Homelessness, drug use and hepatitis C: a complex problem explored within the context of social exclusion.

This commentary explores the relationships between homelessness, drug use and hepatitis C infection, within the broader context of social exclusion. Although it is difficult to estimate the number of homeless people who are misusing drugs or the number of drug users who are homeless, UK and international literature indicate a significant overlap between the two groups. Homeless drug users (HDUs) tend to experience many life problems and their situation has been described as a double jeopardy [Neale, J. (2001). Homelessness amongst drug users: A double jeopardy explored. The International Journ

Evidence-based recommendations for monitoring bone disease and the response to enzyme replacement therapy in Gaucher patients.

BACKGROUND: Bone disease is a serious complication of Gaucher disease. Untreated, it can result in pain, permanent bone damage and disability. Enzyme replacement therapy reverses many of the clinical signs of Gaucher bone disease but early assessment and treatment, and regular monitoring, are essential in optimising outcomes. SCOPE: In September 2005, a group of European experts met to review current knowledge and identify best practice and unmet needs in the monitoring of Gaucher bone disease and the response to enzyme replacement therapy. METHODS: Medline searches of peer-reviewed literatu

A simple nurse-based prompt increases screening and prevention counseling for diabetes.

OBJECTIVE: To determine the impact of a simple nurse-based prompt on fasting glucose screening and counseling regarding diet, exercise and weight loss to persons at increased risk for type 2 diabetes. RESEARCH DESIGN AND METHODS: Patients at risk for diabetes were recruited from 10 primary care practices. Nurses were trained to score a diabetes risk assessment and prompt providers concerning all high-risk subjects. Both univariate and multivariate logistic regression models were used to determine the association between the nurse prompt and subsequent fasting glucose testing or receiving advi

Medical and surgical therapies for alopecias in black women.

Hair loss is a common problem that challenges the patient and clinician with a host of cosmetic, psychological and medical issues. Alopecia occurs in both men and women, and in all racial and ethnic populations, but the etiology varies considerably from group to group. In black women, many forms of alopecia are associated with hair-care practices (e.g., traction alopecia, trichorrhexis nodosa, and central centrifugal cicatricial alopecia). The use of thermal or chemical hair straightening, and hair braiding or weaving are examples of styling techniques that place African American women at high

Evidence-based management of upper respiratory infection in a family practice teaching clinic.

BACKGROUND AND OBJECTIVES: Management of upper respiratory infection (URI) was examined in a family practice clinic to determine evidence-based practices, specifically for medication choice. Scientific evidence supports the use of decongestants and perhaps decongestant/antihistamine combinations in adolescents and adults and antipyretics in all age groups. The use of cold preparations for children younger than age 5 is not evidence based. METHODS: Data on demographics, medications prescribed, and over-the-counter medications recommended were collected from patient charts for 293 URI visits ov

Transfer Hydrogenation of Nitrones to Hydroxylamine Derivatives with PhSiH3.

Herein, we report a practical metal- and additive-free method for transfer hydrogenation of nitrones, employing a readily available PhSiH3 as the hydrogen donor. The findings demonstrated the effective conversion of a wide range of ketone- and aldehyde-derived nitrones into their corresponding hydroxylamine derivatives in good to excellent yields under mild conditions. The reaction is readily scalable and demonstrates broad functional group tolerance, accommodating bromo, chloro, fluoro, trifluoromethyl, thioether, thiophene, and furan substituents. Hence, the proposed operationally simple pro

Strategies for preventing sharps injuries in the operating room.

With the discovery of AIDS and HIV, the medical community began to widely recognize the dangers of serious illnesses spread-ing through contact with contaminated blood and body fluids. In response, the Centers for Disease Control and other groups have developed guidelines for the operating room to prevent the spread of infection from, for example, accidental needle sticks. Unfortunately, those guidelines are not always strictly followed. This article reviews studies that have examined precautionary practices, including such practices as double gloving, the use of blunt suture needles, and the

Urine screening practices in methadone maintenance clinics. A survey of how the results are used.

The urinalysis practices of 324 methadone maintenance clinics were surveyed using a brief self-report questionnaire. Results indicate that there is wide variability in collection practices and clinic responses to positive findings. Virtually all clinics provide counseling and revocation of take-home methadone doses as a response to positive urinalysis results. However, increase in urine screening frequency, methadone dose adjustments, contingency contracting, group therapy, and eventual discharge are interventions also used. The implementation of different interventions varied as a function of

The role of age and AMH on cumulative live birth rates over multiple frozen-thawed embryo transfer cycles: a study based on low prognosis patients of POSEIDON 3 and 4 groups.

BACKGROUND: Among the POSEIDON criteria, group 3 and group 4 have an expected low prognosis. For those patients with inadequate ovary reserve, embryo accumulated from consecutive oocyte retrieval cycles for multiple frozen-thawed embryo transfers (FET) has become more common. It is necessary to inform them of the pregnancy outcomes after single or multiple FET cycles before the treatment. However few studies about cumulative live birth rate (CLBR) for those with low prognosis have been reported. METHODS: This retrospective study included 4712 patients undergoing frozen embryo transfer cycles

The role of sphenoid bone in basilar invagination pathophysiology.

Basilar invagination (BI) is characterized by rostral dislocation of the cervical spine toward the skull base. The craniometrics of the skull base have shown significant differences among craniocervical junction malformations. The sphenoid bone is the center of the skull base; however, no study has evaluated this bone in cases of BI. This was a cross-sectional study of MRI databanks from two institutions of the author's practice between 1985 and 2020. The craniometrics of the sphenoid bone were measured in BI patients and controls. Fifty-eight MRIs were selected, including 28 BI patients and 3

Rhodium-catalyzed directed C-H cyanation of arenes with N-cyano-N-phenyl-p-toluenesulfonamide.

A Rh-catalyzed directed C-H cyanation reaction was developed for the first time as a practical method for the synthesis of aromatic nitriles. N-Cyano-N-phenyl-p-toluenesulfonamide, a user-friendly cyanation reagent, was used in the transformation. Many different directing groups can be used in this C-H cyanation process, and the reaction tolerates a variety of synthetically important functional groups.

[Requirements for the keeping of dangerous exotic animals].

The problem of dangerous dogs receives a lot of public attention. However, there is another group of animals that can threaten public security--the group of dangerous exotic animals. In daily routine mainly venomous snakes, spiders and scorpions or crocodiles, giant snakes and snapping turtles are of practical importance. The paper gives hints how to keep these animals according to animal protection and public safety rules.

Nurses' Practices in the Peripheral Intravenous Catheterization of Adult Oncology Patients: A Mix-Method Study.

A significant number of adult oncology patients require at least one peripheral intravenous catheter to fulfill their therapeutic plan. Recent evidence indicates that catheter failure rates are high in this cohort, impacting care outcomes and patient experience during cancer treatment. This reality represents a challenge to nurses worldwide since in most international settings they are responsible for delivering quality care during the insertion and maintenance of such devices. This study aims to explore current nursing practices regarding the insertion, maintenance, and surveillance of periph
