Multiple cutaneous necrotic lesions associated with Interferon beta-1b injection for multiple sclerosis treatment: A case report and literature review.

Multiple sclerosis (MS) is a chronic and debilitating inflammatory autoimmune disorder of the central nervous system. MS patients may experience severe local inflammatory skin reactions during disease-modifying therapy with subcutaneously injected interferon-beta-1b (IFN-β). We report the case of a 49-year-old woman with relapsing-remitting MS, who developed multiple cutaneous necrotic ulcers on both arms and thighs after 3 months of treatment with subcutaneous IFN-β-1b. The biopsy specimens showed skin and subcutaneous tissue necrosis. We diagnosed the skin lesions as cutaneous necrotic ulcer…

The clinical relevance of multiple sclerosis genetics: from susceptibility to disease severity and progression.

Large-scale genome-wide association studies have identified hundreds of multiple sclerosis risk loci, most of which implicate immune pathways and establish multiple sclerosis as a highly polygenic, immune-mediated disease. These findings help explain why immune-modifying therapies suppress relapses and new lesion formation. However, susceptibility genetics does not adequately explain disability accumulation, variation in disease course, or treatment response. Evidence from rare-variant studies, genetic studies of disease severity and disability progression, epigenetic analyses, gene-environmen…

OCT to Assess Risk of Progression of Multiple Sclerosis: A Report by the American Academy of Ophthalmology.

PURPOSE: To review the current published literature on the utility of OCT of the retina in peripapillary and macular regions to predict or assess risk of disease activity in adult patients with multiple sclerosis (MS) and MS precursors (radiologically isolated syndrome and clinically isolated syndrome). METHODS: A literature search of the PubMed database was last performed in September 2025. Articles were limited to original research in the English language that included adult patients. The search yielded 275 articles that were examined in abstract form to exclude reviews, and 71 were reviewe…

Cognitive Function and Disability in People with Multiple Sclerosis: A Systematic Review and Meta-Analysis.

BACKGROUND: Cognitive dysfunction is a common yet under-recognized component of disability in people with multiple sclerosis (PwMS). Although numerous studies have investigated the relationship between cognitive function and physical disability, the findings remain inconsistent. In this systematic review and meta-analysis, we aim to clarify and quantify the association between physical disability and cognitive performance in PwMS, while also examining potential modifiers and the clinical relevance of this relationship. METHODS: A systematic literature search in multiple databases identified 1…

Development and validation of a social vulnerability index for use in Canadians living with multiple sclerosis.

BACKGROUND: Social factors influence the health outcomes of individuals with multiple sclerosis (MS). Social vulnerability indices (SVIs) can identify populations at risk of poor outcomes and inform targeted interventions, but no SVI has been developed for people with MS to date. OBJECTIVE: We developed a SVI, which aggregates several measures of social vulnerability into a composite, and assessed its performance. METHODS: We conducted a national, cross-sectional survey of people with MS in Canada from June 2025-March 2026. We selected social vulnerability factors across demographic, socioec…

Vitamin D receptor gene variants and their association with clinical phenotypes of multiple sclerosis.

Multiple sclerosis (MS) is a complex neuroinflammatory disorder shaped by environmental and genetic factors. Vitamin D, a well-known environmental factor associated with the disease, exerts its immunoregulatory effects mainly through vitamin D receptor (VDR). Single nucleotide variants (SNVs) within the VDR gene may modify receptor function. Although previous studies have examined VDR SNVs in patients with MS, results have been inconsistent. In addition, their involvement in disease phenotypic variability remains poorly understood. The study aimed to examine potential correlation between FokI …

Air pollution and the risk of multiple sclerosis relapses in the Paris region, the CONFISEP study.

BACKGROUND: Air pollution has been proposed as a short-term trigger of multiple sclerosis (MS) relapses, but delayed effects remain poorly studied in the era of modern disease-modifying therapies (DMTs). We investigated whether weekly air pollution exposure was associated with delayed MS relapse risk up to 15 weeks. METHODS: We conducted a bicentric retrospective cohort study in the Île-de-France region (2018-2021) including adults with relapsing MS from the Observatoire Français de la Sclérose en Plaques registry. Weekly exposure to particulate matter (PM)10, PM2.5, nitrogen dioxide (NO2) an…

Severe autoimmune hepatitis and necrotising pancreatitis following ofatumumab in a patient with multiple sclerosis.

We report the case of a male patient in his 30s with newly diagnosed multiple sclerosis who initially responded well to high-dose corticosteroids and was started on ofatumumab as disease-modifying therapy. Baseline laboratory investigations, viral screening and autoantibody testing were normal. Within days of the first injection, he developed jaundice, abdominal pain and gastrointestinal symptoms. Laboratory studies revealed severe hepatocellular injury with hyperbilirubinaemia, coagulopathy, elevated pancreatic enzymes and positive anti-smooth muscle antibodies, consistent with autoimmune hep…

Development of a self-management journey map for people with multiple sclerosis: a qualitative study.

BACKGROUND: Multiple sclerosis (MS) is an incurable disease and requires active patient participation. Its complex progression needs dynamic self-management. Yet the stage-specific experiences and needs of people with MS (PwMS) remain unexplored and a clear, patient-centered care pathway across different disease stages is lacking. AIMS: To map the patient journey from PwMS perspectives, visualize their self-management experiences and needs, identify key intervention targets, and enhance PwMS’s life quality through medical care cooperation. METHODS: This study employed a qualitative descriptive…

In silico screening and preclinical validation identify bavisant as a therapeutic candidate for multiple sclerosis.

Current treatments for multiple sclerosis (MS) are insufficient to delay the neurodegenerative process that is the main cause of disability progression in patients with MS. Therapeutics aimed at supporting myelin regeneration and neuroprotection are thus a major unmet medical need for the progressive forms of MS. To address this, we developed a strategy combining in silico screening of more than 1500 repurposed compounds with a validation pipeline of models, encompassing rodent and human in vitro assays as well as mouse models of demyelination/remyelination. From the initial library, 273 drugs…

The impact of Information processing speed on visuospatial memory encoding and retrieval in multiple sclerosis.

INTRODUCTION: Information processing speed (IPS) is frequently impaired in people with multiple sclerosis (PwMS) and can potentially interfere with functioning in other cognitive domains. A relationship between IPS and visuospatial memory has been shown, but the impact of IPS on the separate components of visuospatial memory remains unclear. In this study, we aim to investigate the association between IPS impairment and encoding, active and passive retrieval, and learning indices of visuospatial memory in PwMS. METHODS: Cross-sectional data from 92 PwMS with cognitive complaints and 29 matche…

Human calprotectin as a novel biomarker in multiple sclerosis: Can it differentiate disease stages?

This study aims to investigate the relationship between serum calprotectin levels and different stages of multiple sclerosis (MS). This study was designed as a prospective, cross-sectional case-control study. A total of 98 patients diagnosed with relapsing-remitting MS and secondary progressive MS according to the 2017 McDonald criteria, along with 40 control patients whose human calprotectin levels were measured, were included in the study. The average age and human calprotectin levels in the patient group were found to be higher compared to the control group (P = .013 and P < .001, respectiv…

A longitudinal study of depression, anxiety, and disability in multiple sclerosis: Profile and trajectories analyses.

BACKGROUND: Depression and anxiety are prevalent comorbidities in individuals diagnosed with Multiple Sclerosis (MS). Despite this, there is a lack of longitudinal research aimed at exploring the profiles and trajectories of these psychological conditions in this population. OBJECTIVES: This study had three primary objectives: (1) to identify distinct profiles of depression, anxiety, and disability across three time points; (2) to examine the differences among these profiles with respect to demographic and clinical characteristics; and (3) to examine longitudinal associations and patterns of …

Neuroprotective Role of Cannabinoid CB1 and GPR55 Receptors in a Cell Model of Multiple Sclerosis.

Multiple sclerosis (MS) is a chronic neuroinflammatory disorder characterized by demyelination, oligodendrocyte (OLG) loss, and progressive neurological decline. While current disease-modifying therapies primarily target immune responses, they offer limited neuroprotective or remyelinating effects. Through its receptors, including cannabinoid receptor type 1 (CB1R) and the G protein-coupled receptor GPR55, the endocannabinoid system (ECS) has emerged as a promising therapeutic target due to its roles in neuronal survival and glial function. To better study the functional role of these receptor…
