In primary and community health care settings, long-term (> 28 days) urinary catheterisation (LTC) is most commonly used in managing older people and those with neurological conditions. Studies suggest that in the UK LTC is used in 0.5 per cent of people aged 75 or over (Kohler-Ockmore and Feneley, 1996) and in four per cent of people receiving domiciliary care (Getliffe and Mulhall, 1991). Most health-care associated infections (HAIs) are caused by indwelling urinary catheters (Stamm, 1998). Many of these infections are serious and lead to significant morbidity. In acute care facilities, 20-3…

Catheter-associated urinary tract infections (CAUTI) are the commonest nosocomial infections worldwide. While they are often asymptomatic and frequently cost less than nosocomial surgical site infections or nosocomial pneumonia, they are major reservoirs of antimicrobial resistant pathogens. Numerous strategies have been devised in an attempt to reduce the incidence of CAUTI but few have proven effective. Novel technologies such as the potential use of antiseptic or antimicrobial coatings on catheters hold promise for possibly reducing these infections in the fight against antimicrobial resist…

Malnutrition including vitamin A and iron deficiency and parasitic diseases have a strikingly similar geographical distribution with the same people experiencing both insults together for much of their lives. Parasitic infections are thought to contribute to child malnutrition and micronutrient deficiency through subtle reduction in digestion and absorption, chronic inflammation and loss of nutrients. Parasites may affect the intake of food; it's subsequent digestion and absorption, metabolism and the maintenance of nutrient pools. The most important parasites related to nutritional status are…

New therapeutic approaches enable organ transplantations and guarantee longer survival for AIDS patients or patients with haematological neoplasia. The price for these medical advances is immunosuppression and thus enhanced susceptibility to opportunistic fungal infections. As a consequence invasive fungal infections are on the march in modern medicine. Therapeutic limitations and difficulties strongly demand for a deeper understanding of the interaction between the various fungi and the hosts' innate and adaptive immune defence system. This understanding is the essential prerequisite for a po…

Respiratory syncytial virus (RSV) is the most important viral respiratory pathogen of early life. Studies of the immune response in general (and the innate response in particular) to this agent are of interest for a number of reasons. First, severe forms of illness may be a result of enhanced immunologic responsiveness to viral constituents at the time of infection. Secondly, the immune response to RSV may consist principally of innate immune responses at the time of maximum severity of illness. Third, RSV infection in infancy may be linked via immune mechanisms to the development of childhood

BACKGROUND: Human metapneumovirus (hPMV) is a recently identified virus that is recognized as a cause of respiratory tract illness in the pediatric population.
OBJECTIVES: To determine the incidence of respiratory tract infections caused by hPMV in hospitalized infants and to describe the clinical characteristics and possible presence of coinfection with other viral agents.
PATIENTS AND METHODS: We performed a prospective study from September to June 2003 in all children aged less than 24 months who were admitted to the Severo Ochoa Hospital (Leganés, Madrid) with a respiratory tract infecti…

Foot complications are common among diabetic patients; foot ulcers are among the more serious consequences. These ulcers frequently become infected, with potentially disastrous progression to deeper spaces and tissues. If not treated promptly and appropriately, diabetic foot infections can become incurable or even lead to septic gangrene, which may require foot amputation. Diagnosing infection in a diabetic foot ulcer is based on clinical signs and symptoms of inflammation. Properly culturing an infected lesion can disclose the pathogens and provide their antibiotic susceptibilities. Specimens…

A retrospective clinical study was carried out on a group of 18 patients with severe intraperitoneal infections due to various causes. The decision to use laparotomy was supported by the severity of sepsis (APACHE II score varied from 14 to 30), highly septic peritoneal fluid, the features of peritonitis, patients with impaired immunity. Open packing was realized with a Dacron mesh sutured to the fascial margins, associated with multiple peritoneal drainage in all cases. Postoperative mortality was 50%, mainly to rapid evolution of septic shock, unresponsive to intensive care support. Secondar…

Infection of the sacroiliac joint can be pyogenic or granulomatous and is usually unilateral. There are a number of predisposing conditions including drug abuse and intra articular steroid injection, but in 44% of cases, no definite predisposing factors can be identified. Considerable delay between presentation and diagnosis is recognized. The clinical picture may be non-specific and variable, and clinical suspicion may be low due to the relatively low incidence of the condition. This is compounded by difficulties in clinical examination of the SUs. The diagnosis is based on a history suggesti

BACKGROUND: Common variable immunodeficiency (CVID) is a primary immunodeficiency disease, the hallmark of which is hypogammaglobulinemia and poor specific antibody responses. Patients usually have recurrent bacterial infections, but there are a number of other comorbid disorders, including autoimmune disease and neoplasms. Most patients are diagnosed as adults, and delay in identifying the antibody defect is common. In the present report, we illustrate atypical initial presentation without infections in CVID.
CLINICAL OBSERVATION: In 5 out of 30 patients with CVID (16.6 %) diagnosed in our h…

BACKGROUND: There has been a worldwide increased prevalence of tuberculosis (TB) in recent years, with a similar trend observed in Jamaica and more recently in children admitted to the University Hospital of the West Indies, Jamaica. Data regarding paediatric TB, especially as it relates to all aspects of HIV co-infection, are needed from developing countries in diverse geographic settings to enhance prevention and treatment policies (National Institutes of Health, Office of AIDS Research, FY 2005 Budgetary Planning Meeting, March 11, 2003, Washington, DC).

Sexually transmitted infections (STIs) are more dynamic than other diseases prevailing in the community. Their epidemiological profile varies from country to country and from one region to another within a country, depending upon ethnographic, demographic, socioeconomic and health factors. The clinical pattern is also a result of the interaction among pathogens, the behaviours that transmit them and the effectiveness of preventive and control interventions. We reviewed the changing patterns of different STIs (excluding HIV infection) in India and their various risk factors. A MEDLINE search wa…

PURPOSE: To report risk factors, clinical course, and outcome in patients with infectious keratitis following implantation of intracorneal ring segments (ICRS).
METHODS: The records of 8 patients with culture-proven infectious keratitis after ICRS (Ferrara or Intacs) implantation were retrospectively reviewed. Age, gender, corneal findings, ocular abnormalities, the condition that led to ICRS implantation, immediate prior use of a contact lens, elapsed time between implantation and the onset of symptoms, previous medications, and systemic disorders were noted.
RESULTS: Culture-positive infec

The incidence of methicillin-resistant Staphylococcus aureus (MRSA) infections in patients admitted to the intensive care unit has dramatically increased in recent years, with an associated increase in morbidity and mortality and the costs of caring for patients with MRSA infections. Although indiscriminate and inappropriate use of antibiotics has contributed to this phenomenon, horizontal transmission of MRSA between patients and health care providers is the principal cause of this observed increase. This article discusses the pathogenesis, epidemiology, treatment, and prevention of MRSA infe…
