OBJECTIVE: Whether external or plating fixation is more appropriate for high-energy tibial plateau fractures is still being disputed, our aim was to test the hypothesis whether external fixation can provide a fair outcome with fewer complications, when compared to the results with previously reported data of plating fixation for high-energy tibial plateau fractures.
METHODS: An Ovid of Medline, Embase, and Cochrane Library search was conducted for the relevant English orthopedic journals, and eligible studies, including twenty-four case series and one comparative study containing 885 patients

BACKGROUND: Most children with humeral supracondylar fractures can be treated with simple closed reduction and cross-fixation with Kirschner (K)-wires. However, in a small proportion of cases, an acceptable closed reduction cannot be obtained, and open reduction becomes necessary. An alternative to open reduction is the use of a temporary 3-mm K-wire to manipulate and reduce the distal fragment. This report introduces the method of manipulation, as well as indications, and draws comparisons with complete close reduction cases.
METHODS: Of 118 patients, 76 (64.4%) were managed with the standar

The aim of this study was to measure the intramedullary pressure (IP) of a) a reamed intramedullary nail (RN), b) an unreamed intramedullary nail (UN), and c) the AO-plate (PL) for stabilization of an experimental fracture of a sheep femur. The increase in IP whilst reaming or introducing intramedullary nails in long bone fractures is often said to be responsible for fat intravasation and secondary systemic and pulmonary damage after severe blunt trauma. 21 sheep were studied, 7 in each group (RN, UN, PL). The animal study was permitted by the local authorities. Under general anesthesia, the f

Scaphoid fractures are the most common fracture of the carpal bone and frequently occur in athletic populations due to acute trauma or repetitive wrist loading. Management of these injuries in athletes is challenging because of the need to balance reliable fracture healing with timely return to sport. This narrative review summarizes current literature regarding treatment strategies, healing outcomes, and return-to-sport timelines in athletes with scaphoid fractures. We conducted a literature search using PubMed, Scopus, and CINAHL between July 2025 and January 2026. A total of 862 records wer

Pilon tibial fractures are intra-articular fractures of the distal tibial metaphysis, typically resulting from high-energy axial trauma. Accurate classification using AO/OTA type 43-C and advanced imaging, including CT with 3D reconstruction, is essential for preoperative planning. Initial management of displaced fractures includes prompt reduction and temporary immobilization, often with a spanning external fixator. Definitive treatment follows a staged protocol based on soft tissue status and involves open reduction and internal fixation via approach-specific surgical access. Conservative ma

Low-velocity gunshot fractures of the forearm are complex injuries and previously published studies have not emphasized the problems particular to these forearm lesions. Of the twenty-nine patients in this series, thirteen had peripheral nerve injuries, three had impending Voklmann's ischemia, and ten had delayed union or malunion of fractures after treatment by closed methods. Only thirteen had none of these problems. Eight patients had long-term disability resulting either from permanent nerve damage with loss of sensation or weakness of grip, or from significant loss of motion following del

Twenty-nine fractures of the olecranon process of the ulna in horses treated at The Ohio State University College of Veterinary Medicine from 1973 through 1983 were evaluated. The objectives of the study were to determine the prognosis for various configurations of fractures of the olecranon process of the ulna, to determine what preoperative factors influenced the prognosis, and to evaluate the efficacy of the treatment methods used. The fractures were placed in one of 4 categories on the basis of severity of the injury: type-1 fractures involved the physeal plate; type-2 fractures involved t

High tibial osteotomy (HTO) using conventional surgical technique and cast immobilization was compared to HTO using an osteotomy jig, rigid internal fixation, and early motion. Fifteen patients (19 knees) had conventional HTO. The mean preoperative femoral-tibial angle was 2.5 degrees of varus, and the mean postoperative angle was 6.5 degrees of valgus. Two knees were undercorrected and eight knees (42%) had associated complications. Twenty patients (21 knees) had HTO utilizing the new surgical technique and postoperative management. The mean preoperative femoral-tibial angle was 2.3 degrees v

Multiple carpometacarpal (CMC) joint fracture dislocations involving the second to fifth rays are rare injuries, typically resulting from high-energy trauma. Due to the complex anatomy of the CMC joints and associated swelling, these lesions are frequently overlooked, leading to delayed diagnosis and suboptimal outcomes. We report the case of a 21-year-old male patient who sustained simultaneous dislocations of the second through fifth carpometacarpal joints with associated fractures following a high-energy trauma to the hand due to a motorcycle accident. Initial radiographic evaluation confir

CASE: Hardware complications in the perioperative period of a pilon fracture open reduction internal fixation or fusion procedure present a unique challenge. Revision of hardware can lead to additional complications. In this case report, we share a technique of in situ plate bending to manage damaged hardware in a patient who underwent a primary tibiotalar arthrodesis for a pilon fracture.
CONCLUSION: Preoperative planning, protection of the soft tissue envelope, and use of the "F" tool are keys to the success of this treatment method. Proper technique allows for a noninvasive method for corr

OBJECTIVE: Posterior wall (PW) fractures were sometimes associated in both-column acetabular fractures. How to evaluate pre-operatively the necessity for the performance of the posterior approach was an issue to be solved. In order to solve this issue, the computer-assisted virtual surgery technique was used to evaluate if the involved PW in both-column acetabular fractures (BACF) should be managed through posterior approach and verify the feasibility of this method.
METHODS: Data of a consecutive cohort of 72 patients with both-acetabular fractures from January 2012 to January 2020 was colle

PURPOSE: This study aimed to compare the biomechanical pull-out strength of the three different tension band wiring (TBW) methods employed to fix transverse olecranon fractures on bone models.
METHODS: Three different fixation models were created in groups of seven synthetic olecranon fractured bone models. The first technique was fixed the olecranon with the traditional TBW method. The second technique was fixed the olecranon with a large intramedullary screw TBW method. The third technique was fixed the olecranon with the double-screw TBW method. The pull-out force needed for the failure of
