affect any segment of the aorta. These types of aneurysms can be localized on the thoracic segment or on the abdominal portion, with the latter being more frequent. Though there are similarities between thoracic and abdominal aortic aneurysms, these pathologies are distinct entities. In this article, we undertook a review regarding the different mechanisms that can lead to the development of aortic aneurysm, and we tried to identify the different manners of treatment. For a long time, aortic wall aneurysms may evolve in an asymptomati

Due to structural abnormalities in the leaflets, patients with bicuspid aortic valve (BAV) may develop isolated aortic valve disease, such as aortic regurgitation, aortic stenosis, or a combination of both. In addition to valvular pathology, numerous studies have indicated that approximately 40% of BAV patients exhibit aortic pathologies characterized by aortic dilatation. According to guidelines for valvular diseases, patients with BAV who require surgical aortic valve replacement (SAVR) and have a diameter of the aortic sinuses or ascending aorta ≥4.5 cm are recommended to undergo concomitan

BACKGROUND: Acute pancreatitis could be an early symptom of pancreatic cancer. However, repeated pancreatitis caused by pancreatic cancer is very rare.
CASE PRESENTATION: A 69-year-old man was referred to our hospital with severe abdominal pain, and serial imaging studies showed acute distally localized pancreatitis with a pseudocyst. Although he had successful conservative medical treatment followed by discharge from the hospital, he was re-admitted with severe abdominal pain for recurrent distal pancreatitis with splenic artery aneurysm followed by its rupture. No pancreas mass was detected

BACKGROUND: The diagnosis and pathological evaluation of esophageal achalasia have been improved dramatically by the development of high-resolution manometry. It is currently known to be divided into three subtypes. However, the differences between subtypes in terms of esophageal clearance remain unclear.
AIMS: To compare the pathology of subtypes in patients with esophageal achalasia from the perspective of esophageal clearance.
METHODS: We classified the patients diagnosed with esophageal achalasia into three subtypes based on the high-resolution manometry findings and compared the patient

Intracranial aneurysms (IAs) are pathological dilatations affecting cerebral arteries, and their ruptures lead to devasting intracranial hemorrhages. Although the mechanisms underlying the IA formation and rupture are still unclear, some factors have been identified as critical in the control of the vascular remodeling pathways associated with aneurysms. In a preclinical model, we have previously proposed the implication of the vascular serine protease, the tissue-type plasminogen activator (tPA), as one of the key players in this pathology. Here, we provide insights into the mechanism by whic

Viral myocarditis (VMC) is an inflammatory heart condition which can induce dilated cardiomyopathy (DCM). However, molecular mechanisms underlying the progression of VMC into DCM remain exclusive. Here, we established mouse models of VMC and DCM by infecting male BALB/c mice with Coxsackievirus B3 (CVB3), and performed NMR-based metabonomic analyses of mouse sera. The mouse models covered three pathological stages including: acute VMC (aVMC), chronic VMC (cVMC) and DCM. We recorded 1D 1H-NMR spectra on serum samples and conducted multivariate statistical analysis on the NMR data. We found that
