Development and validation of an explainable prediction model for schistosomiasis seropositivity: a population-based screening study in Hunan Province, China.

Early identification of people at risk of schistosomiasis infection is critical to interrupting disease transmission. We develop and validate an explainable machine learning prediction model that integrates demographic, behavioral, and environmental factors to identify these individuals. A total of 103,707 individuals were included to train and internally validate the model, and 16,574 individuals were used for external validation. The Random Forest (RF) model demonstrated the best discriminative performance among the five machine learning models evaluated. It accurately predicted schistosomia

Health System Strategies to Improve the Uptake of Cervical Cancer Screening in Sub-Saharan Africa: A Scoping Systematic Review.

Globally, Sub-Saharan Africa (SSA) has the highest rate of cervical cancer incidence and mortality. Cervical cancer screening (CCS) is essential for identifying and treating pre-cancer and early-stage cancer. However, CCS rates throughout SSA are substantially low due to a multitude of health system barriers, including poor access to CCS services and limited skilled providers and or resources. The aim of this scoping systematic review was to identify evidence-based health system-level strategies addressing barriers to implementing CCS in SSA. A systematic literature search was conducted across

HIV screening and associated factors among adolescent girls and young women in Tanzania: analysis of the 2022 demographic and health survey data.

BACKGROUND: Human immunodeficiency virus (HIV) infection remains a significant global challenge, particularly in resource-limited settings where access to healthcare services is constrained. Despite substantial efforts to combat HIV, the uptake of screening among adolescent girls and young women (AGYW) remains sub-optimal. This study aimed to determine the prevalence of HIV screening and identify the factors associated with screening uptake among AGYW in Tanzania. METHODS: This was a cross-sectional analysis of the 2022 Tanzania Demographic and Health Survey (2022 TDHS). The study population

Brazilian guidelines for the management of tuberculosis infection in immune-mediated inflammatory diseases: is retesting in latent tuberculosis screening appropriate and Safe?

The article "Brazilian Recommendations for the Management of Tuberculosis Infection in Immune-Mediated Inflammatory Diseases" by Viviane de Souza et al. presents important discussions on the subject; however, the recommendation for annual repetition of the TST/IGRA test for three years and after medication changes raises considerable concerns. This approach may lead to overdiagnosis and overtreatment of latent tuberculosis infection (LTBI). Frequent false-positive results in retests can result in unnecessary antibiotic use, contributing to bacterial resistance, a problem of global significance

Projecting the Impact and Costs of Near Point-of-Care Tuberculosis Screening Assays in Community-based Active Case Finding.

BACKGROUND: Community-based active case finding (ACF) may reduce tuberculosis incidence and mortality but is not widely implemented due to resource constraints. How novel screening test characteristics would affect the impact and costs of ACF strategies is not clear. METHODS: We developed mathematical models of tuberculosis natural history and transmission, calibrated to data from 3 high tuberculosis burden countries (India, Vietnam, and the Philippines) and simulated the impact of annual community-based ACF programs. We compared annual screening, with or without an initial symptom screen, fo

Study rationale and design of the PEOPLHE trial.

PURPOSE: Lung cancer screening (LCS) by low-dose computed tomography (LDCT) demonstrated a 20-40% reduction in lung cancer mortality. National stakeholders and international scientific societies are increasingly endorsing LCS programs, but translating their benefits into practice is rather challenging. The "Model for Optimized Implementation of Early Lung Cancer Detection: Prospective Evaluation Of Preventive Lung HEalth" (PEOPLHE) is an Italian multicentric LCS program aiming at testing LCS feasibility and implementation within the national healthcare system. PEOPLHE is intended to assess (i)

Health Catch-UP!: a realist evaluation of an innovative multi-disease screening and vaccination tool in UK primary care for at-risk migrant patients.

BACKGROUND: Migrants to the UK face disproportionate risk of infections, non-communicable diseases, and under-immunisation compounded by healthcare access barriers. Current UK migrant screening strategies are unstandardised with poor implementation and low uptake. Health Catch-UP! is a collaboratively produced digital clinical decision support system that applies current guidelines (UKHSA and NICE) to provide primary care professionals with individualised multi-disease screening (7 infectious diseases/blood-borne viruses, 3 chronic parasitic infections, 3 non-communicable disease or risk facto

What rate of Iranian women perform Pap smear test? Results from a meta-analysis.

In this study, an attempt was made to estimate the aggregated proportion of Pap smear test uptake among women in Iran from 2012 to 2022 using meta-analysis. The data collection process involved reviewing records registered in databases between January 1, 2012, and September 11, 2022. The final data was analyzed using random effects models, and potential heterogeneity was assessed using the I2 index. To examine publication bias, Egger's test and Funnel Plot were employed. Sensitivity analysis, specifically single study exclusion Meta-analysis, was conducted to assess the influence of each indiv

Can we screen for heart disease in children at public health centres? A multicentre observational study of screening for heart disease with a risk of sudden death in children.

Sudden cardiac death in children is a rare event, but of great social significance. Generally, it is related to heart disease with a risk of sudden cardiac death (SCD), which may occur with cardiovascular symptoms and/or electrocardiographic markers; thus, a primary care paediatrician (PCP) could detect them. Therefore, we proposed a study that assesses how to put into practice and conduct a cardiovascular assessment within the routine healthy-child check-ups at six and twelve years of age; that reflects cardiovascular signs and symptoms, as well as the electrocardiographic alterations that ch

Efficient Screening in Obstructive Sleep Apnea Using Sequential Machine Learning Models, Questionnaires, and Pulse Oximetry Signals: Mixed Methods Study.

BACKGROUND: Obstructive sleep apnea (OSA) is a prevalent sleep disorder characterized by frequent pauses or shallow breathing during sleep. Polysomnography, the gold standard for OSA assessment, is time consuming and labor intensive, thus limiting diagnostic efficiency. OBJECTIVE: This study aims to develop 2 sequential machine learning models to efficiently screen and differentiate OSA. METHODS: We used 2 datasets comprising 8444 cases from the Sleep Heart Health Study (SHHS) and 1229 cases from Taipei Veterans General Hospital (TVGH). A Study of the Psychometric Properties of the Pediatric Symptom Checklist-17 for Children With Developmental Delays and Disorders.

Primary care physicians (PCPs) frequently serve pediatric patients with developmental delays and disorders (DD/D). Although the most widely used primary care behavioral health screener, the Pediatric Symptom Checklist-17 (PSC-17), is validated for use with children without DD/D, it is unclear whether this measure accurately identifies behavioral health symptoms in youth with DD/D. Thus, the purpose of this study was to assess the psychometric properties of the PSC-17 for children with DD/D. Medical record data from 3596 pediatric patients at a primary care clinic were analyzed. Descriptive ana

Speckle-Tracking Echocardiography as an Effective Screening Tool for Cardiac Involvement Among Patients With Systemic Sarcoidosis in an Indian Cohort: A Prospective Observational Study.

INTRODUCTION: Diagnosing cardiac sarcoidosis (CS) is challenging due to the lack of a sensitive gold standard diagnostic test. Although advanced imaging techniques like cardiac magnetic resonance imaging (MRI) (cardiovascular magnetic resonance [CMR]) and fluorodeoxyglucose positron emission tomography (FDG-PET) CT are promising, they are limited by their availability and cost. Two-dimensional speckle-tracking echocardiography (2D-STE) is emerging as a valuable tool for the early detection of CS. METHODS: This single-center observational study assessed cardiac involvement and the utility of S
