Effect of systemic steroid therapy in Graves' orbitopathy on regulatory T cells and Th17/Treg ratio.

PURPOSE: Glucocorticoids are a mainstay treatment for Graves' orbitopathy, yet their exact mechanisms of action remain unclear. We aimed to determine whether the therapeutic effects of systemic steroid therapy in Graves' orbitopathy are mediated by changes in regulatory T lymphocytes (Tregs) and T helper 17 lymphocytes (Th17). METHODS: We assessed Treg and Th17 levels in the peripheral blood of 32 patients with active, moderate-to-severe Graves' orbitopathy who received 12 weekly pulses of methylprednisolone, and determined their association with disease severity, disease activity, and treatm

Bleb Revision Using an L-shaped Conjunctival Flap Posterior to the Limbus in Eyes With Bleb Failure After PreserFlo MicroShunt Implantation.

Bleb fibrosis is a major cause of early surgical failure after PreserFlo MicroShunt implantation. Although needling is commonly attempted as first-line management, its effectiveness may be limited in cases with dense Tenon's encapsulation. The optimal surgical approach for open revision has not been well defined. This case series presents four consecutive eyes with bleb failure after PreserFlo MicroShunt implantation treated with revision using a fornix-based L-shaped conjunctival flap posterior to the limbus technique. Intraocular pressure (IOP), best-corrected visual acuity, corneal endothel

From Fusarium solani Keratitis to Full Visual Acuity: A Case of Infection Requiring Multiple Corneal Transplants.

Background and Clinical Significance: The Fusarium solani species complex is a group of filamentous fungi capable of causing severe ocular infections. It is a frequent cause of mycotic keratitis, which is associated with a poor prognosis due to aggressive biological behavior and resistance to most antifungal agents. Contact lens wear is a well-known risk factor; Case Presentation: A 48-year-old female contact lens wearer presented with a persistent corneal ulcer and hypopyon in the right eye. Initial visual acuity was hand movements. Hyphae of the Fusarium solani species complex were detected

Corneal Perforation Two Years After Mitomycin Intravascular Chemoembolization.

Mitomycin intravascular chemoembolization (MICE) is a potential treatment for corneal neovascularization (NV) with encouraging early outcomes. We describe a case of focal corneal perforation occurring two years after MICE. A 65-year-old man with a history of penetrating keratoplasty performed 30 years earlier underwent MICE for treatment of corneal NV. Two years later, he presented with acute visual decline in the left eye. At presentation, the visual acuity was 20/200 improving to 20/50 with pinhole in the right eye and counting fingers improving to 20/400 in the left eye. Examination reveale

A comparative study of mini-monovision, crossed mini-monovision, and emmetropia with enhanced monofocal intraocular lenses.

We compared the visual performance and subjective outcomes of mini-monovision, crossed mini-monovision, and bilateral emmetropia using enhanced monofocal intraocular lenses (IOLs). This retrospective study involved 200 eyes of 100 patients who underwent surgery for bilateral age-related cataract using an enhanced monofocal IOL (TECNIS Eyhance). The dominant eye was identified before surgery. Based on patients' preferences, they were divided into mini-monovision (dominant eye for distance and non-dominant eye for near with 1.0 D anisometropia), crossed mini-monovision (dominant eye for near and

Report of a Rare Syndromic Retinal Dystrophy: Asphyxiating Thoracic Dystrophy (Jeune Syndrome).

Jeune syndrome (JS), first described by Jeune as asphyxiating thoracic dystrophy, is an autosomal recessive osteochondrodysplasia with characteristic skeletal abnormalities and variable renal, hepatic, pancreatic, and ocular complications. Approximately 1 in every 100,000 to 130,000 babies is born with JS. Most patients with JS have respiratory distress due to inadequate lung development and many lose their lives due to respiratory failure. Those who survive have serious comorbidities. In terms of ophthalmological diseases, JS is classified among the hereditary syndromic retinopathies. Most, i

Refractive surgical approaches to keratoconus: A systematic review and network meta-analysis.

Advancements in diagnostic methods and surgical techniques for keratoconus (KC) have increased non-invasive treatment options. Successful surgical planning for KC involves a combination of clinical science, empirical evidence, and surgical expertise. Assessment of disease progression is crucial, and halting the progression should be the focus if it is progressive. While surgeons used to rely on experience alone to decide the surgical method, comparing the network of primary factors, such as visual acuity, across studies can help them choose the most appropriate treatments for each patient and

Suboptimal visual outcome after femto-LASIK ablation in a high myopic patient.

A 28-year-old nurse had an aberration-free femto-laser in situ keratomileusis (LASIK) performed for her myopia of -6.25 -0.50 × 096 and -6.75 -0.50 × 175 in the right and left eye, respectively. Corrected distance visual acuity (CDVA) preoperatively was 20/16. Preoperatively, there were no abnormalities on Scheimpflug imaging, and a pachymetry of 585 μm was measured in both eyes. Flap thickness was 115 μm. The patient was quite nervous during the surgery. Since the surgery, her uncorrected distance visual acuity (UDVA) and CDVA are suboptimal at 20/30 and 20/20 in the right eye, and 20/20 and

Case Report: Ophthalmia Nodosa Caused by Caterpillar Setae.

Caterpillars can damage human health, including visual acuity, as a result of penetration of the setae into the eye or their layout on the eye tissue. However, the path of setae movement in the eye remains unclear. In this study, a 43-year-old woman was injured in the left eye by a caterpillar. Other than rubbing her eye, the patient did not attempt any type of treatment, and she visited our outpatient clinic 5 days after the accident. The patient's left eyelid was markedly swollen, with a visual acuity of 1.0 in the right eye and only 0.04 in the left eye. Images of the setae in the eye were
