Imaging in plasma cell disorders-consensus recommendations of the Asian myeloma network bone imaging workgroup.

The role of imaging in accurate classification and management of plasma cell disorders is substantial with the increasing evidence in the ability of cross-sectional imaging to identify disease related manifestations. International myeloma working group and nation-wide guidelines provide recommendations for guiding practice. However, there are remarkable variations in practice globally and in particular, adoption of incorrect imaging techniques due to lack of awareness/education, lack of equipment or personnel and resource constraints. These limitations are not specific to any particular geogra

COVID-19 Factors Associated With Medication Changes Among Nursing Home Residents With Dementia.

This study examined associations of COVID-19 mortality rates, staffing, and resident behavior with changes in antidementia and psychotropic medication initiation among nursing home (NH) residents with dementia. A nationally representative survey of Directors of Nursing was analyzed to assess changes in medication initiation at the peak of the pandemic. NHs with higher COVID-19 mortality rates were less likely to report increases in antidementia medication initiation. COVID-19 mortality rates were not associated with significant increases or decreases in psychotropic initiation. NH's that repor

Diabetes management during the end of life.

For individuals with diabetes, maintaining optimal glycaemic control is essential to reduce the risk of long-term complications. However, as patients approach the end of life, the emphasis on tight glycaemic targets becomes less relevant. Instead, the primary goal shifts to maintaining blood glucose levels within a range that minimises the risk of hypoglycaemia and prevents symptomatic hyperglycaemia, thereby ensuring comfort and quality of life. This article explores the transition from disease-focused management to a person-centred approach grounded in comfort, dignity and ethical responsibi

Behavioral Challenges and Adaptive Strategies in Providing Mealtime Care to Residents With Dementia: A Qualitative Study of Nursing Home Clinical and Administrative Staff.

This study examined how nursing home staff adapt to challenging mealtime behaviors in residents with dementia, such as turning away or refusing food. Ten focus groups were conducted with 52 clinical and administrative staff from three southeastern U.S. nursing homes. Transcripts were analyzed using four key codes: feeding behaviors, interventions, training, and communication. Staff described a range of mealtime challenges, including cognitive and functional decline, dysphagia, and elopement. Feeding-related behaviors like refusing to eat reflect the complexity of care required. Staff reported

Moisture-Associated Skin Damage: Expanding and Updating Practice Based on the Newest ICD-10-CM Codes.

Moisture-associated skin damage (MASD) occurs when skin is repeatedly exposed to various sources of bodily secretions or effluents, often leading to irritant contact dermatitis with inflammation, with or without denudation of affected skin. In 2020, the Wound, Ostomy and Continence Nurses Society took an initiative that led to the addition of multiple International Classification for Diseases codes for irritant contract dermatitis caused by various forms of MASD for use in the United States (ICD-10-CM). In the last issue of the Journal of Wound, Ostomy and Continence Nursing, a clinical practi

A Clinical Review of Primary Biliary Cholangitis.

Primary biliary cholangitis is a slowly progressive immune-mediated cholestatic disease that causes a destruction of the intrahepatic bile ducts and may lead to cirrhosis of the liver, end-stage liver disease, and the need for liver transplantation. The disease is among the most common reasons why adults require liver transplantation. The primary signs of the disease include the presence of antimitochondrial and antinuclear antibodies, elevated alkaline phosphatase, hyperbilirubinemia, hypercholesterolemia, and histologic features, such as intense inflammation with a florid duct lesion and hep

Fake bus stops for persons with dementia? On truth and benevolent lies in public health.

Fake bus stops are one strategy to keep persons with dementia (PwD) from wandering. By setting up authentic looking shelters and benches in hallways or gardens, nursing homes create the illusion of bus stops, for the purpose of preventing wandering PwD from leaving the facility and getting lost. By attracting wandering PwD to sit down and wait for the bus, they can be supervised more easily by caregivers. However, concerns are expressed that the use of fake bus stops could cause more harm than good for PwD, due to their deceptive nature and the potential stigmatisation of individuals seated at

Burden and challenges of heart failure in patients with chronic kidney disease. A call to action.

Patients with the dual burden of chronic kidney disease (CKD) and chronic congestive heart failure (HF) experience unacceptably high rates of symptom load, hospitalization, and mortality. Currently, concerted efforts to identify, prevent and treat HF in CKD patients are lacking at the institutional level, with emphasis still being placed on individual specialty views on this topic. The authors of this review paper endorse the need for a dedicated cardiorenal interdisciplinary team that includes nephrologists and renal nurses and jointly manages appropriate clinical interventions across the inp

Constant Observation Practices for Hospitalized Persons With Dementia: A Survey Study.

Despite substantial staffing and cost implications, the use of constant observation (CO) has been poorly described in the acute care setting. The purpose of this cross-sectional, multicenter, survey study was to assess hospital provider practices regarding the use of CO. Of the 543 surveys distributed, 231 were completed across 5 sites. Most respondents worked on medical units (67.5%), as nurses (49.1%); 44.8% were white; and 75.6% were female. The majority (84.2%) reported at least 1 patient/wk requiring CO. Most frequent indication for CO was dementia with agitation (60.7%), in patients olde

Simulation-Based Interprofessional Education: A Nursing and Social Work Collaboration.

BACKGROUND: Contemporary teaching and learning pedagogy commands interprofessional collaboration among allied professions such as nursing and social work, two professions that have a natural inclination to partner in the workforce. METHOD: Nursing and social work students participated in a structured simulated learning experience where they demonstrated their respective professional practice skills in a supported learning environment while working collaboratively to assess one of two patient types: high-fidelity or simulated. RESULTS: Both groups expressed initial worry during prebriefing bu

Recognition and management of sepsis in early infancy.

Serious bacterial and viral infections occurring in early infancy are important factors affecting morbidity and mortality. Recognition of sepsis by healthcare professionals is vital and allows initiation of appropriate early treatment which can be life-saving and improve outcomes for these infants greatly. This article outlines the leading causes of sepsis, for example, group B Streptococcus (GBS), Escherichia coli and herpes simplex virus. We discuss the non-specific ways in which an infant with a serious infection may present to health professionals and highlight crucial features to look for

Pharmacogenetics: a reality or misplaced optimism?

The paper aims to review current evidence that supports the application of genetic information in the management and use of psychotropic medication. Although the importance of an individual's genetic makeup in the metabolism of drugs has been known for at least 50 years, it is only recently that such information is finding clinical application. A literature review of recent studies suggest that there are clear variations in the way people respond to psychotropic medication. These variations can be seen across racial and ethnic lines, and are genetically determined. The hope is that, in future

Knowledge, beliefs, and practices regarding exclusive breastfeeding of infants younger than 6 months in Mozambique: a qualitative study.

Only 37% of infants younger than 6 months in Mozambique are exclusively breastfed. A qualitative assessment was undertaken to identify the knowledge, beliefs, and practices around exclusive breastfeeding--specifically, those of mothers, fathers, grandmothers, and nurses--and to identify the support networks. Results show many barriers. In addition to receiving breast milk, infants receive water, traditional medicines, and porridges before 6 months of age. Many mothers had heard of the recommendation to exclusively breastfeed for 6 months. However, other family decision makers had heard less ab

Incontinence-associated dermatitis in patients with spinal cord injury.

Evidence-based patient care for those with urinary and faecal incontinence involves routine tasks that are integral to essential patient care. However, over the past few decades, researchers have demonstrated how ritualistic practice in this area has become. There is also a growing range of skin care products that can be used to prevent incontinence-associated dermatitis and for nurses, deciding which ones to use can be problematic Incontinent patients have a 22% higher risk of developing pressure ulcers - when immobile this risk increases to 30% - and the often indiscriminate use of various l
