Assessment of Plantar Flap Coverage at Various Foot Amputation Levels: A Cadaveric Study.

BACKGROUND: From traumatic injury to diabetic foot infections, minor amputations of the foot and ankle rely on viable and adequate soft-tissue coverage. When employing a plantar foot flap, coverage often determines amputation level. This study examined the relative plantar foot flap length needed to cover various levels of minor foot amputations. METHODS: Demographic data and plantar flap length of ten cadaver specimens were recorded. The minimum plantar flap length needed to cover transmetatarsal, Lisfranc, and Chopart amputations was measured following each amputation. Absolute and percenta…

Foot tissue stress in chronic ankle instability during the stance phase of cutting.

Lower limb biomechanics of chronic ankle instability (CAI) individuals has been widely investigated, but few have evaluated the internal foot mechanics in CAI. This study evaluated bone and soft tissue stress in CAI contrasted with copers and non-injured participants during a cutting task. Integrating scanned 3D foot shapes and free-form deformation, sixty-six personalized finite element foot models were developed. Computed Achilles tendon forces and measured regional plantar pressure were applied as boundary loading conditions for simulation. It was observed that the primary group differences…

[Untreated arthritis urica can cause foot ulcers and amputations in patients with diabetes].

This case report describes an 88-year-old male with severe tophaceous gouty arthritis, diabetes, and kidney impairment. Over 20 years, the patient experienced recurrent gout attacks and developed tophaceous deposits, some of which ulcerated. Walking was severely impaired by foot deformities, pain, and chronic foot ulcers. Despite multiple consultations and radiological evidence of gout, no urate-lowering therapy was prescribed. Finally, microscopic analysis of foot ulcer debris identified urate crystals, confirming gout. The patient was then started on allopurinol therapy.
