Gout is a mostly hereditary metabolic disease and is considered a disease of affluence. The disease is promoted by a purine-rich diet and sh … on in the infratemporal fossa, deriving from the temporomandibular joint, with arrosion of the bony skull base demonstrates gout as a relevant disease for the ENT clinician. Potential diagnostic difficulties as well as recommendations for a therapeutic regimen of gouty lesions in such critical localizations will be reviewed.

In immunocompromised patients, symptoms and the pathogen spectrum of sinusitis are frequently atypical. If progressive loss of vision occurs, an infection of the anterior skull base or nasal sinuses should be considered. We report on four patients with orbit-associated symptoms. CT-imaging revealed bony defects in sinus borders to orbits or endocranium. In all the cases immediate surgical drainage was performed because complications following sinusitis were suspected. Histopathological diagnosis revealed two cases of aspergillosis and mucormycosis. The possibility of opportunistic infections b…

PURPOSE: Penetrating sphenoid sinus injuries in both war and civilian practice are rare. A review of the literature shows no reports of shell fragments or bullets in this region; hence, the rarity of the cases prompted this report.
MATERIALS AND METHODS: A variety of surgical approaches--transethmoidal, transnasal trans-septal, sublabial transnasal, and endoscopic techniques--and the feasibility, efficacy, and safety of these techniques used in the delivery and management of missiles lodged in the sphenoid sinus under conditions of war are described.
RESULTS: Four shell fragments and 2 bulle

We present a case of traumatic pseudoaneurysm of the internal carotid artery (ICA) accompanied by skull base fracture, which was treated by endovascular trapping of the internal carotid artery. A 70-year-old woman met with a traffic accident and was admitted to our institution with epistaxis and accomapanied with shock. Angiography on day 2 showed carotidcavernous fistula. The patient had no eye symptom and was treated conservatively. A second angiography, two weeks aftrer the trauma, revealed development of a pseudoaneurysm on the C3-4 portion. We attempted balloon test occlusion (BTO) of the…

INTRODUCTION: The craniovertebral junction (CVJ) comprises the occiput, atlas, and axis. Radiographic evaluation of this region involves knowledge of only a few anatomical landmarks, as well as basic normal measurements and relationships to perform CVJ craniometry. Occipital bone anomalies and atlanto-occipital non-segmentation typically produce basilar invagination. Atlas anomalies predominantly involve the posterior arch, while the os odontoideum accounts for the majority of axis anomalies.
RESULTS AND DISCUSSION: A number of syndromes are associated with CVJ pathology, the most notable bei

BACKGROUND: Meningiomas involving the jugular foramen and parapharyngeal space are extremely rare. They most commonly occur intracranially and then extend to the extracranial region through the foramen of the skull base, such as jugular foramen. Clinically, these tumors mimic the more common glomus jugulare tumor. Preoperative diagnosis can be correct on the basis of the characteristic imaging findings.
METHODS: A 52-year-old woman was seen with a left neck mass and mixed-type hearing loss. She underwent physical examination, MRI, high-resolution CT, and angiography.
RESULTS: Physical examin…

Fine-needle aspiration (FNA) has become the predominant initial diagnostic technique for lesions at a number of sites within the head and neck. The technique has achieved this level of usage because it is relatively inexpensive, rapid to perform, well accepted by patients, associated with low morbidity, and has a relatively high diagnostic accuracy. Sites in the head and neck amenable to FNA include the thyroid, cervical masses and nodules, salivary glands, intraoral lesions and lesions in the paraspinal area and base of skull. Diagnostic accuracy is dependent on the site of aspiration as well

Intracranial chondromas are unusual tumors, which most commonly arise from cartilage rets in the synchondrosis at the base of the skull. They are most likely found in the sellar and parasellar regions, usually located extradurally. In rare instances, these tumors originate from the dura mater of the convexity. In these cases their neuroradiological features may mimic other intracranial tumors. We present the case of a 50-year-old male presenting a large parasagittal tumor originating in the frontal convexity. MRI-scans revealed a cavitated tumor strongly enhanced after gadolinium infusion mimi

To review the presentation and management of nasal and other facial dermoid cysts and sinuses (excluding external angular dermoids). We report on 28 patients with less common facial dermoids. They presented as a cyst, sinus or both and may be separated into four groups. Group 1 (18 cases) were in the midline on the nose and many (12) had extensions to the septum but only two to the skull base. Group 2 (two cases) were paramedian nasal lesions and one extended through the nasal bones. Group 3 were lesions around the medial orbital wall, and two of these had tracts extending into the orbit. Grou…

Hemangiopericytomas are malignant tumors arising from pericytic cells and account for less than 1% of all vascular neoplasms. We report a rare case of an extra- and intracranial dumbbell-shaped hemangiopericytoma originating from the soft tissue of the neck and penetrating the skull base with invasion into the posterior cranial fossa. The 59-year-old female patient presented with a large pulsating neck mass and reported weakness, abnormal fatigue and headache. MRI revealed an inhomogeneously enhancing tumor and cerebral angiography showed intensive vascularization. Preoperative embolization wa…

BACKGROUND: The maxillary swing procedure has been used as an anterolateral approach to expose the nasopharynx, the central skull base, and its vicinity. The reported incidence of postoperative palatal fistula has ranged from 20% to 25%. The oronasal incompetence especially associated with a large fistula has adversely affected normal speech, eating, and swallowing functions. We describe a modified palatal incision to reduce the incidence of palatal fistula associated with the maxillary swing procedure.
METHODS: Fifteen consecutive patients who underwent maxillary swing procedures for salvage…

PURPOSE: To describe three cases of frontal osteoma which were safely removed with a stereotactic-assisted, endoscopic modified Lothrop approach (EMLP).
METHOD: Retrospective case review.
RESULTS: Three patients presented with extensive frontal sinus osteomas. Two underwent an EMLP and one patient with osteoma extending significantly into the orbit had an EMLP for removal of the medial frontal sinus component combined with an anterior orbitotomy to remove the orbital extension. In all cases stereotactic localization was utilized to ensure preservation of the skull base. Post-operatively, one…

DNazymes is a group of catalytic nucleic acids that can be designed to cleave target mRNA molecules in a base-specific way. Twist is a basic helix-loop-helix transcription factor that is involved in the regulation of cellular differentiation and apoptosis. Moreover, it was shown to function in skull development and cause craniosynostosis. DZ-TWT DNazyme was designed to down-regulate Twist expression. The ability of DZ-TWT to cleave mouse Twist mRNA was first shown in a cell-free environment against full-length Twist mRNA. Following transfections of the DZ-TWT in C3H10T1/2 cells, a significant

Malignant giant cell tumors (MGCTs) of the sphenoid sinus are extremely rare neoplasms. They are challenging to diagnose and difficult to treat because of their skull base location. To the best of our knowledge, we report the first case of a primary MGCT of the sphenoid arising in a patient with Paget's disease. A 77-year-old man presented with epistaxis and a history of Paget's disease. There was normal cranial nerve function although radiographic images disclosed a large mass centered in the sphenoid sinus and extending into the ethmoid and maxillary sinuses. Excisional biopsy revealed a MGC…
