First report of monomicrobial Candida parapsilosis necrotizing fasciitis.

Candida parapsilosis is an emerging pathogen worldwide. It commonly causes soft tissue infection; however, to our knowledge there has been no previous report of monomicrobial necrotizing soft tissue infection (NSTI) secondary to C. parapsilosis. We report the first case of NSTI caused by C. parapsilosis in an immunocompromised renal transplant patient, with the diagnosis proven both histologically and microbiologically. Our patient required aggressive surgical intervention and antifungal therapy, with postoperative survival at 90 days.

Cryptococcemia in primary HIV infection.

Opportunistic infections have been reported infrequently in primary HIV infection. We report a case of cryptococcemia in primary HIV infection. To our knowledge there has not been such a case reported. Our case highlights the need for clinicians to be wary of other opportunistic infections, including cryptococcosis, in primary HIV infection.

Prophylactic oral/topical non-absorbed antifungal agents to prevent invasive fungal infection in very low birth weight infants.

BACKGROUND: Invasive fungal infection is an important cause of mortality and morbidity in very preterm or very low birth weight infants. Uncertainty exists about the effect of prophylactic oral/topical non-absorbed antifungals to reduce mucocutaneous colonisation and so limit the risk of invasive fungal infection in this population. OBJECTIVES: To assess the effect of prophylactic oral/topical non-absorbed antifungal therapy on the incidence of invasive fungal infection, mortality and morbidity in very preterm or very low birth weight infants. SEARCH METHODS: We used the standard search stra

Fungal infections in solid organ transplantation.

Renal, liver, heart and lung transplantation are now considered to be the standard therapeutic interventions in patients with end-stage organ failure. Infectious complications following transplantation are relatively common due to the transplant recipients overall immunosuppressed status. The incidence of invasive mycoses following solid organ transplant ranges from 5 to 42% depending on the organ transplanted. These mycoses are associated with high overall mortality rates. Candida and Aspergillus spp. produce most of these infections. This article will review the risk factors, clinical presen

Steroid responsive late deterioration in Cryptococcus neoformans variety gattii meningitis.

The authors describe the clinical course of Cryptococcus neoformans variety gattii infection in a young immunocompetent man who had a late deterioration characterized by headaches, subarachnoid inflammation, hydrocephalus, and stroke that reproducibly responded to steroids. These findings, in combination with declining markers of CSF infection, are consistent with the late deterioration being caused by sterile arachnoiditis rather than ongoing infection.

Persistently positive cultures and outcome in invasive neonatal candidiasis.

BACKGROUND: A persistently positive culture >24 h after starting antibiotic therapy has been correlated with adverse outcome in several invasive bacterial infections, but few reports address persistent positivity and outcome in infections caused by fungi and other pathogens that replicate more slowly and therefore may succumb less quickly to therapy. METHODS: To assess whether positive culture >24 h after achieving target doses (amphotericin > or =0.5 mg/kg/day or fluconazole > or =6 mg/kg/day) of systemic antifungal therapy predicts focal infectious complication(s) or death from infection, w…

Neutrophil phagocytosis in AIDS patients with azole resistant candidiasis.

OBJECTIVE AND SUBJECTS: This study aimed to evaluate phagocytosis of C. albicans by neutrophils in 10 AIDS patients and 50 control subjects. Five of the AIDS patients were colonised with azole-resistant C. albicans isolates and five with azole-sensitive isolates. RESULTS AND CONCLUSIONS: Percentage phagocytosis was within normal limits for seven of the 10 AIDS patients and was reduced in the remaining three patients. Phagocytosis was unaffected by the carriage of azole resistant C. albicans, and the patients' own strains were phagocytosed as readily as a standard strain of C. albicans. This s

Beyond Candida: Epidemiological Insights into Rare Yeast Fungaemia in Greece from a 15-Year Hospital-Based Study and Literature Review.

Invasive infections attributed to rare yeasts are increasingly recognized and often exhibit resistance to echinocandins or fluconazole. Despite geographical variability, epidemiological data from Greece are limited. To bridge this gap, we conducted a 15-year retrospective study of rare yeast fungaemia at a tertiary teaching hospital in Athens. All microbiologically confirmed cases in hospitalized patients (2010-2024) were included. Demographic and clinical data were retrieved from medical records. Incidence was calculated per 1000 admissions and 10,000 bed days. Isolates were identified using …

Candida (Candidozyma) auris in pediatric population: risk factors, clinical presentation, and outcomes.

INTRODUCTION: Candida (Candidozyma) auris, identified as a human pathogen in 2009, has emerged as a significant nosocomial fungus due to its … amphotericin B and micafungin being the most common. Among 33 clinical isolates, high resistance to fluconazole (60.6%) and variable susceptibility to amphotericin B were observed. Echinocandins, such as micafungin and anidulafungin, showed greater efficacy, although resistance to caspofungin has been reported. CONCLUSION: C. auris in pediatric patients repr …
