Between 1952 and 1986 the mortality from malignant tumours of the head and neck in the German Federal Republic shows a varying picture: a decline of cancer of the lip, equivocal rates for cancer of the nose and ear, increases for cancer of the tongue and of the larynx, a marked increase for cancer of the floor of the mouth and of the hypopharynx, a lesser increase for cancer of the tonsillar area and other parts of the mouth, but almost no increase for cancer of the salivary glands. Acoustic emissions in the form of cubic difference tones (CDT's), 2f1-f2, were measured in the ear canals of gerbils and cats. The state of the cochlea was manipulated by means of acute exposure to noise and was monitored with the aid of the whole-nerve response to tone pips. A 11 year old boy presented with chronic otorrhea and hearing impairment. After repeated tympanorevision macroscopic findings suggested the presence of middle ear tuberculosis. Chest X-ray revealed unusual abnormalities which in conjunction with normal pulmonary function and overall well-being did not support concurrent pulmonary tuberculosis. However, microbiological investigations verified this diagnosis in both sites, and proper tuberculostatic treatment proved successful.

Use of a vascular pedicled temporoparietalis muscle flap resurfaced with a full-thickness skin graft from the opposite ear, instead of less-than-favorable skin coverage, is advocated to achieve more satisfactory aesthetic results in total ear reconstruction handicapped by the presence of scarred local skin. The technical feasibility of the procedure and the authors' experiences in the past 9 years are described.

Following superior cervical ganglionectomy the noradrenaline tissue levels in the base of the rabbit external ear started to decrease after about 12 hours. About 4 hours later noradrenaline started to decrease about simultaneously in the middle piece and the tip. Within 24 hours after ganglionectomy the noradrenaline decreased rapidly in all three pieces. To ascertain risk factors for persistent middle-ear effusions (PMEE), we interviewed the parents of two groups of children. The first consisted of 76 children with PMEE who were admitted to the hospital for tympanostomy-tube insertion. The second, a control group, consisted of 76 children admitted for other types of surgery, who were matched for age, sex, season, and surgical ward. The acoustic middle ear muscle reflex thresholds towards white noise and fast sequences of clicks were determined in 46 normal-hearing young adults using electronic averaging. For white noise the thresholds were normally distributed around an average value of 72.7 dB SPL. An unusual combination of first and second arch anomlies is presented. This has resulted in absent middle and external ears with gross naso- and oropharyngeal defects. A brief reference is made to the embryology of this region to explain the anatomical and functional defects. Vascular abnormality during the peri-gestational period is suggested as a possible cause and a conservative line of treatment has been followed with good result.
