Pattern and Outcome of Thoracic Injuries in a Busy Tertiary Care Unit.

INTRODUCTION: Thoracic traumas are one of the most commonly encountered injuries in the emergency room. They range from blunt chest injuries due to road traffic accidents to penetrating chest injuries. Immediate medical and surgical interventions are essential to improve the outcome. This study was conducted to assess the pattern of thoracic trauma presenting to the emergency room, their outcome and factors contributing to it. METHODS: This prospective, observational, cross-sectional study was conducted in the Department of Thoracic Surgery, Jinnah Post Graduate Medical Center, Karachi from J

Prehospital Factors Associated With Cervical Spine Injury in Pediatric Blunt Trauma Patients.

BACKGROUND: The risk for cervical spine injury (CSI) must be assessed in children who sustain blunt trauma. The Pediatric Emergency Care Applied Research Network (PECARN) retrospectively derived CSI model identifies CSI risk in children based on emergency department (ED) provider observations. The objective of this pilot study was to determine the univariate association of emergency medical services (EMS) provider-observed historical, mechanistic, and physical examination factors with CSI in injured children. Secondarily, we assessed the performance of the previously identified eight PECARN CS

Concomitant Intra-Articular Glenohumeral Lesions in Fractures of the Scapula Body.

BACKGROUND: Scapula body fractures are rare injuries with an incidence of 1% of all fractures accounting for 3% to 5% of all upper extremity fractures. Fractures of the scapula commonly result from high-energetic trauma and fall from great height. While several studies focused on concomitant injuries of chest and head as well as the cervical spine, up to now in the common literature, no study exists analyzing the prevalence of concomitant intra-articular glenohumeral injury following extra-articular scapular fracture. OBJECTIVES: The aim of this study was to analyze the prevalence of concomit

Trauma-induced coagulopathy: standard coagulation tests, biomarkers of coagulopathy, and endothelial damage in patients with traumatic brain injury.

It remains to be debated whether traumatic brain injury (TBI) induces a different coagulopathy than does non-TBI. This study investigated traditional coagulation tests, biomarkers of coagulopathy, and endothelial damage in trauma patients with and without TBI. Blood from 80 adult trauma patients was sampled (median of 68 min [IQR 48-88] post-injury) upon admission to our trauma center. Plasma/serum were retrospectively analyzed for biomarkers reflecting sympathoadrenal activation (adrenaline, noradrenaline), coagulation activation/inhibition and fibrinolysis (protein C, activated protein C, ti

Promoting Access to Positive Sexual Health in Schools to Support Equitable Academic Outcomes.

BACKGROUND: Sexual health is critical to academic success and adolescent well-being. This paper expands Basch's 2011 analysis of sexual health disparities to a comprehensive definition of positive adolescent health, including teen births, sexually transmitted infections (STI), dating violence, and sexual identity discrimination. CONTRIBUTIONS TO THEORY: Over the last two decades, as teen birth rates fall and sexually transmitted infection (STI) rates rise, patterns of disparities stay relatively constant. Driven by systemic factors like institutionalized racism and poverty, Fundamental Cause

Minimally invasive approaches to coronary artery bypass grafting: techniques, current evidence, and future directions.

PURPOSE OF REVIEW: Conventional coronary artery bypass grafting (CABG) is most commonly performed via median sternotomy, an approach that can be associated with postoperative pain, sternal wound complications, and prolonged recovery. Over the past two decades, multiple minimally invasive coronary surgery strategies have been developed, offering sternotomy-sparing alternatives that may reduce surgical trauma and accelerate recovery in appropriately selected patients. This review summarizes contemporary minimally invasive strategies, focusing on their procedural steps, patient selection, clinica

CT of Acute Shoulder Girdle Fractures in Adults: Biomechanics, Classification, and Management.

The shoulder girdle, which includes the clavicle, scapula, and proximal humerus, forms a dynamic scaffold for seamless motion and load transfer to the axial skeleton. CT with multiplanar reformatted and volume-rendered images allows precise characterization, measurement, and injury classification and is particularly useful for complex high-energy disruptions encountered in trauma centers. The spectrum of shoulder girdle injuries spans clavicle fractures, acromioclavicular joint separations, floating shoulder patterns, glenohumeral fracture-dislocations, glenoid fossa fractures, proximal humeru…

Identifying Avoidable Emergency Department Visits and the Potential Role of Community Health Centers in Northern Italy: A Retrospective Observational Study.

BACKGROUND: Emergency departments (EDs) are experiencing a growing burden of non-urgent presentations, which frequently signal deficiencies in access to primary and community-based healthcare. Evidence suggests that a substantial proportion of ED visits could be safely and effectively managed within community care settings. The general aim of the study was to identify which ED visits could have been managed within primary and community care services in a scenario of full implementation of community health centers. METHODS: A retrospective observational study was conducted. This study examined

Factors associated with discharge to home after traumatic rib fractures.

BACKGROUND: Chest wall injury is common among trauma patients. Generally, patients with more medical problems tend to have worse outcomes with rib fractures. Our aim was to determine if surgical stabilization of rib fractures (SSRF) increases the likelihood of discharge to home. METHODS: We performed a retrospective cohort study of patients with ≥1 comorbidity, aged 18-99 with rib fractures from five Chest Wall Injury Society (CWIS) Collaborative Centers. Discharge disposition of patients who underwent SSRF was compared to those who did not undergo surgery. Discharge to home was considered th

Biliary Leaks: Multidisciplinary Approach to Diagnosis and Treatment.

Bile leaks arise from various causes such as trauma, complications after hepatobiliary surgery, and intrahepatic malignancies or their associated liver-directed treatments. Bile leaks can result in significant morbidity and mortality. Delayed diagnosis is not uncommon due to nonspecific manifestations; therefore, a high clinical suspicion is needed. A multidisciplinary approach for treatment of biliary leaks with prompt referral to tertiary care centers with experienced hepatobiliary surgeons, advanced endoscopists, and interventional radiologists is needed to address these challenging complic

The effect of a reduction in irrational beliefs on Posttraumatic Stress Disorder (PTSD), depression, and anxiety symptoms in a group treatment for post-9/11 Veterans.

Previous research has indicated that a Rational Emotive Behavior Therapy (REBT)-Informed Group focused on changing irrational beliefs to address comorbid depression and anxiety (as well as anger and guilt) in a combat Veteran population diagnosed with Posttraumatic Stress Disorder (PTSD) demonstrated significant reductions in depression and PTSD symptoms at posttreatment. However, mechanisms of change associated with improvement have not been evaluated. REBT theory suggests that a decline in irrational beliefs predicts a decrease in PTSD, depression, and anxiety symptoms. This study aimed to t
