A review of the natural course of bipolar disorders (manic-depressive psychosis) in the pre-drug era: review of studies prior to 1950.

A review of the most important original studies describing the natural course of bipolar disorder (manic-depressive psychosis) published in the pre-drug era - before 1950 - is conducted. Discrepancies among studies are detected, most of which are likely explained by methodological differences. However, some conclusions from these old studies remain perfectly valid nowadays: mania is a chronic brain disorder, inherited in most cases, decompensation being more frequent between March and August. It is more common in males, and in some cases, is secondary to other somatic problems. Mixed states ar…

Clinical implications of antipsychotic-induced hyperprolactinemia in patients with schizophrenia spectrum or bipolar spectrum disorders: recent developments and current perspectives.

Hyperprolactinemia is increasingly studied as a frequent and potentially important consequence of antipsychotic medication treatment. Some individuals presenting with hyperprolactinemia remain asymptomatic, but others may exhibit a wide range of clinical symptoms resulting from either the direct effects of prolactin on body tissues (galactorrhea, gynecomastia) or endocrine-related secondary effects (sexual and reproductive dysfunction in the short term, and possibly the risk of tumorigenesis and osteoporosis in the longer term). Short-term side effects may negatively impact medication complian

The comparative clinical phenotype and long term longitudinal episode course of bipolar I and II: a clinical spectrum or distinct disorders?

BACKGROUND: The present analyses were designed to compare the clinical characteristics and long-term episode course of Bipolar-I and Bipolar … ween affective episodes. CONCLUSIONS: The variety in severity of the affective episodes shows that bipolar disorders, similar to unipolar disorders, are expressed longitudinally during their course as a dimensional illness. The similarities of the clinical phenotypes of BP-I and BP-II, suggest that BP-I and BP-II are likely to exist in a disease spectrum. They ar …

The prevalence and disability of bipolar spectrum disorders in the US population: re-analysis of the ECA database taking into account subthreshold cases.

BACKGROUND: Despite emerging international consensus on the high prevalence of the bipolar spectrum in both clinical and community samples, many skeptics contend that narrowly defined bipolar disorder with a lifetime rate of about 1% represents a more accurate estimate of prevalence. This may in part be due to the fact that higher figures proposed for the bipolar spectrum (5-8%) have not been based on national data and have not included all levels of manic symptom severity. In the present secondary analyses of the US National Epidemiological Catchment Area (ECA) database, we provide further cl…

The effects of lithium discontinuation and the non-effect of oral inositol upon thyroid hormones and cortisol in patients with bipolar affective disorder.

Thyroid and adrenal function was assessed in euthymic bipolar patients, stable on prophylactic lithium for at least 1 year, before and after lithium discontinuation in a randomised double-blind placebo-controlled trial. All hormonal measurements were within the reference range, but a significant increase (P less than 0.001) in plasma thyroxine (T4) levels and a decrease (P less than 0.01) in TSH levels were observed 1 month after lithium withdrawal; cortisol concentrations showed a non-significant decrease in the same period. No relationship could be demonstrated between the magnitude of the c

Differential changes in frontolimbic functional connectivity following 12-week psychostimulant monotherapy in ADHD youth with and without familial risk for bipolar I disorder.

BACKGROUND: Although attention-deficit/hyperactivity disorder (ADHD), psychostimulant medications, and BD family history are risk factors for developing bipolar I disorder (BD), their contribution to the aberrant ventrolateral prefrontal cortex (VLPFC) and amygdala (AMY) functional connectivity (FC) implicated in the pathophysiology of BD remain unclear. METHODS: We investigated the effects of 12-week daily oral psychostimulant monotherapy on AMY-VLPFC FC during emotional and task-related attentional processing in ADHD youth with ('high-risk', HR) and without ('low-risk', LR) a first-degree r

Psychotic and bipolar disorder diagnoses following contact with child and adolescent mental health services-a longitudinal, nationwide register study of 2 million individuals.

BACKGROUND: Identification of individuals at elevated risk of schizophrenia spectrum and bipolar disorders is a clinical research priority. Current strategies for psychosis risk identification, however, identify only a small proportion of future cases. Additional high-risk approaches are needed, ideally with higher sensitivity, which may include focusing on systems in which risk factors for psychosis are naturally concentrated, such as child and adolescent mental health services (CAMHS). METHODS: We examined the proportion of psychosis and bipolar disorder cases occurring in individuals who h…

Regional cerebellar structural deficits distinguish psychostimulant-free ADHD youth with and without familial risk for bipolar I disorder: a cross-sectional morphometric analysis.

BACKGROUND: Although attention-deficit/hyperactivity disorder (ADHD) with familial risk for bipolar I disorder (BD) may represent a more severe illness conferring greater risk for developing BD, associated neurostructural substrates remain poorly understood. This study examined cerebellum structural alterations, which prior studies suggested may be associated with BD risk. METHODS: We enrolled 151 youth (ages 10-18 years) in three groups: psychostimulant-free ADHD youth with a biological parent or sibling with BD (high-risk, n = 52, mean age 13.8 ± 2.6 years), psychostimulant-free ADHD youth

Chronotherapeutic intervention targeting emotion regulation brain circuitry, symptoms, and suicide risk in adolescents and young adults with bipolar disorder: a pilot randomised trial.

BACKGROUND: Mood episodes and high suicide risk of bipolar disorder (BD) are thought to derive from amygdala-ventral prefrontal cortex emotion regulation brain circuitry dysfunction and resulting emotion dysregulation, making these potential intervention targets. OBJECTIVE: To assess feasibility, acceptability, and preliminary efficacy in engaging the emotion regulation targets of two Brain Emotion Circuitry-targeted Self-Monitoring and Regulation Therapy (BE-SMART) variations in adolescents and young adults with BD (BDAYA): BE-SMART-ER, which directly targets emotion regulation, and BE-SMART…

Computational Modeling of PI3K/AKT Pathway in Bipolar Disorder and Type 2 Diabetes: Implications for Lithium Treatment and Curcumin as a Potential Alternative.

Bipolar disorder (BD) exhibits a high comorbidity rate with type 2 diabetes (T2D), suggesting shared pathophysiological mechanisms. Although lithium serves as the first-line treatment for BD, its underlying therapeutic mechanism and potential effects on T2D remain incompletely understood. This study identified the PI3K/AKT pathway as a key link between these disorders. Using an ordinary differential equation (ODE) model that integrates the PI3K/AKT pathway with the phosphatidylinositol (PI) cycle, we simulated lithium's regulatory effects in BD treatment. Our simulations revealed that lithium'…

Exploring Behavioural Patterns in Youth Predisposed to Bipolar Disorder and the Role of Interpersonal Trauma Using the Adolescent Brain Cognitive Development (ABCD) Dataset.

INTRODUCTION: Bipolar disorder (BD) is a severe, persistent disorder that causes functional impairment. Besides heritability, environmental factors, such as traumatic experience, impact the development of BD. Little is known about the early developmental signs of this disorder; therefore, this study aims to look at the impact of interpersonal trauma on the early developmental signs of BD. Specifically, differences in psychopathological behaviours were investigated between (1) at-risk children and controls and (2) at-risk children who experienced an interpersonal traumatic event and those who d…

Bipolar Disorder and Ubiquitin Proteasome System Dysfunction: Peripheral Blood Levels of Molecules Playing a Role in Ubiquitination and Their Relationship to Sleep Quality.

BACKGROUND: Bipolar disorder (BD) is a serious mood disorder, notable for its morbidity and prevalence. It ranks among the top 10 diseases globally in terms of functional impairment among affected individuals. Studies investigating neurobiological processes in the development of BD also aim to identify biological markers. Ubiquitin is a protein that is abundant in all eukaryotic cells and regulates many processes through the ubiquitin-proteasome system. It has been reported to be associated with circadian rhythm and sleep disorders. Circadian rhythm plays a key role in maintaining mood stabili…

Copy number variations in RNF216 and postsynaptic membrane-associated genes are associated with bipolar disorder: a case-control study in the Japanese population.

AIM: Bipolar disorder (BD) is a common psychiatric disorder characterized by alterations between manic/hypomanic and depressive states. Rare pathogenic copy number variations (CNVs) that overlap with exons of synaptic genes have been associated with BD. However, no study has comprehensively explored CNVs in synaptic genes associated with BD. Here, we evaluated the relationship between BD and rare CNVs that overlap with synaptic genes, not limited to exons, in the Japanese population. METHODS: Using array comparative genome hybridization, we detected CNVs in 1839 patients with BD and 2760 cont…
