Arthroscopic-Assisted Intraosseous Bioplasty of the Humeral Head for Osteonecrosis.

Osteonecrosis of the humeral head is an uncommon condition, and treatment options are controversial. The shoulder is the second most common location for osteonecrosis, typically presenting between the second and fifth decades of life. Early diagnosis and treatment are essential because osteonecrosis may progress and lead to significant pain and loss of function. Nonoperative options are limited and are based on addressing the cause of the osteonecrosis. Multiple surgical treatments have been described, and these techniques continue to evolve. Open core decompression of the humeral head has bee

[Surgical treatment of a post-traumatic giant pseudoaneurysm of the axillary artery: a case report].

A false aneurysm of the axillary artery is an extremely rare complication of a lesion of this vessel. As few as several dozens of similar cases have been described in the available literature. We herein report a clinical case concerning surgical treatment of a 41-year-old patient who after a knife-inflicted injury had developed a 66x67 mm pseudoaneurysm of the axillary artery. The treatment consisted of several stages, thus requiring 2 hospitalizations. The first stage included an attempt to resect the aneurysm in an open fashion, followed by deployment of stent grafts in the axillary artery t

Cauda Equina Syndrome in a Lactating Mother - A Safe Treatment Approach.

Cauda equina syndrome is widely considered as a surgical emergency. The cause of cauda equina syndrome usually is a large central lumbar disc herniation, prolapse or sequestration. Decompression at the earliest has been suggested by many authors but the planning of surgical management becomes challenging when the patient is a breastfeeding mother. Fear of harmful effects of the drugs (administered in the mother) on the infant, always confuses clinicians regarding the treatment approach. So the multidisciplinary approach is necessary with involvement of anaesthetist, paediatrician and also a gy

Transforaminal Endoscopic Solution to a Kyphoplasty Complication: Technical Note.

Kyphoplasty is a minimally invasive spine surgical procedure performed to stabilize and treat the pain caused by a spine compression fracture. Complications are rare with kyphoplasty and include cement extrusion into the vertebral canal leading to spinal cord or nerve root compression. Herein, the authors present a case of a 72-year-old woman who presented with symptoms of a right L2 radiculopathy after a kyphoplasty procedure. Computed tomography imaging showed leakage of the kyphoplasty cement into the neural foramen above and medial to the right L2 pedicle. A transforaminal endoscopic surgi

Unruptured aneurysm of the left sinus of Valsalva compressing the left main coronary artery: successful percutaneous treatment.

Aneurysm of the left sinus of Valsalva is an extremely rare entity. It may be asymptomatic and incidentally discovered, or may be symptomatic and manifest acutely with compression of adjacent cardiac structures. Encasement of the left main coronary artery by such an aneurysm is a recognized but infrequent complication that can lead to severe coronary insufficiency. Surgical decompression of the left main coronary artery is the standard treatment for such conditions. We describe a patient presenting with extrinsic compression of the left main coronary artery by a large unruptured aneurysm of th

Percutaneous cholecystostomy: the radiologist's role in treating acute cholecystitis.

Acute cholecystitis is a common condition, with laparoscopic cholecystectomy considered the gold-standard for surgical management. However, surgical options are often unfavourable in patients who are very unwell, or have numerous medical co-morbidities, in which the mortality rates are significant. Percutaneous cholecystostomy (PC) is an image-guided intervention, used to decompress the gallbladder, reducing patient's symptoms and the systemic inflammatory response. PC has been shown to be beneficial in high-risk patient groups, predominantly as a bridging therapy; allowing safer elective chol

Preoperative detection of the neurovascular relationship in trigeminal neuralgia using three-dimensional fast imaging employing steady-state acquisition (FIESTA) and magnetic resonance angiography (MRA).

Microvascular decompression is an accepted treatment for trigeminal neuralgia (TN). Preoperative identification of neurovascular compression, therefore, could aid determination of the appropriate treatment for TN. To preoperatively visualize the neurovascular relationship, three-dimensional fast imaging employing steady-state acquisition (3D FIESTA) and magnetic resonance angiography (MRA) were performed on 37 patients with TN in our study. 3D FIESTA in combination with MRA identified surgically verified neurovascular contact in 35 of 36 symptomatic nerves. The offending vessel (artery or vein

Dorsal laminectomy in a two-toed amphiuma (Amphiuma means).

An adult male two-toed amphiuma (Amphiuma means) was evaluated for kyphosis and a self-inflicted ulcerative dermal lesion at the site of a midbody spinal deformity. Radiology revealed a proliferative bony lesion at the 39th vertebra. A dorsal laminectomy was performed to alleviate pressure on the spinal cord believed to be secondary to bone remodeling. Resolution of the scoliosis occurred at the surgical site, and the ulcerative lesion of the body wall healed. Three weeks postoperatively, the amphiuma resumed self-mutilation caudal to the surgery site. Euthanasia was elected. Necropsy revealed

Lipofibromatous hamartoma of the median nerve.

Lipofibromatous hamartoma is a rare tumour of peripheral nerves which is characterised by an excessive infiltration of the epineurium and perineurium by fibroadipose tissue. To the best of our knowledge, only approximately 88 cases are reported in the literature. We report a rare case of lipofibromatous hamartoma of the median nerve causing secondary carpal tunnel syndrome in a 25 year old patient. This patient was treated conservatively with decompression and biopsy and experienced a complete resolution of symptoms post-operatively. Magnetic resonance imaging may be used to diagnose this lesi

Malunited calcaneal fracture fragments causing tarsal tunnel syndrome: a rare cause.

This is a report of tarsal tunnel syndrome (TTS) due to a specific malunited calcaneal fracture fragment in a 46-year-old man. He was treated non-operatively for extra-articular calcaneal fracture. Four months later he presented with pain, tingling and hypoaesthesia over the medial aspect of the heel. He had a positive Tinel's sign and a positive dorsiflexion-eversion test. Radiography revealed malunited calcaneal fracture along medial wall producing bony prominence. The tarsal tunnel was surgically decompressed by excising the malunited fragments. The branches of the posterior tibial nerve we

Spinal hydatid disease and its neurological complications.

Bone infection is estimated to represent 0.5-2.5% of cases of hydatid disease. Of these cases, the spine is affected in about 50%. 11 cases of histologically confirmed spinal hydatid disease treated surgically in 2 institutions in Spain and Lebanon were followed up clinically and radiologically for an average of 20 y (1-34 y). On presentation, all patients had severe neurological deficits. All patients underwent posterior spinal decompression, curettage, drainage and washout of paravertebral cyst cavities. Over the period of follow-up, patients underwent an average 4.8 spinal surgical procedur

[Central nervous system granulocytic sarcoma after bone marrow transplant: case report].

Granulocytic sarcoma is a solid tumor, composed by granulocytic precursor cells at various levels of differentiation, located at an extra-medullary site. It is associated with acute myeloid leukemia, and its presence reveals a bad prognostic factor. The treatment usually consists of radiotherapy and chemotherapy. A case of an intracranial granulocytic sarcoma occurring six months after a bone marrow transplant in a patient with acute myeloid leukemia is reported. The patient presented with headache and left hemiplegia caused by a large fronto-parietal lesion with significant mass effect. After

Isthmic spondylolisthesis.

Isthmic spondylolisthesis, which is demonstrated in 4 to 8% of the general population, is one of the most common types of spondylolisthesis. The three subtypes of this condition all manifest some variation of a pars interarticularis defect as a result of recurrent injury to that structure. A multifactorial origin is postulated for this disease; mechanical, hereditary, and hormonal factors are believed to play a role. Presenting signs and symptoms may include those referable to neurological compromise or those related to the spinal deformity. The majority of patients with spondylolysis and spon

Downbeat nystagmus with compression of the medulla oblongata by the dolichoectatic vertebral arteries.

A case is reported of downbeat nystagmus associated with compression of the caudal brain stem by the dolichoectatic vertebral arteries, which was revealed by magnetic resonance imaging. Before surgery, the patient complained of oscillopsia at rest. Downbeat nystagmus was observed in the primary eye position and was increased in lateral gaze, especially to the left. Surgical repositioning of the left vertebral artery succeeded in resolving the downbeat nystagmus in the primary position within 1 month, and the nystagmus in all the other gaze positions almost completely disappeared over the subse

Evoked electromyography in Bell's palsy: a clinically useful test?

One hundred and forty-four patients presenting with a Bell's palsy were seen at Sunnybrook Medical Centre, University of Toronto, between the years 1978 and 1985. Only patients with a complete paralysis and not treated with either steroids or surgical decompression of the facial nerve were included in the study. The results of electrical tests performed on days 8, 9 or 10 after the onset of paralysis in 23 patients were used to predict clinical recovery at 12 weeks, six months and 12 months. There was good statistical correlation for values of conduction latency and amplitude with good clinica

New techniques for decompression of lumbar spinal stenosis.

Central and lateral stenoses of the lumbar neural canal are relatively less common causes of chronic pain in the lumbar back and the legs. However, their incidence is much higher than was previously thought. Accurate anatomical diagnosis was difficult until the advent of high resolution computed tomographic scanning. Appropriate decompressive surgery can now be designed and "guided" by the scans and the clinical syndrome. New or highly modified surgical procedures are now being used with good to excellent clinical results in as high as 80% of the cases depending upon the technique chosen. Many

Biomechanical comparison of unilateral and bilateral pedicle screws fixation for transforaminal lumbar interbody fusion after decompressive surgery--a finite element analysis.

BACKGROUND: Little is known about the biomechanical effectiveness of transforaminal lumbar interbody fusion (TLIF) cages in different positioning and various posterior implants used after decompressive surgery. The use of the various implants will induce the kinematic and mechanical changes in range of motion (ROM) and stresses at the surgical and adjacent segments. Unilateral pedicle screw with or without supplementary facet screw fixation in the minimally invasive TLIF procedure has not been ascertained to provide adequate stability without the need to expose on the contralateral side. Functional Outcomes After Decompressive Craniectomy.

Decompressive craniectomy is a surgical procedure performed to manage increased intracranial pressure after traumatic brain injury or stroke. The timing of this procedure, patient age, preoperative Glasgow Coma Scale score, and size of the lesion have been used to predict functional outcomes for patients. Much of the literature also focuses on mortality rates and broad functional outcome categories. There is a need for more information regarding the specific physical and cognitive impairments that patients experience as well as studies that evaluate long-term functional outcomes beyond 1 year.

Fully endoscopic cervical spine surgery: What does the future hold?

Fully endoscopic cervical spine surgery is an emerging novel approach to address cervical spinal pathology. Techniques, both anterior and posterior have been adapted to address various cervical pathologies. The primary goal of these procedures like other open techniques is to surgically decompress the canal centrally and/or along the foramen. The narrative review aims to provide the reader an overview of the rapidly advancing field of endoscopic cervical spinal surgery and evaluate whether these newer approaches could potentially reduce the cost and the risk associated with instrumented cervic
