Depression in public community long-term care: implications for intervention development.

The objective of this paper is to increase understanding of geriatric depression in the public community long-term care system to guide intervention development. Protocols included screening 1,170 new clients of a public community long-term care agency and interviewing all clients with major, dysthymia, or subthreshold depression (n = 299) and a randomly selected subset of nondepressed older adults (n = 315) at baseline, 6-month, and 1 year. Six percent had major depression, one-half of a percent had dysthymia only, and another 19% had subthreshold depression. Over the year observation period,

[Post-stroke depression].

In recent years, post-stroke depression (PSD) has attracted a worldwide interest. This review addresses the major research issues of PSD. PSD is the most frequent psychic complication among stroke survivors. Depression has a significant impact on post-stroke recovery and mortality. The diagnosis of PSD is based on the structured mental state examination and DSM-IV criteria as well as on the scores of the clinical symptoms scale (the Hamilton Depression Rating Scale) and self-rated inventories. PSD is multifactorial in origin and develops under the influence of biopsychosocial factors. The anat

Frequency, phenomenology and anatomical-clinical correlates of major post-stroke depression.

BACKGROUND: The meaning of post-stroke depression is controversial. AIMS: To investigate the hypothesis that major post-stroke depression (PSD) may be due to organic factors (left frontal lesions) immediately after the stroke, but to psychosocial factors in later stages. METHOD: We studied 153 consecutive stroke patients, categorised on the basis of time elapsed since stroke, lesion location and presence/absence of major PSD. Fifty-eight were examined in the first two months following the stroke, 52 between two and four months, and 43 after four months or more. The symptom profiles and anato

Evidence against the existence of a temporal lobe epilepsy personality syndrome.

The existence and specificity of a characteristic behavioral syndrome among patients with temporal lobe epilepsy (TLE) remains controversial. The behavioral pattern of many epilepsy patients differs from that of age-, sex-, and socioeconomic-matched control subjects. Patients with TLE and other forms of epilepsy can experience changes in cognitive function, personality, affect, and drive-related behavior (e.g., libido, aggression). Biologic factors (e.g., underlying brain pathology, epileptogenic process, localization and lateralization, recurrent seizures, family history), antiepileptic drugs

Limb lengthening in Turner syndrome.

We report the results and complications of eight consecutive patients who underwent bilateral tibial lengthenings for dwarfism associated with Turner syndrome. Lengthening was performed via distraction osteogenesis with monolateral external fixation. Tibias were lengthened an average distance of 9.2 centimeters or 33 percent of the original tibial length. The average total treatment time was 268 days. The overall complication rate was 169 percent for each tibia lengthened and each segment required an average of 1.7 additional procedures. Seven cases (44 percent) required Achilles tendon length

A retrospective study of patient care needs on admission to an inpatient hospice facility.

BACKGROUND: The care and support of dying patients and their families are among the most important skills of a family physician. In this century, an increasing proportion of deaths have occurred in hospitals with resulting medicalization of the dying process. Hospice care has emerged to focus on the relief of suffering rather than the cure of illness. This descriptive study reports information about the diagnoses, care needs, and attending physicians of a cohort of patients admitted to a free-standing, inpatient hospice program. METHODS: We undertook a retrospective chart review of 335 patien

Quality of life and coping in patients awaiting heart transplantation.

The psychosocial adaptation of patients awaiting heart transplantation has not been defined. Forty-one patients (36 men, 5 women; mean age, 48 years) completed standardized questionnaires before transplantation to assess quality of life, physical symptoms, marital/social adjustment, psychiatric morbidity, coping, and compliance to medical regimens. Also, data were obtained from spouses/partners and the transplantation nurse coordinator. Unlike previously reported findings with patients after transplantation, those awaiting transplantation report moderate dissatisfaction with quality of life. P

Clinical care of the aging hypertensive patient.

That elderly hypertensive patients need to be treated is now well established, as major clinical trials have confirmed the reduction in serious complications and organ damage in the treated patient. However, the concomitant findings on the high rates of adverse side effects of drug therapies to lower blood pressure are of concern. The physiologic and disease changes that occur with aging may explain some of the adverse drug responses encountered in this population. Nondrug treatment of hypertension, including sodium restriction and weight reduction, would be desirable in the elderly, but there

A prospective longitudinal study of neuropsychological and psychosocial factors in asymptomatic individuals at risk for HTLV-III/LAV infection in a methadone program: preliminary findings.

To test the hypothesis that cognitive impairment may be present early in the course of HTLV-III/LAV infection, intravenous drug abusers (IDVAs) without overt symptoms of AIDS related illness were tested with standard neuropsychological and psychosocial measures. This study is the baseline for a prospective longitudinal study of the natural history of HTLV-III/LAV infection in this high risk population. Of 211 subjects initially evaluated, 70 (33%) were HTLV-III/LAV seropositive and 141 (67%) were seronegative. At the baseline, by univariate analysis, the seropositive IVDAs were significantly (

Intersectional stigma among LGBTQIA+ adolescents with chronic primary musculoskeletal pain and its association with functioning and depressive symptoms.

OBJECTIVE: Many adolescents with chronic primary musculoskeletal pain (CPMP) feel stigmatized by their pain condition. Studies have not previously evaluated whether having another stigmatized identity (i.e., intersectional stigma) contributes to poorer physical and psychosocial outcomes in this population. The current study evaluated the hypothesis that adolescents with CPMP identifying as having a minoritized sexual orientation and/or gender identity (lesbian, gay, bisexual, transgender, queer, intersex, and asexual [LGBTQIA+]) experience greater pain-related stigma, perceived injustice, func

Long-term quality of life and functional outcomes in extracorporeal membrane oxygenation survivors.

Extracorporeal membrane oxygenation (ECMO) has become an increasingly vital intervention for patients with severe cardiac and respiratory failure, significantly improving short-term survival rates. However, there is growing recognition of the importance of long-term outcomes, including health-related quality of life (HRQoL), functional recovery, and psychological well-being. This narrative review synthesizes clinical studies, systematic reviews, and cohort analyses from the past 20 years using PubMed and Google Scholar, focusing on adult ECMO survivors. Studies consistently demonstrate that al

Targeting symptoms of fibromyalgia through transcutaneous electrical nerve stimulation (TENS): a systematic review of current systematic reviews.

BACKGROUND: Fibromyalgia syndrome (FMS) is a chronic condition characterized by widespread musculoskeletal pain, fatigue, and cognitive dysfunction. Considering that current treatments often provide limited relief, interest has grown in non-pharmacological options such as transcutaneous electrical nerve stimulation (TENS). OBJECTIVE: To systematically and critically assess and synthesize current evidence from systematic reviews and meta-analyses on the efficacy and safety of TENS for managing symptoms of FMS. METHODS: An umbrella review was conducted in accordance with the PRISMA 2020 guidel

Health-related quality of life and predictors in differentiated thyroid cancer patients during the transitional period: A Cross-sectional study.

BACKGROUND: Despite the increasing prevalence of differentiated thyroid cancer (DTC) in mainland China, the health-related quality of life (HRQoL) of DTC patients remains suboptimal. The transitional period from hospital to home is crucial for patients to adapt to their new status as survivors. However, such patients usually face several challenges and their HRQoL remains poorly understood. Therefore, this study aimed to explore the HRQoL and predictors in Chinese DTC patients during the transitional period. METHODS: This single-center cross-sectional study including 300 patients was conducted

Oral Behaviors, Anxiety, and Depression in Temporomandibular Disorders: A Conceptual Narrative Review Within the DC/TMD Axis II Framework.

Background and Objectives: Temporomandibular disorders (TMDs) are heterogeneous conditions whose clinical expression cannot be fully explained by local or structural findings alone. DC/TMD Axis II provides a psychobehavioral framework for assessing pain, disability, jaw functional limitation, psychological symptoms, and oral behaviors. This conceptual narrative review aimed to clarify how oral behaviors, anxiety, and depressive symptoms can be clinically interpreted together within the DC/TMD Axis II framework. Materials and Methods: A structured search was conducted in PubMed/MEDLINE and Scop

Associations of AI-Based Facial Metrics With Patient-Reported Outcomes in Idiopathic Facial Paralysis.

OBJECTIVE: Conventional grading scales and static image assessments may not capture dynamic facial movement in facial paralysis. We developed a video-based, dynamic, artificial intelligence (AI) application, DynaFace, to objectively quantify facial metrics and determine which measures correspond with patient-reported appearance, function, and psychosocial outcomes, providing insight into how objective dynamics relate to subjective patient experience. METHODS: DynaFace automatically extracted dynamic facial metrics (facial asymmetry index [FAI], bilateral palpebral fissure, and oral commissure
