Prevalence of secondary hypertension in a population sample of Swedish women.

In a comprehensive population study of 1462 women aged 38--60 in Göteborg, Sweden, the prevalence of secondary hypertension was estimated to be 0.8% of the total population sample and 4.6% of those who were hypertensive (defined as systolic blood pressure greater than or equal to 160 mmHg or diastolic blood pressure greater than or equal to 95 mmHg or both or antihypertensive treatment irrespective of blood pressure levels). These figures were based on thorough clinical and laboratory investigations and a follow-up period of 12 years. Women with arterial hypertension reported a history of toxa

The salivary sodium/potassium ratio in hypertension: relation to race and plasma renin activity.

We have studied the relationship between plasma renin activity (PRA) and the salivary Na:K ratio, an index of mineralocorticoid effect, in 223 patients with essential hypertension. In 24 white patients with low PRA the median Na:K ratio was 0.74, which was significantly lower than the ratio of 1.40 in 54 normal white subjects (P less than .005) and the ratio of 1.10 in 34 white hypertensive patients with normal PRA (P less than .005). The Na:K ratio in 71 black patients with low PRA was 1.06, which was not significantly lower than the ratio of 1.50 in 38 black normal subjects or the ratio of 1

Adrenocortical factors in hypertension. I. Significance of 18-hydroxy-11-deoxycorticosterone.

Low renin essential hypertension and the syndrome of mineralocorticoid excess have two features in common, low plasma renin activity and volume-sensitive hypertension. The proposal that both disorders share a common mechanism--because of the ability of agents that inhibit or antagonize the adrenocortical secretion to lower blood pressure in the low renin hypertensive group--appears to be based on a circular argument. The beneficial effect of removal or neutralization of the adrenocortical contribution only constitutes evidence for volume-dependency or sensitivity, which is how the low renin gr

Association between plasma aldosterone after saline infusion test and nocturnal hypertension in patients with suspected primary aldosteronism.

BACKGROUND: Nocturnal hypertension (NH) is a significant risk factor for target organ damage and all-cause mortality. Post-saline infusion test plasma aldosterone concentration (post-SIT PAC) serves as an indicator of autonomous aldosterone secretion level, but its link to NH is uncertain. This study aims to explore the association between post-SIT PAC levels and NH in patients with suspected primary aldosteronism (PA). METHODS: Participants were classified into 3 groups after a saline infusion test (SIT): negative (post-SIT PAC < 5 ng/dL, n = 86), borderline (post-SIT PAC 5-10 ng/dL, n = 180

Spectrum of Primary Aldosteronism and Risk of Cardiovascular Outcomes: The Atherosclerosis Risk in Communities Study.

IMPORTANCE: Mounting evidence suggests that renin-independent aldosteronism is common and often underrecognized. Yet, whether aldosteronism across this broader spectrum is associated with incident cardiovascular disease (CVD) has not, to the authors' knowledge, been comprehensively evaluated. OBJECTIVE: To determine whether aldosterone measures are associated with incident CVD events in community-dwelling older adults. DESIGN, SETTING, AND PARTICIPANTS: This prospective cohort analysis included participants from the Atherosclerosis Risk in Communities (ARIC) study with serum aldosterone and

[Characteristics of cardiovascular and cerebrovascular diseases in patients with primary aldosteronism: a propensity score matching study].

Objective: To analyze the characteristics of cardiovascular and cerebrovascular diseases in patients with primary aldosteronism (PA) and compare them with age-, sex-, and blood pressure-matched patients with essential hypertension (EH). Methods: This cross-sectional study was conducted at the West China Hospital of Sichuan University. Patients with PA and EH between December 2018 and December 2021 were included. Demographic characteristics, stroke events, coronary artery diseases, peripheral vascular diseases, and cardiac structure and function indicators were analyzed. Propensity score matchi

Molecular Imaging Versus Adrenal Vein Sampling for the Detection of Surgically Curable Primary Aldosteronism : A Prospective Within-Patient Trial.

BACKGROUND: Primary aldosteronism (PA) causes hypertension and is potentially surgically curable when it is caused by a unilateral aldosterone-producing adrenal adenoma (APA). Adrenal vein sampling (AVS) is required to guide surgery, but it is invasive, is technically difficult, and has limited availability. OBJECTIVE: To determine whether the accuracy of post-dexamethasone [11C]metomidate ([11C]MTO) positron emission tomography-computed tomography, a diagnostic test for APAs, is superior or noninferior to the accuracy of AVS in predicting outcomes from unilateral adrenalectomy, and whether [

CXCR4-directed PET with 68Ga-pentixafor versus adrenal vein sampling for the diagnosis of unilateral primary aldosteronism.

OBJECTIVE: To explore the accuracy of unilateral primary aldosteronism (UPA) classification via adrenal vein sampling (AVS) and C-X-C chemoking receptor 4 (CXCR4)-directed positron emission tomography (PET) with 68Ga-pentixafor (CXCR4-directed molecular imaging). METHODS: A retrospective cohort study was conducted with 89 patients who were diagnosed with UPA and who underwent unilateral adrenalectomy (ADX) at West China Hospital of Sichuan University from January 2021 to June 2023. For these patients, surgical decisions were made on the basis of either AVS (AVS group) or CXCR4-directed molecu

Establishment and evaluation of a patient-derived organoids-based xenograft model of primary aldosteronism using [18F]AlF-NOTA-pentixather PET/CT: bridging preclinical and clinical imaging.

BACKGROUND: Primary aldosteronism (PA) is a leading cause of secondary hypertension, but the lack of representative preclinical models hampers translational research and imaging evaluation. We aimed to establish a patient-derived organoids-based xenograft (PDOX) model of PA and to assess its imaging characteristics using [18F]AlF-NOTA-pentixather PET/CT, with validation in clinical patients. METHODS: Adrenal tissues from patients with PA were used to generate adrenal organoids in vitro. After serial passaging, these organoids were implanted subcutaneously into immunodeficient mice to establis

Elevated plasma aldosterone concentrations relate to renal impairment in Chinese southwestern people.

BACKGROUND: Evidence on the biochemical phenotypes of primary aldosteronism and their link to renal function in Chinese populations is limited. This study explored the associations of plasma aldosterone concentration (PAC), plasma renin concentration (PRC), and the aldosterone-to-renin ratio (ARR) with renal function. METHODS: In this cross-sectional study of 1700 participants from Southwest China, associations between PAC, PRC, ARR, and renal parameters (serum creatinine [SCR], blood urea nitrogen [BUN], estimated glomerular filtration rate [eGFR]) were analyzed using a generalized linear mi

Rare correlation of somatic PRKACA mutations with pregnancy-associated aldosterone- and cortisol-producing adenomas: a case report and literature review.

BACKGROUND: Somatic mutations have been observed to induce aldosterone-producing adenomas (APAs). These may be accelerated during pregnancy. Somatic PRKACA mutations are common in cortisol-producing adenomas (CPAs). However, their role in APAs, particularly aldosterone- and cortisol-producing adenomas (A/CPAs), is not well understood. This study aims to investigate the association between PRKACA mutations and the accelerated development of A/CPAs during pregnancy. CASE PRESENTATION: A patient with primary aldosteronism (PA) associated with severe Cushing's syndrome (CS) underwent surgical res
