Subjective signs in premotor epilepsy: confirmation by stereo-electroencephalography.

Subjective manifestations inaugurating frontal seizures are less well known than those observed in temporal seizures. We report eleven consecutive patients who underwent surgery for premotor epilepsy. Six of them had focal cortical dysplasia. Ictal symptomatology was analysed to establish electroclinical correlations. The localisation of the epileptogenic zone was assessed by stereoelectroencephalographic studies. Subjective manifestations were described in all cases, more frequently in a sensory rather than an emotional or psychological fashion. Focal seizures limited to subjective features w

Different adaptations in ventral tegmental area dopamine neurons in control and ethanol exposed rats after methylphenidate treatment.

BACKGROUND: Methylphenidate (MPH) is a psychostimulant effective in treating attention-deficit/hyperactivity disorder (ADHD). Repeated MPH treatment may increase substance abuse risk because of adaptations in dopaminergic (DA) function associated with sensitization to subsequent stimulant exposure. However, this possibility is based on observations in normal animals and may not apply to animals with attention problems linked to compromised DA function such as prenatal ethanol exposed (PE) animals. METHODS: The electrical activity of ventral tegmental area (VTA) DA neurons was studied after th

Is intraoperative electrocorticography reliable in children with intractable neocortical epilepsy?

PURPOSE: To study the relation between the spike frequency during intraoperative electrocorticography (ECoG) under general anesthesia with isoflurane and that during extraoperative ECoG monitoring in children with intractable neocortical epilepsy. METHODS: Twenty-one children (age, 1-16 years; 15 boys and six girls) who underwent intraoperative and extraoperative ECoG monitoring with subdural electrode arrays were studied. The spike frequency and the spatial pattern of spike frequency were compared between intraoperative and extraoperative ECoGs for each patient (by using Wilcoxon signed-rank

From hypothalamic hamartoma to cortex: what can be learnt from depth recordings and stimulation?

Patients having a hypothalamic hamartoma (HH) frequently present gelastic or dacrystic seizures, and they often later experience multiple additional seizure types which lead to a severe epileptic encephalopathy. There is now increasing evidence that the HH itself plays a crucial role in this syndrome, but the relationships between the lesion and the different types of seizures remain a questionable issue. Stereotactic intracerebral EEG recordings were performed in 5 patients suffering from a medically intractable epilepsy associated with a HH. The hamartoma was investigated in all cases, and v

Anatomical, physiological, and pharmacological characteristics of histidine decarboxylase knock-out mice: evidence for the role of brain histamine in behavioral and sleep-wake control.

The hypothesis that histaminergic neurons are involved in brain arousal is supported by many studies. However, the effects of the selective long-term abolition of histaminergic neurons on the sleep-wake cycle, indispensable in determining their functions, remain unknown. We have compared brain histamine(HA)-immunoreactivity and the cortical-EEG and sleep-wake cycle under baseline conditions or after behavioral or pharmacological stimuli in wild-type (WT) and knock-out mice lacking the histidine decarboxylase gene (HDC-/-). HDC-/-mice showed an increase in paradoxical sleep, a decrease in corti

Usefulness and pitfalls of intraoperative spinal motor evoked potential recording by direct cortical electrical stimulation.

OBJECTIVE: The aim of this study is to investigate the usefulness and problems with spinal motor evoked potential (MEP) recording, especially the reasons for failed recording. We report our personal experience over the last 8 years in patients with lesions adjacent to the primary motor cortex. METHODS: MEP records of 50 consecutive patients were retrospectively reviewed. MEP was recorded by a catheter electrode inserted in the cervical epidural space. Stimulation electrodes were placed on the cortical surface during surgery. SEP recording was also performed in 29 of 50 patients. RESULTS: MEP

Long-term outcome of patients with multiple (> or = 3) noninfected transvenous leads: a clinical and echocardiographic study.

To prospectively assess the incidence and clinical significance of thromboembolic complications in patients with multiple (> or = 3) noninfected transvenous leads; 48 consecutive patients were evaluated. Half of the patients had two ventricular leads and one atrial lead, 15 patients had two atrial leads and one ventricular lead, while 9 patients had two ventricular and two atrial leads. No additional care was provided except for aspirin (80 mg bid) and annually performed echo-Doppler studies. Clinical follow-up included signs and symptoms of subclavian and/or axillary vein thrombosis, the pres

Frontal lobe seizures: electroclinical syndromes.

To define further the electroclinical manifestations of frontal lobe epilepsy (FLE), we studied 150 seizures manifested by 24 patients; 18 patients had subdural electrode arrays (SEA). The findings in these patients clearly overlapped presumably reflecting the interconnections between functionally related frontal zones; yet the manner in which the symptoms clustered and the sequence in which they occurred generally indicated the anatomic site of the epileptogenic zone. We divided the patients into three major groups: (a) those with supplementary motor seizures, (b) those with focal motor seizu

A comparison of gastro-oesophageal reflux in volunteers assessed by ambulatory pH and gamma monitoring after treatment with either Liquid Gaviscon or Algicon Suspension.

This study was designed to compare the effectiveness of single doses of either Liquid Gaviscon or Algicon Suspension in the suppression of food and acid reflux into the oesophagus after a test meal in volunteers. After the pH electrode and gamma detector were positioned 5 cm above the cardia, the volunteers received a refluxogenic radiolabelled meal. The subjects then remained untreated, or thirty minutes later they were given a dose of unlabelled Algicon Suspension, or Liquid Gaviscon and a recording was made for a minimum of 3 hours. Allocation to the treatment group was randomized with the

Perioperative use of temporary diaphragm pacing in surgical patients with unilateral diaphragm dysfunction to decrease mechanical ventilation.

BACKGROUND: Unilateral diaphragmatic dysfunction, most commonly from phrenic nerve injury, is associated with difficult ventilator weaning and increased postoperative pulmonary complications. Diaphragm pacing prevents ventilator-induced diaphragm dysfunction and supports ventilation during periods of perioperative vulnerability. We report perioperative utilization of temporary diaphragm pacing in patients with pre-existing or anticipated postoperative unilateral diaphragmatic dysfunction. METHODS: This cohort analysis was derived from a prospective, institutional review board-approved databas

More severe joint disease and lower patient oxygenation are associated with less corrosive in vivo synovial fluid in patients with knee osteoarthritis.

AIMS: Electrochemical properties of synovial fluid are variable among patients and can lead to implant corrosion, negatively impacting their longevity. The purpose of this study was to explore the relationships between electrochemical properties of synovial fluid of knee osteoarthritic (OA) patients undergoing total knee arthroplasty (TKA) and their clinical and demographic data. METHODS: Knee OA patients undergoing TKA were enrolled in this study, and samples of their synovial fluid were collected during surgery and immediately injected into a three-electrode electrochemical cell to measure

Post Pacing Interval Following Failed Anti-Tachycardia Pacing Predicts Epicardial Ventricular Tachycardia Circuits in Patients With Non-Ischemic Cardiomyopathy.

BACKGROUND: Anti-tachycardia pacing (ATP) that entrains but fails to terminate ventricular tachycardia (VT) provides an opportunity to use post-pacing interval minus tachycardia cycle length (PPI - TCL) as a diagnostic tool. We evaluated whether PPI - TCL from failed ATP could predict the presence of epicardial re-entrant VT circuits in patients with ischemic cardiomyopathies (ICM) vs. non-ischemic cardiomyopathies (NICM). METHODS: A retrospective analysis was conducted of 51 patients who underwent VT ablation at Beth Israel Deaconess Medical Center from January 2020 to March 2025 and had fai

Narrative Review on Vestibular Complaints After Cochlear Implantation in Adults: Defining Heterogeneous Common Symptoms.

Cochlear implantation (CI) effectively restores hearing across the whole lifespan but may be followed by vestibular complaints, especially in adult recipients. The aim of this narrative review is to provide a comprehensive characterization of vestibular complaints after CI in adults, collecting clinical and instrumental data, as well as discussing the risk factors for their development. From data reported in the literature, we defined five recurring clinical presentations of postoperative vestibular disturbances (phenotypes): acute postoperative vestibular syndrome, benign paroxysmal positiona

The role of an ultrasound-responsive injectable piezoelectric hydrogel in promoting nerve regeneration and alleviating neuropathic pain.

BACKGROUND: Neuropathic pain (NP) resulting from peripheral nerve injury (PNI) represents a major clinical challenge. Although electrical stimulation (ES) has demonstrated therapeutic benefits, its clinical translation is limited by the invasiveness of electrode implantation, which often requires large surgical incisions and percutaneous wiring, increasing tissue damage and infection risk. METHODS: This study developed an ultrasound-responsive, injectable, and biodegradable piezoelectric hydrogel (sPLLA-Gel) composed of electrospun poly-L-lactic acid (PLLA) and methacryloyl gelatin (GelMA). Prevalence of Temporal Lobe Epilepsy (TLE) Subtypes and Response to Resective Surgery in Patients with Presumed TLE Undergoing Limbic and Paralimbic Network Exploration with Stereo-Electrodes.

Background/Objectives: Temporal lobe epilepsy (TLE) responds well to surgical treatment, although a considerable percentage of patients experience seizure recurrence after resection. Relapse from the contralateral mesial temporal lobe, extratemporal lobe epilepsy mimicking TLE, or temporal plus epilepsy might account for surgical failures. Methods: We included patients with a pre-implantation hypothesis suggesting TLE, who underwent stereo-EEG (SEEG) evaluation at our institution and had an individual SEEG exploration paradigm with at least twelve stereo-electrodes placed to sixteen brain regi

Deep Brain Stimulation: Overview and Applications in the Context of Neuropsychiatric Conditions.

Deep brain stimulation (DBS) is a neuromodulatory therapy involving the implantation of electrodes into brain structures in order to help normalize brain activity in a variety of neurological disorders. The mechanisms of DBS operate at micro-, meso-, and macroscale levels to influence neuronal signaling, synaptic reorganization, and network-wide connectivity between brain regions. Recent advances in connectomics and sensing technologies have allowed for more precise and adaptive stimulation strategies, increasing the potential to target complex, heterogeneous neuropsychiatric conditions. Electrical and mechanical interventricular dyssynchrony coupling in patients with bradycardia: A UHF-ECG validation trial.

BACKGROUND: Ultra-high-frequency electrocardiography (UHF-ECG) is a noninvasive tool visualizing the ventricular activation sequence. It was never compared with other methods of dyssynchrony assessment in patients with bradycardia. OBJECTIVE: We aimed to compare UHF-ECG interventricular electrical dyssynchrony (e-DYS) with interventricular mechanical delay (IVMD) measured by echocardiography in patients receiving right ventricular pacing (RVP) or conduction system pacing (CSP). METHODS: Fifty-three patients with advanced atrioventricular conduction disease and preserved ventricular systolic

A pre-existing chronic Toxoplasma gondii infection promotes epileptogenesis and neuropathology in a mouse model of mesial temporal lobe epilepsy.

OBJECTIVE: There is initial evidence that the common neurotropic parasite Toxoplasma gondii is a risk factor for the development of epilepsy; however, whether it influences epileptogenesis is unknown. This study investigated whether a pre-existing chronic T. gondii infection alters epileptogenesis and neuropathology in a mouse model of mesial temporal lobe epilepsy. METHODS: Male and female C57BL/6Jax mice were intraperitoneally administered T. gondii tachyzoites or vehicle control. After 6 weeks, mice underwent self-sustained electrical status epilepticus (SSSE) through an implanted bipolar
