Implementation of a territorial geriatric hotline in Southwestern France: a monocentric observational study to improve access to geriatric expertise.

BACKGROUND: France is experiencing rapid population aging, increasing demand for healthcare services and pressure on unscheduled care pathways for frail and multimorbid older adults. A significant proportion of transfers to acute care, including emergency departments, are considered potentially avoidable, particularly when timely access to geriatric expertise is lacking. In 2024, a territorial geriatric hotline was implemented at Toulouse University Hospital to provide community-based physicians with rapid access to geriatric advice and optimize patient care pathways. The purpose of this study

Adjusting the 15-method to Danish general practice: a participatory design approach.

BACKGROUND: The 15-method is a primary healthcare tool for opportunistic screening and brief intervention for alcohol-related problems. A Danish feasibility test of the 15-method suggested that adjustments might enhance its fit to Danish general practice. This study reports on user-involving workshops aimed at refining a Danish version of the 15-method. METHODS: Using a participatory design approach, we conducted iterative cycles of planning, user workshops, evaluating and revising design. Workshops engaged five general practitioners, three nurses, four patients, four researchers, a project m

Individualized pharmacotherapy: background and development.

Most drug prescriptions are still based on empiricism and not on solid biological data, which often results in considerable patient variability and, sometimes, low patient benefits. Although variability in patient response to pharmacotherapy has long been recognized, only in recent decades have new molecule analytical methods provided insight into some of the causes, which are often related to somatic or germline genetic variations. Based on this insight, different predictive biomarker tests have been developed to optimize and individualize pharmacotherapy. These biomarker tests are classified

A multicenter prospective observational cohort study to improve diagnostic yield and clinical management of persons with a Bleeding Disorder of Unknown Cause (BDUC-iN study protocol).

BACKGROUND: In half of individuals referred for evaluation of bleeding symptoms, no hemostatic defect is identified with current standard hemostasis tests. These individuals are classified as having a bleeding disorder of unknown cause (BDUC). The pathophysiological mechanisms underlying the bleeding tendency in BDUC are unknown. The absence of a clear, molecular diagnosis leads to significant challenges for healthcare professionals in terms of diagnostics and clinical management, as well as insecurity in patients and caregivers. OBJECTIVES: The BDUC in the Netherlands (BDUC-iN) study aims to

Music therapy preferences among breast cancer patients undergoing perioperative chemotherapy: from qualitative insight to quantitative evidence.

BACKGROUND: Patients with breast cancer (PWBC) have a high risk of developing psychological disorders, and interventions should be implemented with care. For music therapy (MT) interventions to be more effective among these patients, providing therapeutic programs tailored to their preferences is desirable. Given the limited research on the preferences for MT of PWBC undergoing chemotherapy, we had previously conducted a qualitative study to clarify their preferences, yielding the formation of related hypotheses. Building upon these hypotheses, the current quantitative study aimed to clarify t

Impact of non-steroidal anti-inflammatory drugs exposure on persistent cough after pulmonary resection in a Chinese tertiary hospital: a prospective cohort study protocol.

INTRODUCTION: Previous studies have pointed out that persistent cough is a common complication after pulmonary resection and its occurrence is closely related to inflammatory response. However, there are no clinical studies to date that directly verify that the use of non-steroidal anti-inflammatory drugs (NSAIDs) can reduce the incidence of persistent cough after pulmonary resection (CAP). In view of this, this study aimed to explore and confirm whether exposure to NSAIDs can effectively reduce the incidence of CAP through a prospective cohort study. METHODS AND ANALYSIS: We will conduct a s

Moderating the Relationship Between Ulcerative Colitis and Crohn's Disease and Symptoms of PTSD: The Role of Remission.

GOALS: This study examined whether inflammatory bowel disease (IBD) patients endorse clinically significant symptoms of post-traumatic stress disorder (PTSD), and tested whether remission status and remission expectations effectively moderate the relationship between endorsements of PTSD symptoms and aspects of IBD. BACKGROUND STUDY: The enduring somatic threat model speaks to the presentation of symptoms of trauma that result from ongoing somatic concerns rather than discrete external events. Literature shows patients living with acute conditions experience symptoms of PTSD; however, few stu

Non-coding RNAs, cancer treatment and cardiotoxicity: A triad of new hope.

The global health landscape has experienced a shift towards non-communicable diseases, with cardiovascular diseases and cancer as leading causes of mortality. Although advancements in healthcare have led to an increase in life expectancy, they have concurrently resulted in a greater burden of chronic health conditions. Unintended consequences of anticancer therapies on various tissues, particularly the cardiovascular system, contribute to elevated morbidity and mortality rates that are not directly attributable to cancer. Consequently, the field of cardio-oncology has emerged to address the pr

Identifying Non-Traditional Approaches to Swallowing Rehabilitation: A Scoping Review.

In recent years, dysphagia care has shifted toward a more patient-centered approach. This means that dysphagia clinicians are considering more factors in the treatment process including the goals of care, quality of life, and cultural values. The purpose of this scoping review was to examine relevant research and identify dysphagia interventions outside of those traditionally used by dysphagia clinicians that may improve swallowing function. A review of relevant studies was conducted using search terms related to swallowing and treatment. The search yielded 8439 unique studies. Of the 283 arti

Oropharyngeal dysphagia telerehabilitation in the Intensive Care Unit for COVID-19: a case report.

The face-to-face assessment of and training for dysphagia are considered aerosol-generating procedures, and thus are contraindicated for patients who are positive or suspected of having severe acute respiratory syndrome coronavirus 2 infection. Considering the extremely infectious nature of the virus, transmission to other individuals during rehabilitation is possible. Some patients in the intensive care unit and those who are on endotracheal intubation and mechanical ventilation often have dysphagia. Therefore, assessment and training for oropharyngeal dysphagia are provided by rehabilitation

Effectiveness of Advance Care Planning: What Works, What Doesn't, and What Needs to Change?

An increasing recognition over the past five decades of the importance of patients' autonomy and the right to be able to choose to limit medical treatment at the end of life has led to the development of a number of documents related to advance care planning, including the advance directive, medical power of attorney, and portable orders for life-sustaining treatment (POLST). While these documents are important aspects of advance care planning, without having goals-of-care conversations, a specific plan, and necessary supports to achieve these goals, the documents alone offer a false promise.

Mental Health of Physicians During COVID-19 Outbreak in Bangladesh: A Web-Based Cross-Sectional Survey.

There have been numerous studies about the health implication of COVID-19 on patients, but little attention has been paid to the impacts of the pandemic on physicians. Our paper attends to this gap by exploring the mental health of physicians in Bangladesh during the COVID-19 pandemic. This is particularly important since the mental health of physicians impacts not only on themselves, but also their professional performance and hence the care of patients. This study examined physicians' mental health outcomes by evaluating the prevalence and associated potential risk factors of anxiety and dep
