Sarcoidosis in a 65-year-old woman presenting with a lung mass and pericardial effusion: a case report.

INTRODUCTION: Sarcoidosis is a multi-systemic disorder of unknown origin and most commonly affects the lungs. Diagnosis relies on the presence of non-caseating granulomas on histologic specimens. In high-resolution computed tomography, the most characteristic findings are peribronchovascular thickening, perilymphatic nodular distribution, and bilateral hilar adenopathy. Confluent nodular opacities or large masses are rare manifestations of the disease. It is well recognized that sarcoidosis can mimic infectious, malignant, and granulomatous conditions. Here, we report a case with a high initia

Unfocused extracorporeal shock waves induce anabolic effects in rat bone.

BACKGROUND: Extracorporeal shock waves are known to stimulate the differentiation of mesenchymal stem cells toward osteoprogenitors and induce the expression of osteogenic-related growth hormones. The aim of this study was to investigate if and how extracorporeal shock waves affected new bone formation, bone microarchitecture, and the mechanical properties of bone in a healthy rat model, in order to evaluate whether extracorporeal shock wave therapy might be a potential treatment for osteoporosis. METHODS: Thirteen rats received 1000 electrohydraulically generated unfocused extracorporeal sho

Comparison of 2 methods of making surgical models for correction of facial asymmetry.

PURPOSE: Stereolithography is useful in reconstructive surgery in that the surgical template or customized implant can be prefabricated on the models. To correct facial asymmetry, prior reshaping of the replica of the original structures is frequently required before it can be used as a surgical model. This is traditionally accomplished by direct sculpturing. This method has its limitations in clinical use. Recently, we developed a method using computer techniques to reconstruct the required structures. We herein report several of its applications in a variety of clinical situations and compar

Early Greater Trochanteric Avulsion Fracture After Posterior Approach Primary Total Hip Arthroplasty: A Clinically Relevant and Potentially Avoidable Complication.

BACKGROUND: A greater trochanteric (GT) fracture after primary total hip arthroplasty (THA) is a clinically relevant complication that can lead to gait abnormalities and chronic lateral hip pain. Early postoperative GT avulsion fractures are rare, possibly resulting from poor bone quality or the use of two transosseous drill holes for posterior capsular repair. Computer- and robotic-assisted surgery may increase fracture risk due to cortical defects from temporary pins. This study aimed to assess the rate of early GT fractures and identify contributing factors and prevention strategies. METHO

Case Report: A rare case of breast leiomyoma with literature review.

Breast leiomyoma is a rare benign mesenchymal tumor that typically manifests as a solitary, slow-growing mass. This article presents a case of a 58-year-old female with a history of follicular lymphoma confirmed by pathological examination of resected lymph nodes in November 2019. Follow-up positron emission tomography/computed tomography (PET-CT) revealed a breast soft-tissue mass initially classified as likely benign. Subsequent chest computed tomography (CT) in May 2024 demonstrated enlargement of the lesion, raising suspicion of lymphomatous infiltration. The mammogram shows an irregular m

Rapidly enlarging port-site mass after thoracoscopic lung resection: desmoid-type fibromatosis mimicking pleural recurrence.

BACKGROUND: Desmoid-type fibromatosis is a rare, benign fibroblastic neoplasm characterized by infiltrative growth and a tendency for local recurrence despite the absence of metastatic potential. It has been associated with prior trauma or surgery, and postoperative lesions arising in the chest wall can present a major diagnostic challenge. In patients after lung cancer resection, a new pleural-based or chest wall mass at a previous port site is often interpreted as pleural dissemination, chest wall recurrence, or port-site implantation, even when the primary tumor is pathologically low risk.

Primary mediastinal ectopic thyroid in a 43-year-old woman: A diagnostic and surgical challenge - a case report and literature review.

RATIONALE: Primary mediastinal ectopic thyroid (PMET) is exceedingly rare, and its imaging features often overlap with those of thymic epithelial tumors, lymphoma, Castleman disease, and paraganglioma. The absence of anatomical continuity with the cervical thyroid and the presence of an independent intrathoracic blood supply are important considerations in differential diagnosis and surgical planning. This report aims to highlight the diagnostic value of thyroid-like enhancement and the absence of cervical continuity and to demonstrate how preoperative computed tomography angiography (CTA)-bas

Diagnosis of Graves' orbitopathy: imaging methods, challenges, and new perspectives.

Precise assessment of Graves` orbitopathy (GO) predicts therapeutic strategies. Various imaging techniques and different measurement methods are used, but there is a lack of standardization. Traditionally, the Clinical Activity Score (CAS) has been used for assessing GO, especially for evaluating disease activity to predict response to glucocorticoid (GC) therapy, but technological developments have led to a shift towards more objective imaging methods that offer accuracy. Imaging methods for Graves' orbitopathy assessment include ultrasonography (USG), computed tomography (CT), magnetic reson

Reliability of a face scanner in measuring the vertical dimension of occlusion.

OBJECTIVE: This study evaluated the reliability of a face scanner in measuring the vertical dimension of occlusion (VDO). METHODS: Fully dentate volunteers (n = 20; mean-age = 30.0 ± 10.7 years) were recruited. Clinical facial measurements were obtained using a digital caliper and a face scanner (Obiscanner, Fifthingenium, Italy). The scans were imported into a mesh-processing software, and the distances were measured digitally. Measurements were obtained for each participant with the jaws positioned in maximal intercuspation (MI) and with increased vertical distances of 2, 4, and 6 mm. Verti

Linac- and CyberKnife-based MRI-only treatment planning of prostate SBRT using an optimized synthetic CT calibration curve.

PURPOSE: CT Hounsfield Units (HUs) are converted to electron density using a calibration curve obtained from physical measurements of an electron density phantom. HU values assigned to an MRI-derived synthetic computed tomography (sCT) may present a different relationship with electron density compared to CT HU. Correct assignment of sCT HU values is critical for accurate dose calculation and delivery. The goals of this work were to develop a sCT calibration curve using patient data acquired on a clinically commissioned CT scanner and assess for CyberKnife- and volumetric modulated arc therapy

3D extrusion printing of density gradients by variation of sinusoidal printing paths for tissue engineering and beyond.

During extrusion printing of pasty biomaterials, internal geometries are mainly adjusted by positioning of straightly deposited strands which does not allow realization of spatially adaptable density gradients in x-, y- and z-direction for anisotropic scaffolds or anatomically shaped constructs. Herein, an alternative concept for printing patterns based on sinusoidal curves was evaluated using a clinically approved calcium phosphate cement (CPC). Infill density in scaffolds was adjusted by varying wavelength and amplitude of a sinus curve. Both wavelength and amplitude factors were defined by

No difference between tibia-first and femur-first techniques in TKA using computer-assisted surgery.

PURPOSE: The measured resection technique and the gap-balancing technique are two philosophies used in total knee surgery. It is still unknown whether one or the other technique provides superior results when computer-assisted surgery is performed. We hypothesized that the gap-balancing technique improves joint stability because the technique relies primarily on the soft tissue. METHODS: A prospective controlled study was performed in 116 patients using the tibia-first or femur-first technique. Hypofractionated, 3-week, preoperative radiotherapy for patients with soft tissue sarcomas (HYPORT-STS): a single-centre, open-label, single-arm, phase 2 trial.

BACKGROUND: The standard preoperative radiotherapy regimen of 50 Gy delivered in 25 fractions for 5 weeks for soft tissue sarcomas results in excellent local control, with major wound complications occurring in approximately 35% of patients. We aimed to investigate the safety of a moderately hypofractionated, shorter regimen of radiotherapy, which could be more convenient for patients. METHODS: This single-centre, open-label, single-arm, phase 2 trial (HYPORT-STS) was done at a single tertiary cancer care centre (MD Anderson Cancer Center, Houston, TX, USA). We administered preoperative radio

Strategic management of altered passive eruption in anterior implant therapy: A case report.

BACKGROUND: A clinical scenario in the esthetic zone is the presence of short clinical crowns, which, in the absence of occlusal wear and other injuries, is referred to as altered passive eruption (APE). The presence of APE can lead to esthetic issues, which can impact the patient's smile harmony. In esthetically demanding cases, APE must be addressed prior to or in conjunction with implant therapy, and failure to do so can result in esthetic complications and failure in prosthetic planning. This case report exhibits a digital workflow in treating such patients to achieve optimal outcomes. ME

Rating the Efficacy of Diagnostic Imaging Studies Conducted for the Workup of Musculoskeletal Tumors.

BACKGROUND: Concerns for missed diagnoses have led to increased ordering of diagnostic imaging. Patients with suspected musculoskeletal tumors may undergo nondiagnostic and unnecessary imaging studies before referral to a musculoskeletal oncologist. This can result in patients receiving excessive radiation exposures, accruing unnecessary costs, delays in treatment, and an unnecessary burden on healthcare systems. PURPOSE: The purposes of this study were to (1) internally rate the usefulness of imaging tests completed for musculoskeletal oncology patients undergoing evaluation using a novel sc
