Mangling upper limb injuries in industry.

The salvage of upper limbs mangled by industrial machinery became possible with the development of predictable techniques of vascular and microvascular anastamosis. Unfortunately, many of these salvaged limbs are associated with fair and poor functional outcomes. The quality of the skeletal fixation can have a substantial effect on the functional outcome and should be a major focus of the limb repair process. Internal plate fixation facilitates wound care and limb mobilization without tethering muscle-tendon units and is safe in the majority of severe upper limb injuries provided that all devi

Use of the free medialis pedis flap in orofacial reconstruction: report of two cases.

The medialis pedis flap is a fasciocutaneous flap based on a branch of the medial plantar artery. The authors have used this flap in the reconstruction of skin defects of the hands and fingers because its skin is very similar in texture, color, and thickness. Because of its characteristics, this flap can also be applicable for orofacial reconstruction, when small or thin skin coverage is needed. Two cases are reported in which free medialis pedis flaps were used to reconstruct the eye socket in one patient and the alveolar ridge in the other. The authors concluded that the skin of the medialis

Periocular skin reshaping by CO2 laser coagulation.

When blepharoplasty is performed by transconjunctival route, sometimes the excess of skin created by overlaxness of the lower lid does not retract to achieve the desired cosmetic result. We propose a method for controlled retraction of the eyelid by coagulating the skin using the CO2 laser. Thanks to this approach, the lower eyelid's tone recovers and the desired cosmetic appearance is achieved. Results of 37 patients that were treated by 10 W CO2 laser, 5-mm defocused beam diameter, and pulses of 100 ms, are presented. Follow-up shows that periocular skin reshaping by CO2 laser for laxened ey

Use of dermal-fat suspension flaps for thigh and buttock lifts.

A new technique for the correction of trochanteric bulges is described. A buried dermal-fat flap is used as a suspensory mechanism to hang the weight of the thigh skin and subcutaneous tissues on the deep fascia, rather than on the upper skin flap. This technique is particularly applicable to patients with asymmetry of the buttocks and thighs as well as those with ptosis of the buttocks. Another advantage is that it creates a new gluteal fold at a predetermined higher level.

Repair of a Large Ear Defect Including the Triangular Fossa.

Large ear defects in dermatologic surgery can present a difficult challenge to achieve excellent functional and cosmetic results. This article describes a difficult case of a large ear defect as well as considerations for resolution. A 67-year-old male presented for Mohs micrographic surgery of a basal carcinoma of the left ear. Prior skin biopsy showed a nodular and sclerosing basal cell carcinoma that extended to biopsy margins. The lesion was excised in 3 Mohs stages with negative margins in the last stage. Final defect size was 3.5×2.3 cm and extended across the scapha and the crura of the

The Expanding Role of Virtual Surgical Planning in Maxillofacial Trauma Management.

Advances in virtual surgical planning (VSP) and computer-aided design/computer-aided manufacturing continue to transform the management of craniomaxillofacial surgical care. There has been a demonstrable shift toward fully personalized surgical care in postablative head and neck reconstruction, orthognathic and temporomandibular joint surgeries, and in surgery for congenital and acquired anomalies. Surgical management of facial trauma exhibits timing and budgetary challenges that have delayed the adoption of VSP into common practice in the area. This review explores the scope, advantages, and

Customized Surgical Plates for Computer-Assisted Maxillary and Mandibular Reconstruction.

Jaw reconstruction after ablative surgeries poses significant challenges for maxillofacial reconstructive surgeons. Offering maximum functional rehabilitation, osseous free flaps have become the standard of care. The conventional approach involves shaping donor bone segments into the jaw configuration in a free-hand fashion, which can lead to unpredictable and suboptimal outcomes. Recent advancements in computer-assisted surgery, virtual surgical planning (VSP), and 3-dimensional printing technologies offer promising solutions to these challenges, enabling the fabrication of patient-specific s

Orbital Floor Reconstruction With Patient-Specific Implants: Comparable Outcomes Between Mirroring and Virtual Reduction Techniques.

BACKGROUND: Orbital floor fractures may cause enophthalmos, diplopia, and facial asymmetry, often requiring surgical intervention to restore form and function. Advances in virtual planning enable customized reconstruction; however, the influence of specific planning techniques on surgical accuracy remains uncertain. PURPOSE: This study aimed to compare the accuracy of patient-specific implant (PSI) placement, using 2 the virtual reconstruction techniques: mirroring and virtual fracture reduction. STUDY DESIGN, SETTING, AND SAMPLE: This retrospective cohort study was conducted at the Galilee

Keystone design perforator island flap for reconstruction of vermillion defects: A case series and literature review.

BACKGROUND: Many repair techniques have been recommended to treat lower lip defects. Although effective in many instances, some techniques are accompanied by complications, such as flap necrosis, dysesthesia of the lip, lip flattening, shortening, and loss of the vermillion roll. This study aimed to report the use of keystone design perforator island flap (KDPIF) in small to medium defects to achieve an effective and functional reconstruction of vermillion defects after Mohs surgery for lip cancer. METHODS: A retrospective review of consecutive patients who underwent lower lip reconstruction

The Cheek Interpolation Flap for Reconstruction of Auricular Mohs Defects.

BACKGROUND: Surgical reconstruction of auricular defects after Mohs surgery may prove challenging secondary to the complex topography of the ear and few options for local flap repair that do not introduce inordinate risk of anatomic distortion. The matched preauricular skin may be used to overcome these limitations. OBJECTIVE: This study aims to examine the utility, technique, and outcomes of preauricular interpolated flaps used for auricular reconstruction. To the authors' knowledge, this study represents the only published series of cheek interpolation flaps for auricular reconstruction. M

Uniportal thoracoscopic left main bronchus resection with new lobar carina reconstruction.

Isolated endobronchial tumors of the left main bronchus are quite rare. They are treated by surgical resection, preferably using lung-sparing techniques, when possible. These procedures are technically challenging and are usually performed through a standard thoracotomy.  This video tutorial shows a left main bronchus sleeve resection with distal lobar carina reconstruction, performed through a uniportal thoracoscopic approach, for a typical carcinoid tumor of the distal left main bronchus.

[Development of the facial feminization surgery patient's satisfaction questionnaire (QESFF1): Qualitative phase].

OBJECTIVE: Facial feminization surgery is becoming a more frequently requested procedure in transsexual male to female patients transformation. A global way of reporting outcomes data and showing the beneficial impact of this specific procedure is necessary. The objective of this study is to develop a reliable and valid tool to report patients' outcomes after facial feminization surgery. METHODS: A systematic literature review, input from experts working with transsexual patients and patient interviews were used to develop the conceptual framework of the questionnaire. It includes the outcome

Reconstructive Operation of Nonsyndromic Multiple-Suture Craniosynostosis Based on Precise Virtual Plan and Prefabricated Template.

This report presents a case of a 4-year-old girl suffering from multiple-suture nonsyndromic craniosynostosis. The patient is characterized with fronto-orbital malformation and poor visual acuity. The preoperational three-dimensional computed tomography scan showed premature closure of the sagittal suture, bilateral coronal, and lambdoidal suture. The virtual surgical planning was performed before operation for the reposition of bone segments and communication with the patient's family. The navigational plate for reshaping was made based on the surgical plan with three-dimensional printed mode
