Defining the schizophrenia spectrum: issues for genetic linkage studies.

Genetic linkage studies of schizophrenia depend on accurate psychiatric diagnosis of relatives within multiply affected families. Each investigator makes a series of explicit or implicit decisions to define which relatives will be assumed to share a schizophrenia-related genotype, that is, who is an "affected relative." In this article we delineate issues that we believe should be considered in such studies and review the relevant literature. Issues include criteria for selecting probands; whether broader criteria should be used to select affected relatives; approaches to including or excludin

Health service utilization patterns for substance use disorders in Paraguay: real-world evidence from a specialized treatment centre in a low- and middle-income setting.

Substance use disorders (SUDs) impose a substantial and evolving burden globally, yet real-world evidence on how addiction services are utilized in low- and middle-income countries remains limited. This study examined patterns of SUD-related service utilization (ICD-10 F10-F19) in Paraguay using routinely collected data from a national specialized treatment centre in 2024. A total of 18,149 attendances were analysed. Service utilization was heavily concentrated among young adult males (83.8%), with individuals aged 20-29 years accounting for the largest proportion of attendances (42.1%). Polys

Dimensionality of hallucinogen and inhalant/solvent abuse and dependence criteria: implications for the Diagnostic and Statistical Manual of Mental Disorders-Fifth Edition.

BACKGROUND: Prior research has demonstrated the dimensionality of Diagnostic and Statistical Manual of Mental Disorders-Fourth Edition (DSM-IV) alcohol, nicotine, cannabis, cocaine and amphetamine abuse and dependence criteria. The purpose of this study was to examine the dimensionality of hallucinogen and inhalant/solvent abuse and dependence criteria. In addition, we assessed the impact of elimination of the legal problems abuse criterion on the information value of the aggregate abuse and dependence criteria, another proposed change for DSM-IV currently lacking empirical justification. A case of phobic anxiety related to the inability to smell cyanide.

This report describes a case of phobic anxiety relating to cyanide in a process operator who is unable to smell hydrogen cyanide. This case demonstrates that hazardous substances in the workplace can provoke this mental disorder in individuals who are unable to detect by special senses whether or not a specific hazard is present. The clinical management of such individuals is complicated since they must be able to perceive the feared object or substance in order to overcome their anxiety.

Pharmacotherapy of comorbid mood, anxiety, and substance use disorders.

Mood and anxiety disorders commonly co-occur with substance use disorders. Exploration of the neurobiology of substance use disorders and mood and anxiety disorders have found that the neural circuitry in mood, anxiety, and substance use disorders is clearly overlapping. These discoveries have encouraged the exploration of a number of pharmacotherapeutic agents in the treatment of co-occurring mood, anxiety, and substance use disorders. In this article, recent data on the pharmacotherapeutic treatment of mood and anxiety disorders in individuals with substance use disorders are reviewed. Some

All-cause mortality among individuals with disorders related to the use of methamphetamine: a comparative cohort study.

BACKGROUND: Understanding the mortality rate of methamphetamine users, especially in relation to other drug users, is a core component of any evaluation of methamphetamine-related harms. Although methamphetamine abuse has had a major impact on United States (U.S.) drug policy and substance-abuse treatment utilization, large-scale cohort studies assessing methamphetamine-related mortality are lacking. METHODS: The current study identified cohorts of individuals hospitalized in California from 1990 to 2005 with ICD-9 diagnoses of methamphetamine- (n=74,139), alcohol- (n=582,771), opioid- (n=67,

Clinical predictors of treatment in a population of adolescents with alcohol use disorders.

The purpose of this paper is to identify potential predictors of treatment utilization, among both psychiatric and drug and alcohol variables. The subjects were 393 adolescents and young adults, age 12.9 to 18.11 years, who met DSM-IV criteria for a lifetime history of either alcohol abuse or alcohol dependence at baseline assessment. DSM-IV psychiatric and AUD diagnoses were obtained by semi-structure interviews (K-SADS and SCID). Other alcohol and drug variables were obtained by the Alcohol Consumption Questionnaire and other self-reports. The results of these analyses suggest that there are

Differences in the symptom profile of methamphetamine-related psychosis and primary psychotic disorders.

We examined the lifetime experience of hallucinations and delusions associated with transient methamphetamine-related psychosis (MAP), persistent MAP and primary psychosis among a cohort of dependent methamphetamine users. Participants were classified as having (a) no current psychotic symptoms, (n=110); (b) psychotic symptoms only when using methamphetamine (transient MAP, n=85); (c) psychotic symptoms both when using methamphetamine and when abstaining from methamphetamine (persistent MAP, n=37), or (d) meeting DSM-IV criteria for lifetime schizophrenia or mania (primary psychosis, n=52). Relationship between substance use and body mass index in young males.

Recent findings in basic scientific research, such as neurobiological and neuroimaging studies, have suggested common pathways for food and drug intake. It was hypothesized that both compete for the same brain reward sites, and that a higher body mass index (BMI) may be associated with lower substance use. The aim of this study was to investigate the relationship between BMI and substance use in a large sample of young male adults. The sample consisted of 1,902 18-year-old males from a province of Austria in a naturalistic cross-sectional setting. Questionnaires were administered to assess alc

Relapse is a defining feature of substance use disorders, yet relapse risk varies widely across individuals. This variability has been linked to individual differences in incentive salience attribution to drug-related cues. We used fMRI to identify cortical and subcortical circuits underlying individual differences in incentive salience attribution among people with tobacco use disorder. Fifty daily cigarette users viewed pleasant, unpleasant, cigarette-related and neutral images during task-based fMRI and completed a resting-state scan. We extracted per-condition BOLD activity from a function

Iowa Gambling Task scores predict future drug use in bipolar disorder outpatients with stimulant dependence.

Poor decision-making is associated with poor recovery in persons with bipolar disorder and drug relapse in persons with stimulant dependence. Cognitive predictors of stimulant use in those with comorbid bipolar and stimulant dependence are surprisingly absent. Our goal was to determine if a single baseline assessment of decision-making (Iowa Gambling Task, IGT) would predict future drug use in bipolar disorder outpatients with comorbid stimulant dependence. Ninety-four men and women of multiple race/ethnic origins consented to participate in a 20-week study. Data analyses were performed on 63

Adolescent Age of Sexual Initiation and Subsequent Adult Health Outcomes.

OBJECTIVES: To examine the mechanisms of the association between age of sexual initiation and adult health. METHODS: Data from the Seattle Social Development Project (n = 808), in Seattle, Washington, included outcomes when participants were in their 30s (2005-2014): substance use disorders, depression, poor health, and obesity. Sexual consequence mediators included sexually transmitted infection, adolescent pregnancy, and a high number of sexual partners. We used linear logistic regression to model main effect and mediated associations. RESULTS: Age of sexual initiation was related to nicot

Changes in psychiatric disorder incidence patterns during the COVID-19 pandemic in Korea: a study using the nationwide universal health insurance data.

BACKGROUND: Few studies have evaluated the incidence of various psychiatric disorders during the coronavirus disease (COVID-19) pandemic using hospital visit data for the entire population of a nation. We used Korea's universal compulsory health insurance data to conduct a descriptive analysis to evaluate changes in psychiatric disorder incidence during the COVID-19 pandemic. METHODS: Hospital visit data related to psychiatric disorders were retrieved from the National Health Insurance Database. First-ever diagnosis for specific psychiatric disorders for each Korean was identified (from Janua

An Update about Mechanisms of Acupuncture Treatment for Mental Disorders: We Focus on Signaling Pathways, Neurotransmitters, and Neural Circuits.

This article serves to summarize the mechanism of acupuncture in the treatment of mental disorders. Mental disorders currently have a wide impact, and the number of patients is increasing every year, imposing a heavy burden on the entire society. Among the current first-line treatment methods, psychotherapy is questioned due to its limited efficacy, while drug treatment often comes with adverse reactions and symptom recurrence. As a historical complementary therapy, many clinical studies have confirmed the efficacy of acupuncture in treating mental disorders. However, the treatment rules and p

Receipt of Pharmacotherapy and Association with Healthcare Utilization Among US Adults with Alcohol or Opioid Use Disorder, 2022-2023.

BACKGROUND: Every year, millions of US adults with substance use disorders (SUDs) interface with the healthcare system, with high rates of emergency department (ED) visits and hospitalizations. FDA-approved medications for alcohol (AUD) and opioid use disorder (OUD) have been shown to decrease acute care utilization for addiction-related issues. However, the characteristics of adults who receive these medications and how receipt is associated with healthcare utilization are not well understood. The objective of this study was to characterize US adults with AUD or OUD who received addiction pha

Cognitive Bias Modification for Behavior Change in Alcohol and Smoking Addiction: Bayesian Meta-Analysis of Individual Participant Data.

Cognitive Bias Modification (CBM) refers to a family of interventions targeting substance-related cognitive biases, which have been found to play a role in the maintenance of addictive behaviors. In this study, we conducted a Bayesian meta-analysis of individual patient data from studies investigating the effects of CBM as a behavior change intervention for the treatment of alcohol and tobacco use disorders, in individuals aware of the behavior change goal of the studies. Main outcomes included reduction in the targeted cognitive biases after the intervention and in substance use or relapse ra
