Prehospital Damage Control: The Management of Volume, Temperature… and Bleeding!

Damage control resuscitation should be initiated as soon as possible after a trauma event to avoid metabolic decompensation and high mortality rates. The aim of this article is to assess the position of the Trauma and Emergency Surgery Group (CTE) from Cali, Colombia regarding prehospital care, and to present our experience in the implementation of the "Stop the Bleed" initiative within Latin America. Prehospital care is phase 0 of damage control resuscitation. Prehospital damage control must follow the guidelines proposed by the "Stop the Bleed" initiative. We identified that prehospital pers

Predation risk mediates cognitive constraints following physical exertion in schoolmaster snapper.

A large body of literature suggests that physically exhausted fish, including those that are released following fisheries interactions, experience behavioural and cognitive constraints and are at elevated risk of predation during homeostatic recovery. However, previous studies have focused on exhausted fish subsequently encountering predators, and not on fish that had been exposed to elevated predation risk prior to exhaustive exercise. Here, we exercised individual schoolmaster snapper (Lutjanus apodus) for 0, 1, or 4 min via hand chases following exposure to conspecific chemical alarm cues o

Eye-tracking research in eating disorders: A systematic review.

OBJECTIVE: Those with eating disorders (EDs) show attentional biases to disorder-relevant stimuli, such as food and body shape information. However, attentional bias research in EDs largely relies on reaction time based measures, which are limited in their ability to assess different components and the time course of attention. Eye-tracking paradigms have therefore been utilized to provide greater ecological validity, and directly capture the detailed sequence of processes in perception and attention. While numerous studies have examined eye movements in the mood, anxiety, and psychotic disord

Frameless and Maskless Stereotactic Navigation with a Skull-Mounted Tracker.

OBJECTIVE: In this series, we present 3 cases that show the use of a skull-mounted tracker for image-guided navigation for anterior skull base surgery and ventricular catheter placement procedures. This system obviates fiducials or face masks during the surgical procedure itself and allows for the performance of facial incisions using the Weber-Ferguson approach. METHODS: Our series presents the use of a novel intraoperative navigational system that uses a skull-mounted tracker to navigated anterior skull base surgery. RESULTS: We present 3 cases using this new system: 1 anterior skull base

A 5-year retrospective review of fungal keratitis in the region of Cap Bon.

INTRODUCTION: Microbial keratitis is a serious ocular infection and a leading cause of morbidity and blindness worldwide. METHODS: A retrospective review of the charts of 30 patients (30 eyes) diagnosed with presumed or culture-proven fungal keratitis among 100 patients with infectious keratitis. All patients initially received hourly 0.5% Amphotericin B eye drops. Systemic antifungal agents consisted mainly of oral Fluconazole. After treatment, a healing time of less than 3 weeks from presentation was considered a good result. Mean follow up was 10.4 months. RESULTS: Risk factors for fungal

Phantom hand and wrist movements in upper limb amputees are slow but naturally controlled movements.

After limb amputation, patients often wake up with a vivid perception of the presence of the missing limb, called "phantom limb". Phantom limbs have mostly been studied with respect to pain sensation. But patients can experience many other phantom sensations, including voluntary movements. The goal of the present study was to quantify phantom movement kinematics and relate these to intact limb kinematics and to the time elapsed since amputation. Six upper arm and two forearm amputees with various delays since amputation (6months to 32years) performed phantom finger, hand and wrist movements at

Mal de debarquement syndrome: a systematic review.

Mal de debarquement (MdD) is a subjective perception of self-motion after exposure to passive motion, in most cases sea travel, hence the name. Mal de debarquement occurs quite frequently in otherwise healthy individuals for a short period of time (several hours). However, in some people symptoms remain for a longer period of time or even persist and this is then called mal de debarquement syndrome (MdDS). The underlying pathogenesis is poorly understood and therefore, treatment options are limited. In general, limited studies have focused on the topic, but the past few years more and more int

Ovine ear model for fully endoscopic stapedectomy training.

Endoscopic surgery of the middle ear is progressively gaining the interest of otologists, as technological advances have overcome some of its main drawbacks. The long learning curve required to master this technique, urges the search for models to practice it. After the validation of sheep's ear as a proper training model for microscopic stapedectomy, our objective is to demonstrate its adequacy for practicing stapes surgery but performed through a fully endoscopic approach. Endoscopic stapedectomy was performed by two surgeons in 40 sheep ears (20 specimens each). To analyze the effects of th

Ego-dystonic pregnancy and prenatal consumption of alcohol among first-time mothers.

This study examines predictors of drinking during pregnancy among first-time mothers, in order to distinguish those in need of targeted screening and intervention. Data from the prenatal panel of the Parenting for the First Time study were used in hierarchical linear regressions to determine likelihood of prenatal alcohol consumption among a sample of 645 women. African-American women and those of race/ethnicities other than White were less likely to drink, regardless of age or level of education. Among all women, being in school was associated with abstention (P = 0.05). Among teens, endorsin

Time since stroke influences the impact of hemianopia and spatial neglect on visual-spatial tasks.

OBJECTIVE: Spatial neglect results in an ipsilesional misorientation of attention in visual-spatial tasks. Hemianopia impairs visual perception but its influence on visual-spatial tasks is subject to debate. Here, we investigated the influence of the time since stroke on the respective impacts of hemianopia and spatial neglect. METHOD: A total of 29 patients with a right hemisphere stroke were included in the study. Nine had severe neglect and hemianopia, six had severe neglect only, four showed hemianopia with little or no neglect after infarction of the posterior cerebral artery, and 10 had

Developmental dyslexia and spatial relationship perception.

According to wide literature, a global impairment in the temporal and spatial domains as well as an increased crowding effect is common of dyslexics. The aim of the study was to evaluate if such subjects suffer from a more general impairment of spatial relationship perception (SRP) and in particular from anomalous spatial relationship anisotropy (SRA) thus accounting both for their global perceptual distortions and abnormal crowding. SRP of 39 young disabled readers and 23 normal subjects were measured by a specifically designed psychophysical technique based on circular and elliptical target

Pre- and postsynaptic excitation and inhibition at octopus optic lobe photoreceptor terminals; implications for the function of the 'presynaptic bags'.

Synaptic transmission was examined in the plexiform zone of Octopus vulgaris optic lobes using field-potential recording from optic lobe slices. Stimulation of the optic nerve produced pre- and postsynaptic field potentials. Transmission was abolished in calcium-free seawater, L- glutamate or the AMPA/Kainate receptor blocker CNQX (EC(50), 40 microm), leaving an intact presynaptic field potential. ACh markedly reduced or blocked and d-tubocurarine augmented both pre- and postsynaptic field potentials, while alpha-bungarotoxin and atropine were without effect. Paired-pulse stimulation showed sh

Is neonatal intensive care justified in all preterm infants?

A proactive policy of resuscitation at birth and prompt initiation of intensive care have been shown to be associated with an improvement in the survival of very preterm infants in both institution-based and population-based studies. As a greater percentage of live births were offered intensive care, the survival rate rose progressively in all birth weight and gestation subgroups among extremely low birth weight infants, including those who were born at borderline viability between 23 weeks and 25 weeks of gestational age. Their quality-adjusted survival rate also rose progressively, since the
