No evidence of pulmonary hypertension revealed in an echographic evaluation of right-sided hemodynamics in hyperthyroid cats.

OBJECTIVES: Hyperthyroidism is a common endocrinopathy affecting middle-aged to elderly cats, with multisystemic repercussions. Hyperthyroid humans show decreased lung compliance and increased cardiac output with subsequent left heart failure leading to pulmonary capillary congestion. Prognosis worsens with the development of increased pulmonary vascular pressures (ie, pulmonary hypertension [PH]) in hyperthyroid humans. The effect of excess thyroid hormone concentration on pulmonary arterial hemodynamics is unknown in cats. Assessing pulmonary vascular pressures in veterinary medicine relies

Proteinuria versus albuminuria in 24-hour urine collection: prevalence and clinical outcome in non-hypoxemic adult patients with congenital heart disease.

Congenital heart disease (CHD) patients, especially cyanotic ones, usually have renal function impairment. However, little information exists in non-cyanotic CHD patients. The objective of this study is to determine renal failure in non-hypoxemic CHD patients by measuring the amount of protein and albumin released in urine over a 24-hour period and determining the glomerular filtration rate (GFR). Prospective study of consecutive outpatient non-hypoxemic CHD patients followed up in a single tertiary referral hospital. Demographic, clinical, blood test and 24-hour urine collection were recorded

Evidence-based Utilization of Noninvasive Ventilation and Patient Outcomes.

RATIONALE: Strong evidence supports use of noninvasive ventilation (NIV) for patients with respiratory distress from chronic obstructive pulmonary disease and heart failure (strong evidence conditions [SECs]). Despite unclear benefits of NIV for other causes of acute respiratory failure, utilization for conditions with weaker evidence is increasing, despite evidence demonstrating higher mortality for patients who suffer NIV failure (progression from NIV to invasive mechanical ventilation [IMV])) compared with being treated initially with IMV. OBJECTIVES: To determine the association of hospit

Survival After Orthotopic Heart Transplantation in Patients Undergoing Bridge to Transplantation With the HeartWare HVAD Versus the Heartmate II.

BACKGROUND: Our objective was to determine whether the choice of a HeartWare HVAD as opposed to a Heartmate II left ventricular assist device (HMII LVAD), impacts survival after heart transplantation after controlling for patient, donor, and center characteristics. METHODS: We queried the United Network for Organ Sharing (UNOS) database, which has recently made pretransplantation device duration available, for all adult patients undergoing bridge to transplantation (BTT) between January 2011 and March 2016. Recipient, donor, and transplant-specific characteristics were compared between patien

Association of proportional pulse pressure with cardiac function and complications in patients hospitalized with acute exacerbation of chronic heart failure.

BACKGROUND: Proportional pulse pressure (PPP), calculated as pulse pressure divided by systolic blood pressure, is a simple, non-invasive hemodynamic index. Previous studies suggest its potential role in assessing cardiac function and prognosis in heart failure (HF) patients, particularly in identifying low cardiac output. This study aimed to investigate the relationship between PPP and key cardiac functional indices (Cardiac Index [CI] and Cardiac Output [CO]) in patients with hospitalized with acute exacerbation of chronic heart failure (CHF). METHODS: This cross-sectional study included 80

Impactor Displacement as a Predictor of Thoraco-Abdominal Organ Injury: Comparison of Isolated Organ to Whole Body Tests.

PURPOSE: Body armor is used to protect the wearer from penetrating injuries. However, when the armor defeats the projectile, it deforms at high rates into the wearer, referred to as back face deformation (BFD). This deformation can cause a variety of superficial and internal injuries and consequently should be considered when designing body armor. However, current design standards do not adequately consider the effects of BFD, and new data are necessary to inform future design thresholds for BFD. The objective of this study was to develop blunt impact injury risk curves for in vitro thoraco-ab

Worsening Renal Function in Patients With Acute Heart Failure Undergoing Aggressive Diuresis Is Not Associated With Tubular Injury.

BACKGROUND: Worsening renal function (WRF) in the setting of aggressive diuresis for acute heart failure treatment may reflect renal tubular injury or simply indicate a hemodynamic or functional change in glomerular filtration. Well-validated tubular injury biomarkers, N-acetyl-β-d-glucosaminidase, neutrophil gelatinase-associated lipocalin, and kidney injury molecule 1, are now available that can quantify the degree of renal tubular injury. The ROSE-AHF trial (Renal Optimization Strategies Evaluation-Acute Heart Failure) provides an experimental platform for the study of mechanisms of WRF dur

Stress in cardiovascular diseases.

INTRODUCTION: Minor oral surgeries done under local anesthesia can be quite traumatic to the patient; thus, the use of conscious sedation can improve patient comfort, reduce stress levels, provide better pain management, smoother surgical experiences, and better recovery. Combinations of ketamine-dexmedetomidine and midazolam-pentazocine were used as conscious sedation regimens, and they have unique strengths and weaknesses. Evaluating the sedation effects of ketamine-dexmedetomidine versus midazolam-pentazocine in maxillofacial surgeries is crucial for refining anesthetic practices. Therefore

Ischemic Mitral Valve Regurgitation in Patients Undergoing Coronary Artery Bypass Grafting-Early and Late-Term Outcomes of Surgical Treatment.

Background: Coronary heart disease (CHD) remains the most prevalent pathology within the circulatory system. Among its chronic complications, ischemic mitral valve regurgitation (IMR) is observed in approximately 15% of patients with sustained myocardial ischemia. The presence of this complex valvular defect significantly increases both overall mortality and the incidence of adverse cardiovascular events. Notably, the presence of moderate to severe mitral regurgitation in patients undergoing surgical revascularization has been shown to double the risk of death. Despite the well-established eti

Association between hemoglobin level and clinical outcomes in ischemic stroke patients with high-grade carotid artery stenosis.

OBJECTIVE: Abnormal hemoglobin levels may influence stroke outcome, while high-grade carotid artery stenosis (CAS) is linked to distal hemodynamic compromise. The relationship between hemoglobin and ischemic stroke (IS) outcome in patients with high-grade CAS remains unclear. We aimed to investigate this association in acute IS patients with high-grade CAS. METHODS: To compare the characteristics and outcome in acute IS patients with high-grade CAS across different hemoglobin levels, we conducted an observational cohort study from January 2007 to April 2012 and followed for 5 years. RESULTS:
