Partial long-stem resection Austin-Moore hip endoprosthesis in the treatment of metastases to the proximal femur.

Background. The proximal femur is a frequent location of bone metastases, and pathological fractures in this area are common. A pathological fracture and/or pain associated with bone destruction are indications for surgery. Among many methods of surgical treatment, partial resection and subsequent arthroplasty appears to be the most efficient. The low cost of the implant and the relatively simple surgical technique incline us to apply long-stem (305mm) partial Austin-Moore hip prosthesis in these cases. Material and methods. Between June 2003 and February 2005 we operated 31 patients (10 men a

[Four cases of hangman's fracture].

Four cases of hangman's fracture were reported. The cases were disturbed between 67 and 85 years of age and all were males. The exact mechanisms of injuries of these cases were not precisely documented, but all struck the head by falling for some distance. Three of them showed cerebral concussion immediately after injuries. One of four patients had complete transverse cord lesion at C5 level and the other showed right Brown-Séquard syndrome at C4 level, while another two showed no neurological deficits except for neck pain. Radiological findings disclosed fracture of the pedicles or the lamina

The traumatic acromion fracture: review of the literature, clinical examples and proposal of a treatment algorithm.

PURPOSE: Traumatic acromion fractures are rare and typically occur in patients with multiple fractures, which often delays diagnosis. Limited guidance exists on the treatment of these fractures. We present a review of the literature from the last 20 years and describe our experience in treating five patients-two conservatively and three with open reduction and internal fixations (ORIF). METHODS: We used the U.S. National Library of Science database, MEDLINE®, to search for all pertinent publications from January 1999 to December 2017. Included were retrospective or prospective studies, includ

Unstable Distal Radius Fractures Treated by Volar Locking Anatomical Plates.

INTRODUCTION: Fracture of the distal end of radius represents the most common fracture of the upper extremity accounting for 16-20% of all fractures. Plating is now emerging as the gold standard for management of distal radius fractures due to increased rate of complications such as malunion, subluxation/dislocation of distal radio-ulnar joint or late collapse of fracture. Procedures such as closed reduction and cast immobilization, ligamentotaxis with external fixator and percutaneous pin fixation are no longer acceptable. AIM: The purpose of the study was to evaluate the functional and radi

Treatment of fractures of the ulnar coronoid process.

OBJECTIVE: To discuss the classification, management and outcome of fractures of the ulnar coronoid process. METHODS: Retrospective analysis was carried out in 31 patients (19 men and 12 women of average age 29.8 years [range, 18-52 years]) with fractures of the ulnar coronoid process. The fractures were classified into four major groups based on the extent of injury to the ulnar coronoid process, the state of the anterior bundle of the ulnar collateral ligaments (UCL) and elbow stability. A fracture of the coronoid process less than halfway up was defined as type I (eleven cases); of the mid

[Fractures of the wrist with cartilage rupture in young adults. Apropos of 18 cases].

Distal radius impaction fractures are not rare. They present a dual problem for the surgeon: the difficulty of establishing the exact pathological anatomy of the fracture, and of obtaining stable anatomic reduction. The authors report their experience of 18 distal radius impaction fractures in young patients after violent trauma (motor vehicle accident). We subdivided these fractures using Kapandji and Müller's classifications: 12 type 9 or C3, 4 type 5 or B1 and 2 type 4 or C1. The mean depth of impaction of the scaphoid fossa or lunate fossa ("die punch fracture"), or centrally was measured

[Contrast roentgenography of the elbow joint in children].

The investigation of roentgenograms made after a preliminary contrast study of the elbow joint has shown that traumas may be followed by pathological changes of the cartilaginous tissue, elbow bursa and filling of the fossae of the olecranon and coronary processes by scarry and osseous tissue. The pathological changes mentioned above result in disturbances of the movement of the elbow joint.

Acute patellar dislocation in children and adolescents. Surgical technique.

BACKGROUND: The treatment of acute patellar dislocation in children is controversial. Some investigators have advocated early repair of the medial structures, whereas others have treated this injury nonoperatively. The present report describes the long-term subjective and functional results of a randomized controlled trial of nonoperative and operative treatment of primary acute patellar dislocation in children less than sixteen years of age. METHODS: The data were gathered prospectively on a cohort of seventy-four acute patellar dislocations in seventy-one patients (fifty-one girls and twent

Terrible triad injuries of the elbow.

Terrible triad injury of the elbow is a complex injury that is classically defined as elbow dislocation along with fractures of the coronoid process of the ulna and the radial head. The injury is usually associated with typical soft-tissue disruptions (with common involvement of the lateral collateral ligament complex, elbow joint capsule, as well as the common extensor and flexor-pronator tendons) that are best understood in the context of injury mechanism as well as the role and relevance of the various elbow stabilizers. The goals of this article are to review the pertinent anatomy, mechani

Misplaced incus: an unusual complication of a temporal bone fracture.

A 24-year-old man sustained significant injuries as a result of a motor bicycle accident. CT scans demonstrated multiple intracranial injuries including bilateral temporal bone fractures. On emergence from sedation, the patient reported right-sided hearing loss which was attributed to ossicular chain disruption after the incus was found to be in the external auditory canal. Unexpectedly, the initial conductive hearing loss resolved with conservative management and an ossicular chain reconstruction was not required. This is an unusual case of impressive long-term good hearing outcomes following

Concomitant osteomyelitis and avascular necrosis of the talus treated with talectomy and tibiocalcaneal arthrodesis.

The goal with Lisfranc fracture-dislocations is to regain joint congruity and reestablish midfoot stability to avoid debilitating posttraumatic arthrosis and chronic pain in the sensate patient. In the diabetic population, dense peripheral neuropathy and/or vascular disease are equally important and may alter the surgical approach to traumatic tarsometatarsal injuries. The initial diagnosis in the diabetic population may be delayed due to subtle radiographic findings and/or patient unawareness of trauma in the insensate foot. Failure to initiate treatment in the early stages of acute diabetic

Intravenous ketamine is as effective as midazolam/fentanyl for procedural sedation and analgesia in the emergency department.

The study compared the effectiveness of ketamine and midazolam/fentanyl as procedural sedation and analgesia agents for reduction of fractures and dislocated joints. Forty-one adult patients were enrolled by convenience sampling. They were randomized to receive ketamine or midazolam/fentanyl. Depth of sedation, pain score, procedural outcome and memory of the procedure were documented. The ketamine group had deeper sedation, but there was no statistical difference in other variables between the two groups. Three patients in the midazolam/fentanyl group had oxygen desaturation. Acute ulnar collateral ligament injury in the athlete.

The functional thumb is a necessity for successful athletic participation. It not only allows the athlete to manipulate athletic equipment but also allows the precise manipulation of objects in the athlete's hand. Injury to the thumb often negates power grip and finesse of the athletic hand. Injuries can range from fractures, to dislocations, to ligamentous injuries. The relatively unconstrained thumb metacarpophalangeal joint is particularly vulnerable to injury from an abduction moment to its distal segment. Such injuries occur from direct contact and with falls on equipment such as racquet

Trigeminal nerve injury following accidental airbag deployment and assessment with quantitative sensory testing.

High velocity motor vehicle accidents are associated with an increase in mortality rates and a significant number of facial injuries. Accidental deployment of airbags and the associated release of hot gases can result in both thermal and mechanical injuries. The more commonly reported maxillofacial injuries include temporomandibular joint fractures and dislocations, dental trauma, facial nerve paralysis, and other orofacial pain complaints. The following case report describes a patient with facial trauma from the accidental deployment of an airbag resulting in complaints consistent with a neur

[Pathology of injuries due to snow surfing].

The amount of injured snowboarders is increasing each year. The reason is due to the success of snowboarding for teenagers. Accidents occur more during the first hours of practice. Upper limb is the first location: fore-arm breaks and wrist fractures are very common. Elbow and shoulder dislocations also occur often for snowboarders. The location of most injuries for lower leg is the ankle; we found many ankle sprains and astragalus's breaks. The knee sprain is rare unlike alpine ski. Prevention has to be developed to give a better image of this sport, in fact the attitude of the participants i

Common elbow problems in the athlete.

Because of the popularity of sports participation, sports physical therapists must recognize in the athlete the many clinical conditions that occur about the elbow. The purpose of this paper is to present the most common elbow problems that an athlete may encounter and to provide information to facilitate recognition of elbow pathology. This information is essential before initiating treatment. An attempt is made to include sprains, strains, neuropathies, dislocations, fractures, contusions, vascular insults, and skin problems in the distal humerus, elbow, and proximal forearm of both the imma

[Magnetic resonance tomographic diagnosis of internal lesions of the shoulder following luxation. A comparison with arthroscopic findings].

Fifteen patients with a recurrent dislocation of the glenohumeral joint were preoperatively investigated by MRI. We created a special protocol with axial T1-, T2-, proton-density- and sagittal T1-weighted images. MRI findings such as defects of the glenoid labrum and Hill-Sachs compression fractures proved to be correct during surgery. In the chosen protocol the diagnostic imaging of rotator cuff lesions was not specific. In 10 cases lesions were seen but in only 5 patients did these findings prove to be correct during surgery. However, MRI imaging of glenohumeral joints is a reliable non-inva

Should We Use the Largest Femoral Head With the Smallest Acetabular Component in Primary Total Hip Arthroplasty?

BACKGROUND: There has been an increasing use of larger femoral head sizes to mitigate dislocation risk after total hip arthroplasty. Manufacturers have created thinner, highly cross-linked polyethylene (HXLPE) liners to maximize effective head size. We investigated whether there was a survivorship penalty if the largest head was chosen at the smallest compatible acetabular component size, and as a secondary aim, we analyzed whether there was a decreased dislocation risk. METHODS: We reviewed 1,860 posterior approach primary total hip arthroplasties with HXLPE from 2005 to 2021. Subjects were

Acute instability of the patella: is magnetic resonance imaging mandatory?

PURPOSE: Acute dislocations of the patella represent 2 to 3% of traumatic injuries of the knee. When patients are seen in the emergency department with a dislocated patella clinical diagnosis is often very evident. However, in cases of short-lived subluxation or dislocations that have reduced spontaneously, the diagnosis can be challenging. The aim of this study was to evaluate the utility of MRI in the evaluation of acute patellar instability. METHODS: This was a prospective study of 39 cases of acute patellar instability. The study protocol included a clinical examination by an emergency me
