The high retear rate after a successful repair of the rotator cuff (RC) is a major clinical challenge. Muscle atrophy and fat accumulation of RC muscles over time adversely affect the rate of retear. Since current surgical techniques do not improve muscle degenerative conditions, new treatments are being developed to reduce muscle atrophy and fat accumulation. In the previous study, we have shown the efficacy of aligned electroconductive nanofibrous fabricated by coating poly(3,4-ethylene dioxythiophene): poly(styrenesulfonate) (PEDOT:PSS) nanoparticles onto aligned poly(ε-caprolactone) (PCL)

Although the superior portion of the shoulder joint capsule is a clearly distinct structure, its ability to prevent vertical humeral head translation in irreparable rotator cuff tears remains controversial. The clinical results of superior capsule reconstruction (SCR) in this context are satisfactory but the radiological outcomes are poor in up to two-thirds of patients. Moreover, there is no proof that SCR is more beneficial than simply doing a subacromial debridement, a partial repair or a tenotomy of the long head of biceps. Furthermore, graft healing is not predictive of success. Given the

OBJECTIVE: To evaluate effects of loop diameter of a modified Kessler locking-loop (LL) suture on in vitro tensile strength and gapping characteristics of canine flexor tendon repairs.
SAMPLE: 48 cadaveric superficial digital flexor tendons from 24 adult medium- to large-breed dogs.
PROCEDURES: Flexor tendons were randomly assigned to 4 groups (n = 12/group) and repaired with 2-0 polypropylene in a LL pattern with loops measuring 1, 2, 3, or 4 mm in diameter. Biomechanical loads, gap formation between tendon ends, and failure modes were evaluated and compared between groups.
RESULTS: Increa

Extensor mechanism disruptions following total knee arthroplasty are devastating injuries with complication rates following surgical intervention ranging from 25% to 45%. Primary repair with and without augmentation is appropriate in certain limited clinical settings. Allograft reconstruction has been a popular option; however, synthetic grafts are showing promise and good results. In this article the authors discuss an algorithm for treating these difficult injuries as well as detail the surgical techniques for each approach.

Elbow dislocation is the second most common type of joint dislocation in adults and the most common joint dislocation in children. These dislocations are commonly associated with significant soft tissue injury and/or fractures. Anterior dislocation of the radio-capitellar articulation is uncommon. Failure of relocation of the radial head due to soft tissue imposition requiring open reduction is rare, however, is extremely important to recognise clinically and radiologically. This case report describes a case of anterior dislocation of the radio-capitellar joint due to entrapment of the brachia

Full-thickness tears of the rotator cuff are common, and surgical treatment is indicated in most cases. Arthroscopic repair has become the standard technique, but despite improved surgical techniques, instrumentations, and implants, failure rates continue to be high and can reach 40% and more. Augmented repairs with allograft patches or autograft tissue, such as long head biceps tendon or fascia lata, have been used with mixed results. However, autograft augmentation seems to be resulting in superior outcomes and higher healing rates. Treating skin defects with split-skin grafts is the gold st

Irreducible phalangeal fractures are uncommon, and if neglected can cause fracture non-union or functional deficits of the affected digit. We present two patients with irreducible phalangeal fractures - one in a child with a juxta-epiphyseal middle phalanx fracture incarcerated into the triangular ligament of the extensor apparatus and another in an adult following incarceration of a fractured proximal phalanx into the common intrinsic tendon on the radial side. Both required open reduction to extricate the incarcerated fracture ends, achieving good union and excellent functional outcomes. Lev

PURPOSE: To build upon previous literature to identify a complete analysis of cellular contents of subacromial bursal tissue as well as the matrix surrounding the rotator cuff.
METHODS: Samples of subacromial bursal tissue and surrounding matrix milieu from above the rotator cuff tendon and above the rotator cuff muscle bellies were obtained from 10 patients undergoing arthroscopic rotator cuff repair. Samples were analyzed using fluorescent-activated cell sorting and histologic analysis with staining protocols (Oil Red O, Alcian Blue, and Picro-Sirius Red), for identification of matrix compo

BACKGROUND: Inflammation and mechanical demands play a role in the development of tendon conditions and the dysregulation of tendon healing. In patients with obesity, high levels of pro-inflammatory cytokines and a high mechanical demand promote chronic low-grade inflammation. Although controversial results have been reported, we aimed to summarize current evidence while highlighting the role of obesity in tendinopathy.
QUESTIONS/PURPOSES: (1) Do patients with obesity have a greater risk of tendinopathy, stratified by upper and lower extremity sites, than patients who do not have obesity? (2)

Rotator cuff repairs are a common orthopedic procedure that have a relatively high failure rate when tendon quality is poor. New biotechnology exists that can make tendons with poor quality more amenable to repair. This study examined the biomechanical effects of augmenting a rotator cuff repair with an absorbable fiber patch. Six human cadaveric supraspinatus tendons were prepared into 1-cm wide strips and then repaired to the rotator cuff footprint using a titanium anchor and a single mattress suture. Each shoulder underwent repair with and without a fiber patch. The specimens were subjected

STUDY DESIGN: Mechanistic cadaver study.
OBJECTIVES: Patients in groups 4 and 5 of the International Classification for Surgery of the Hand in Tetraplegia have a few options for tendon transfer donors, but in general the needs for reestablishing motor power exceed the donor options, such that any increase in the number of potential transfers can enhance function. Although transfer of brachioradialis (BR) in these patients is well-established, pronator teres (PT) may also be a suitable donor due to its strength and excursion. It has not been extensively studied in this role, possibly because o

Avulsion fractures of the first metatarsal (MT1) base at the peroneus longus (PL) tendon attachment are rare and may be undiagnosed during an emergency visit. If the injury is not treated properly, chronic pain or persistent impairment for inversion and plantar-flexion of the first ray may occur. This case report presents a 30-year-old woman who presented 10 weeks post trauma to a foot and ankle surgeon due to a swollen right midfoot with diffuse tenderness over the medial Lisfranc joint. Further evaluation showed an isolated avulsion fracture of the first metatarsal, which was undiagnosed dur

BACKGROUND:: A reliability generalization study of a questionnaire is necessary to provide higher-level evidence for its reliability. This has not been performed for the Victorian Institute of Sport Assessment-Achilles tendinopathy (VISA-A) questionnaire. The VISA-A has been a commonly used questionnaire to evaluate the symptoms of Achilles tendon disorders and their impact on physical activity, one of the most common disorders among athletes and sports persons (9%-40%). Furthermore, this questionnaire has been translated to several languages and due to its simplicity is one of the most widely

Long bone fractures and disorders of tendons and ligaments represent a significant proportion of surgical orthopedic cases presented to ruminant veterinarians. The presentation of these patients, their diagnostic work-up, surgical treatment, and expected outcome will be discussed. The outcome of these cases depends largely on the presenting problem; however, accurate diagnosis and prompt surgical intervention can greatly improve the outcome of many of these cases.

Extensor tendon rupture in chronic Madelung deformity, as a result of tendon attrition on the dislocated distal ulna, is a rare occurrence. It is, however, seen more often in rheumatoid arthritis. There are few case reports in the English-language literature on this issue. We report a case of multiple tendon ruptures in a previously undiagnosed Madelung deformity.

BACKGROUND: Isolated dislocation of the radial head is an uncommon injury among pediatric patients. Although closed reduction may be successful, some patients require open reduction with or without soft-tissue repair or reconstruction. If the radiocapitellar joint is not properly reduced, the long-term complications of pain, stiffness, neuropathy, and dysplasia can result from chronic radial head dislocation.
METHODS: While performing a cartwheel, a 13-year-old gymnast sustained a radiocapitellar dislocation that was unable to be reduced by closed means and failed 2 attempts at open reduction
