The German National Strategy for Gene- and Cell-Based Therapies: Generating Impact by Employing a Novel Multi-Stakeholder Approach.

Gene- and cell-based therapies (GCTs) represent a disruptive and transformative class of biomedical innovations. They address diseases by adding, removing, repairing, or replacing genes and/or by endowing distinct living cells with additional biological functions. Through this plethora of options, numerous conditions-including genetic disorders, cancers, and degenerative diseases-have become potential targets for a curative therapy. Thus, GCTs are considered the "Future of Medicine" as they (i) offer a potential cure, particularly for rare and severe disorders previously considered untreatable

A Cross-Sectional Study on the Quality of Life of Caregivers of Children With Disability in Chennai, India.

Background Disability influences a person's quality of life (QoL) in social, psychological, physical, and environmental dimensions. The caregivers of the disabled person equally share the burden, and their QoL is also greatly affected, as caregivers face substantial challenges in multiple dimensions. This limits their ability to focus their attention or time elsewhere, negatively impacting their social life and overall QoL. Consequently, understanding the QoL of caregivers, the variety of strategies they used to cope, and their mental health status, particularly depression, is crucial for prov

"To make a positive difference": diverse youth perspectives on getting involved in mental health research.

BACKGROUND: Mental health problems amongst young people in the UK have been rising, yet many still do not receive appropriate support. Involving young people in research can lead to more inclusive services. However, certain groups of young people, including those from ethnic minority backgrounds, lower-income families, refugee, asylum-seeking and other immigrant backgrounds, non-native English speakers, disabled young people and those with long term health conditions remain under-represented in involvement in mental health research. Little research explores the perspectives of these young peop

Risk and Dose-Response Relationship for Pentosan Polysulfate Sodium Maculopathy: A Systematic Review and Meta-Analysis.

TOPIC: To determine the pooled relative risk (RR) of pentosan polysulfate sodium maculopathy (PPSM) in patients using pentosan polysulfate sodium (PPS) and model the dose-response relationship of this association from existing literature. CLINICAL RELEVANCE: Pentosan polysulfate sodium maculopathy is an acquired, progressive retinal pigmentary disease associated with oral PPS use. Although several observational studies suggest a dose-response relationship of this association, to date, there remains no literature-pooled synthesis on the risk of PPSM across strata of cumulative dose. METHODS:

Addressing the HIV/AIDS investment gap through stronger public financial management systems: a human-centered approach.

BACKGROUND: UNAIDS estimated that US$29 billion will be required by 2025 to meet HIV/AIDS service demands, with 53 percent expected to come from domestic sources. The PEPFAR-funded, USAID-implemented Sustainable Financing Initiative for HIV/AIDS (SFI), starting in 2014, supported domestic resources mobilization efforts and activities to strengthen countries' public financial management (PFM) systems, positively contributing to much-needed increase in domestic resources for health and HIV. PROGRAM APPROACH: SFI was implemented in 12 countries, supporting activities to build the capacity of gov

Willingness-to-pay for community-based health insurance among formal and informal doctors in Dhaka, Bangladesh: a comparative cross-sectional study.

INTRODUCTION: Health insurance enables financial protection and reduces medical burdens. Currently, less than 1% of Bangladeshi citizens have health insurance coverage. We aimed to assess the willingness-to-pay for community-based health insurance and explore associated factors among formal and informal doctors in Bangladesh. METHODS: This comparative cross-sectional study was conducted on Bangladeshi doctors with formal and informal employment. For each group, 116 doctors were selected via purposive sampling, as no national register of doctors was available, indicating their current employme

Barriers and facilitators of healthcare access for long COVID-19 patients in a universal healthcare system: qualitative evidence from Austria.

BACKGROUND: Long COVID-19 challenges health and social systems globally. International research finds major inequalities in prevalence and healthcare utilization as patients describe difficulties with accessing health care. In order to improve long-term outcomes it is vital to understand any underlying access barriers, for which relevant evidence on long COVID-19 is thus far lacking in a universal healthcare system like Austria. This study aims to comprehensively identify access barriers and facilitators faced by long COVID-19 patients in Austria and explore potential socioeconomic and demogra

Decision regret among couples experiencing infertility: a mixed methods longitudinal cohort study.

BACKGROUND: Decisions for how to resolve infertility are complex and may lead to regret. We examined whether couples and individuals who sought a consultation from a reproductive specialist for infertility later expressed decisional regret about their family-building choices and whether regret was associated with parental role, family-building paths, or outcomes. METHODS: This longitudinal mixed methods study included women and their partners who completed a questionnaire prior to their initial consultation with a reproductive specialist and 6 years later. The six-year questionnaire included

Is there a renoprotective value to leukodepletion during heart valve surgery? A randomized controlled trial (ROLO).

BACKGROUND: Acute Kidney Injury (AKI) adversely affects outcomes after cardiac surgery. A major mediator of AKI is the activation of leukocytes through exposure to the cardiopulmonary bypass circuit. We evaluate the use of leukodepletion filters throughout bypass to protect against post-operative AKI by removing activated leukocytes during cardiac surgery. METHODS: This is a single-centre, double-blind, randomized controlled trial comparing the use of leukodepletion versus a standard arterial filter throughout bypass. Elective adult patients undergoing heart valve surgery with or without conc

Scaling a waterfall: a meta-ethnography of adolescent progression through the stages of HIV care in sub-Saharan Africa.

INTRODUCTION: Observational studies have shown considerable attrition among adolescents living with HIV across the "cascade" of HIV care in sub-Saharan Africa, leading to higher mortality rates compared to HIV-infected adults or children. We synthesized evidence from qualitative studies on factors that promote or undermine engagement with HIV services among adolescents living with HIV in sub-Saharan Africa. METHODS: We systematically searched five databases for studies published between 2005 and 2016 that met pre-defined inclusion criteria. We used a meta-ethnographic approach to identify fir

Institutional and patient criteria for heart-lung transplantation.

Heart-lung transplantation is a complex, expensive, and resource-intensive procedure. It is performed more often where there is coexistence of end-stage pulmonary disease with advanced cardiac disease that is not secondary to pulmonary hypertension (e.g., some forms of congenital heart disease, some situations with Eisenmenger's syndrome, and possibly the coexistence of end-stage heart and end-stage lung disease). Available data indicate that some disease states are associated with a higher probability of successful outcome after HLT than are others. Objectively validated and reliable patient

Treatment-resistant and multi-therapy-resistant criteria for bipolar depression: consensus definition.

BACKGROUND: Most people with bipolar disorder spend a significant percentage of their lifetime experiencing either subsyndromal depressive symptoms or major depressive episodes, which contribute greatly to the high levels of disability and mortality associated with the disorder. Despite the importance of bipolar depression, there are only a small number of recognised treatment options available. Consecutive treatment failures can quickly exhaust these options leading to treatment-resistant bipolar depression (TRBD). Remarkably few studies have evaluated TRBD and those available lack a comprehe

Predictors of Intention to Stay in Healthcare Among Healthcare Students and Professionals in Czechia and Slovakia.

BACKGROUND: Workforce stability remains a major challenge for sustainable healthcare systems. Retention research increasingly distinguishes between organizational conditions, professional identification, and mobility or place-related preferences. However, less is known about how these factors operate within closely related Central European contexts and in samples that include respondents connected to healthcare education and practice. This study examined whether workplace relationships and leadership, professional commitment, and place-related mobility preferences predict intention to stay in

Developing an information systems strategy for nursing.

With the rapidly changing health care environment and information technology advances, organizations need to engage in strategic, planned change in order to allocate limited resources, achieve the organization's goals, and fulfill its mission [1]. One of the most important aspects of the organization's planned strategies for change concerns the information systems. The involvement of the nursing department in this process is critical. This poster presentation will communicate how nurses can develop an information systems strategic plan that will enable them to play an active role as contributo

Financing of adolescent health and wellbeing in Bangladesh, Ethiopia and Nigeria: a case study of the Global Financing Facility.

BACKGROUND: Adolescent health and wellbeing (AHW) remains largely underfunded with uneven progress across countries. This study investigates the Global Financing Facility for Women, Children, and Adolescents (GFF) in-country approach to AHW financing - including investment case prioritization, multisectoral stakeholder platforms, program implementation, monitoring and evaluation, and co-financing - examining what these processes have achieved for AHW, and the barriers limiting their impact, across Bangladesh, Ethiopia, and Nigeria. METHODS: This collective case study triangulates country data

Immunotherapy 3.0: Breakthroughs Steering the Next Generation of Cancer Control.

BACKGROUND: Hematologic disorders, including sickle cell disease, anemias, hemoglobinopathies, leukemias, lymphomas, and bleeding disorders, constitute a substantial and growing burden of morbidity and mortality across Africa. Structural health system constraints-limited diagnostic capacity, shortages of trained hematology specialists, fragmented blood transfusion services, and inequitable access to essential medicines-continue to drive delayed diagnosis, suboptimal treatment, and poor outcomes. Despite these challenges, recent years have witnessed important advances in laboratory strengthenin

Cost Savings Modeling of Integrated Telehealth Services for Hepatitis C Virus Infection-Cherokee Nation, 2016-2020.

CONTEXT: American Indian and Alaska Native (AI/AN) people experience the highest rates of hepatitis C virus (HCV) infection and HCV-related mortality in the United States. However, access to HCV care remains limited in many tribal and rural communities. Telehealth models such as the Extension for Community Healthcare Outcomes (ECHO) have been used to expand access by supporting primary care providers in delivering complex treatments locally. OBJECTIVES: To assess the cost savings of implementing Cherokee Nation Health Services' (CNHS) HCV ECHO program compared with conventional, specialist-ba

The psychosocial impacts of skin-neglected tropical diseases (SNTDs) as perceived by the affected persons: A systematic review.

BACKGROUND: Neglected Tropical Diseases (NTDs) disproportionately affect marginalised groups within impoverished communities, conferring devastating physical, financial and psychosocial effects. Skin-NTDs (SNTDs) are uniquely stigmatising due to their visible nature, rendering affected individuals vulnerable to psychosocial risk and the associated decline in social participation, quality of life and mental health. In response to knowledge gaps identified by current global efforts for integrated control of SNTDs this review gathers existing evidence on the psychosocial effects of SNTDs, with co
