Navigating Targeted Therapeutics in Dermatology: Biologics and Small Molecules.

Dermatology is entering an exciting era with new, targeted immune-modulating medications for treating a variety of dermatologic conditions including psoriasis, atopic dermatitis (AD), and hidradenitis suppurativa. Previously, mainstay treatments consisted of topical corticosteroids or broad systemic immunosuppressants. Recently, our understanding of cytokine signaling cascades has grown, presenting new opportunities to target skewed immune responses. Two major classes are biologics and small molecules. Herein, we highlight the similarities and differences between these two categories of target

Comorbidities in dermatology.

Recently, comorbidities have been rediscovered in dermatology. Although numerous associations between skin diseases and other conditions have been reported, only a few are well documented. The association of comorbidities and dermatoses is complex and multifactorial. Life-style factors, impaired health-related quality of life, depression, therapeutic interventions, and several biases may confound the relationship between skin diseases and comorbidities. This article discusses observational studies that assess comorbidities in psoriasis, atopic dermatitis, vitiligo, and nonmelanoma skin cancer,

Defective monocyte oxidative metabolism in a child with Smith-Lemli-Opitz syndrome.

We present a patient with Smith-Lemli-Opitz syndrome with immunodeficiency. The patient suffered numerous infectious episodes, atopic dermatitis and wheezing. Immunological investigations demonstrated severely reduced oxidative burst-responsiveness of the blood monocytes, whereas chemotaxis, phagocytosis and interleukin-1 production were normal. Tests of neutrophils and lymphocytes were normal excluding previously described immune deficiency disorders. The father proved to have diminished monocyte oxidative metabolism as well, whereas the mother had normal monocyte function. The genetic and im

Systemic Interventions Targeting Epidermal Barrier Function: Translational Insights from Oral Probiotics.

The epidermal barrier is increasingly recognized as a dynamic and pharmacologically targetable system that extends beyond its traditional role as a passive structural defense. In inflammatory skin diseases such as atopic dermatitis and psoriasis, barrier dysfunction contributes not only to increased permeability and environmental susceptibility but also to complex bidirectional interactions involving immune dysregulation, microbial imbalance, oxidative stress, and systemic inflammation. Emerging evidence further supports the concept of the gut-skin axis as a systemic regulatory network linking

Dupilumab and lymphoma risk among patients with asthma: a population-based cohort study.

BACKGROUND: Dupilumab is approved for the treatment of atopic dermatitis, asthma, and other allergic diseases. Recent studies suggest that patients with atopic dermatitis receiving dupilumab are at a higher risk of developing cutaneous lymphoma. This study aimed to investigate the risk of lymphoma among patients with asthma receiving dupilumab. METHODS: This population-based cohort study included patients in the United States with asthma who initiated dupilumab or the active comparator (combination therapy with inhaled corticosteroids (ICS) plus long-acting β-agonists (LABA), or ICS/LABA), be

Shared loci but distinct variants underlie genetic architecture of allergic diseases.

Asthma, allergic rhinitis, and atopic dermatitis are common, complex traits that are frequently co-morbid and have strong genetic correlation. However, the extent to which genome-wide genetic correlation between traits reflects shared causal variants or risk genes remains unclear. To address this question, we used functional fine-mapping. We generated genomic annotations from primary cells treated with immunomodulatory stimuli, then used these data to identify likely causal variants mediating genetic risk for allergic diseases including adult-onset asthma, childhood-onset asthma, allergic rhin

Bidirectional two-sample mendelian randomization analysis identifies a causal relationship between major depressive disorder and allergic diseases.

BACKGROUND: Several studies have indicated an association between major depressive disorder (MDD) and allergic diseases (ADs), but the exact causal relationship remains inconclusive. Thus, this study aimed to explore the causal relationship between MDD and ADs employing bidirectional two-sample Mendelian randomization (MR). METHOD: The summary statistics for MDD were sourced from the Psychiatric Genomics Consortium. Single nucleotide polymorphisms (SNPs) associated with allergic asthma (AAS), allergic rhinitis (AR), and atopic dermatitis, were extracted from the FinnGen Consortium. The invers

Efficacy and safety of dupilumab in chronic hand eczema: a systematic review.

Chronic hand eczema (CHE) significantly impacts patients' quality of life and remains challenging to manage, especially in topical steroid-refractory cases. Dupilumab, a monoclonal antibody that targets the interleukin-4 receptor alpha, has demonstrated efficacy in moderate-to-severe atopic dermatitis and various other type 2 inflammatory conditions. This systematic review evaluates the effectiveness and safety of dupilumab in the treatment of CHE. A comprehensive search was performed of the Ovid MEDLINE and Embase databases from their inception until March 2024, identifying 20 studies, includ

The Influence of a Plant-Based Diet on Skin Health: Inflammatory Skin Diseases, Skin Healing, and Plant-Based Sources of Micro- and Macro-Nutrients.

Dietary patterns have been shown to worsen or alleviate several dermatological diseases. A well-balanced, plant-based diet is known to have anti-inflammatory, probiotic, and antioxidant properties, along with weight loss-promoting effects. Moreover, a plant-based diet has a low glycemic load, improving metabolic disease. Due to these qualities, plant-based diets may have beneficial effects on inflammatory skin conditions. In this review, we aim to discuss the possible mechanisms by which a plant-based diet reduces disease severity in psoriasis, acne, hidradenitis suppurativa, and atopic dermat

The associations between gut microbiota and inflammatory skin diseases: a bi-directional two-sample Mendelian randomization study.

BACKGROUND: Accumulating evidence shows that dysregulation of intestinal flora is associated with inflammatory skin diseases, specifically atopic dermatitis (AD), psoriasis (PSO), and rosacea (ROS). However, the causality is still unclear. OBJECTIVES: To study the underlying causality between gut microbiota (GM) and AD, PSO, and ROS, a bi-directional two-sample Mendelian randomization (2SMR) analysis was conducted. METHODS: Summary statistics of gut microbiota, AD, PSO, and ROS were extracted from large-scale genome-wide association studies (GWASs). In 2SMR analysis, in addition to the inver

The Role of VD/VDR Signaling Pathway in Autoimmune Skin Diseases.

BACKGROUND: Immune-related cutaneous diseases are a series of disorders, such as alopecia areata, psoriasis, atopic dermatitis, systemic lupus erythematosus and autoimmune bullous dermatoses. Vitamin D is a fat-soluble vitamin, which is known for its classical pleiotropic effect. Recent studies have found that vitamin D, after catalyzed into its biologically active form [1,25(OH) 2D], correlated with its receptor, vitamin D receptor, plays a vital role in multiple pathophysiological processes, including immune-related dermatoses. This review mainly summarizes evidence on the role of vitamin D/

Single-cell RNA sequencing reveals dysregulated fibroblast subclusters in prurigo nodularis.

Prurigo nodularis (PN) is an intensely pruritic, chronic inflammatory skin disease that disproportionately affects black patients. However, the pathogenesis of PN is poorly understood. We performed single-cell transcriptomic profiling, ligand receptor analysis and cell trajectory analysis of 28,695 lesional and non-lesional PN skin cells to uncover disease-identifying cell compositions and genetic characteristics. We uncovered a dysregulated role for fibroblasts (FBs) and myofibroblasts as a key pathogenic element in PN, which were significantly increased in PN lesional skin. We defined seven

Predictors of adherence to Narrowband ultraviolet B first-month treatment dosage plan.

BACKGROUND: The adherence to a narrowband ultraviolet B (NB-UVB) treatment plan is derived, in large part, from the patient's skin tolerance to the phototherapy dose. At present, the initial and first-month incremental phototherapy doses are determined prior to treatment initiation based on the patient's Fitzpatrick skin phototyping. OBJECTIVES: To identify variables that predict adherence to NB-UVB first-month treatment dosage plan. METHODS: Charts of 1000 consecutive patients receiving NB-UVB at a hospital-based phototherapy unit were retrospectively analyzed. We included patients receivin
