Ipsilateral proximal and shaft femoral fractures treated with bridge-link type combined fixation system.

BACKGROUND: Although many treatments for ipsilateral proximal and shaft femoral fractures have been developed, controversy exists regarding their optimal management. The purpose of this retrospective study was to discuss the effectiveness of the bridge-link type combined fixation system (BCFS) and evaluate functional outcomes in treating patients with these complex fractures. PATIENTS AND METHODS: We retrospectively reviewed 14 cases of ipsilateral proximal and shaft femoral fractures treated from January 2012 to December 2016. All cases were treated by BCFS combined with minimally invasive p

Evaluation of Midterm Results of the Panta Nail: An Active Compression Tibiotalocalcaneal Arthrodesis Device.

A tibiotalocalcaneal intramedullary rod is a powerful construct often reserved for difficult and salvage cases. The Panta Nail (Integra LifeSciences, Plainsboro, NJ) is a second-generation rod that offers active axial compression compared with previous rod designs. We characterized our experience with this device and identified factors associated with nonunion. We retrospectively identified 19 patients receiving the Panta Nail during a 4-year period with a mean follow-up period of 16 months. We used a consecutive series approach toward inclusion. Fusion, asymptomatic fibrous union, or nonunion

A comparison of intervention and conservative treatment for angulated fractures of the distal forearm in children (AFIC): study protocol for a randomized controlled trial.

BACKGROUND: Angulated fractures of the distal forearm are very frequent lesions in childhood. Currently, there are no standard guidelines on whether these children should be treated conservatively with a cast; with reduction and a cast; or with reduction, pinning and a cast under anesthesia. Minor prospective and retrospective studies have shown that the distal physis of the forearm possesses high remodeling capacity leading to reliable correction of malalignment. The aim of this trial is to answer the question about whether operative and conservative treatment show equivocal results. METHODS

Quality and Utility of Immediate Formal Postoperative Radiographs in Ankle Fractures.

BACKGROUND: Patients who undergo internal fixation of ankle fractures commonly have postoperative imaging performed immediately after surgery. As these patients typically are typically immobilized, radiographs provide limited visualization. The purpose of this study was to evaluate the utility and quality of formal radiographs performed immediately following ankle fracture surgery. METHODS: Ankle fractures undergoing open reduction and internal fixation at a single institution from January 1, 2011, to January 1, 2013, were reviewed. Intraoperative and formal postoperative radiographs were eva

Multi-directional Le Fort III midfacial distraction using an individual prefabricated device.

BACKGROUND: Midfacial distraction following Le Fort III osteotomy has become an established procedure for midfacial advancement of syndromic craniosynostosis patients. A frequent difficulty is the precise three dimensional (3D) alignment of the distracted midface and the proper fixation of the distraction device in the midfacial area. In this study we present an individual modification method for commercial distraction devices comprising the establishment of prefabricated fixation plates and parallel connecting pins. MATERIAL: Individual prefabricated fixation plates for the zygomatic buttres

Why it's better to believe in a larger definition of the diametaphyseal junction zone in pediatric distal radius fractures.

INTRODUCTION: Diametaphyseal radius fractures (DMRF) in children are characterized by special biomechanical behavior. They present with a stereotypical loss of reduction (radial translation and ulnar tilt) of the distal fragment when stabilized with elastic stable intramedullary nailing (ESIN), the standard osteosynthesis for pediatric diaphyseal forearm fractures. No consensus has been found on the definition of the diametaphyseal junction zone (DMJZ). Some authors claim that diaphyseal fractures immediately proximal to the metaphysis, as defined by the AO Pediatric Comprehensive Classificati

Intramedullary nailing in segmental fractures of shaft of femur: analysis of parameters influencing time to union.

PURPOSE: Segmental fractures, being associated with high energy trauma, are quite intimidating for the treating surgeon due to wide incidence of complications. The soft tissue damage and the fracture pattern in segmental fractures further disrupt the precarious nutrition of the floating intercalary segment, affecting fracture healing. Our study analyses the parameters influencing time to union in segmental fractures of femur. METHODS: A retrospective analysis of all patients who presented to our level 1 trauma centre between January 2015 and December 2020 with traumatic segmental fractures of

Assessing the Importance of Anteromedial Cortical Support in Fixing Pertrochanteric Fractures: A Comprehensive Study.

INTRODUCTION: Pertrochanteric fractures are among the most common hip fractures in the elderly population, typically resulting from low-energy trauma such as a simple fall. This demographic is particularly susceptible due to factors like decreased bone density, frailty, and impaired balance. It can occur in the younger patient due to high-velocity trauma - road traffic accidents. Medial cortical support (MCS) reduction is a key element, as it allows limited sliding of head-neck fragments and it provides good fracture healing and MCS acts as non-anatomic buttress reduction providing secondary s

Biomechanical comparison of femoral neck system and cannulated screws coupled with medial plate for treating Pauwels III femoral neck fractures.

BACKGROUND: The femoral neck system (FNS) has been considered as a novel strategy for femoral neck fracture. The diversity of internal fixation creates difficulties in choosing an effective option for Pauwels III type femoral neck fractures. Therefore, it is significant to investigate the biomechanical effects of FNS versus conventional approaches on bones. OBJECTIVE: To evaluate the biomechanical characteristics of FNS versus cannulated screws coupled with medial plate (CSS+MP) for the treatment of Pauwels III type femoral neck fractures. METHODS: Through three-dimensional computer software

Simulation-based prediction of bone healing and treatment recommendations for lower leg fractures: Effects of motion, weight-bearing and fibular mechanics.

Despite recent experimental and clinical progress in the treatment of tibial and fibular fractures, in clinical practice rates of delayed bone healing and non-union remain high. The aim of this study was to simulate and compare different mechanical conditions after lower leg fractures to assess the effects of postoperative motion, weight-bearing restrictions and fibular mechanics on the strain distribution and the clinical course. Based on the computed tomography (CT) data set of a real clinical case with a distal diaphyseal tibial fracture, a proximal and a distal fibular fracture, finite ele

The Comparison of Dynamic Condylar Screw Plate to Proximal Femoral Nail in Reverse Oblique and Transverse Intertrochanteric Fractures: A Retrospective Study on 61 Patients.

OBJECTIVE: Reverse oblique (RO) and transverse intertrochanteric fracture patterns constitute a challenge for the operating surgeon. Currently, no gold standard fixation method exists. This study aimed to retrospectively compare proximal femoral nail (PFN) to dynamic condylar screw (DCS) plating in the treatment of RO and transverse intertrochanteric fractures. METHODS: A total of 61 patients fixated by PFN or DCS were included. Of these, 36 were treated with PFN (21 females and 15 males; mean age: 65.52 years), and 25 were treated with DCS (12 females and 13 males; mean age: 59.36 years). Th

Outcomes After Revision Fixation With Cement Augmentation for Failed Intertrochanteric Fracture Fixation in Older Adult Patients.

INTRODUCTION: Intertrochanteric (IT) fractures that fail fixation are traditionally treated with arthroplasty, introducing significant risk of morbidity and mortality in frail older adult patients. Revision fixation with cement augmentation is a relatively novel technique that has been reported in several small scale international studies. Here we report a clinical series of 22 patients that underwent revision fixation with cement augmentation for IT fracture fixation failure. METHODS: This retrospective case series identified all patients that underwent revision intramedullary nailing from 2

Technique and clinical results of a new intramedullary support nail and plate system for fixation of 3- or 4- part proximal humeral fractures in older adults.

BACKGROUND: Internal fixation of complex proximal humeral fractures (PHF) with osteoporosis is associated with a high incidence of complications. This study introduces the technique and clinical results of a novel intramedullary support nail and plate system (ISNPs) for the internal fixation of 3- or 4- part PHF in older adults. The ISNPs combines the concept of intramedullary support and dynamic fixation into a locking plate fixation system that can be applied using a minimally invasive surgical approach. METHODS: A total of 46 consecutive patients diagnosed with 3- or 4-part PHF that met th
