Modified Gritti-Stokes amputation for periprosthetic fracture with irremovable femoral nail: case report and review of an overlooked procedure.

The management of retained orthopedic hardware during amputation procedures presents significant challenges. A case utilizing a modified Gritti-Stokes through-knee amputation to address a periprosthetic fracture with an exposed intramedullary nail (IMN) in a patient with multiple comorbidities is reported. A 47-year-old male with a history of human immunodeficiency virus, hepatitis C virus, and type 2 diabetes mellitus presented with a left femoral periprosthetic supracondylar fracture. The patient had an existing IMN from a previous femoral shaft fracture 20 years prior, along with a left bel

Distal locking screw migration in the depuy-synthes retrograde femoral nail-advanced-an unexpected consequence of an implant upgrade.

AIMS: The Retrograde Femoral Nailing Advanced (RFNA) system is a new generation of retrograde femoral nail (RFN) introduced to extend the range of distal fractures treated by intramedullary (IM) nailing. There is concern that the rate of distal interlock screw back-out may be higher for the RFNA. This study aims to highlight a higher-than-expected incidence of distal locking screw migration. METHOD: Patient and implant data were prospectively collected from theatre and electronic patient records. The study focused on monitoring the migration of distal locking screws, treating each screw as an

Retrograde Intramedullary Nailing vs. Lateral Locking Plate Fixation for Distal Femur Fractures: Which Technique is Superior?

OBJECTIVE: Distal femur fractures are challenging to treat, especially in elderly osteoporotic patients. Retrograde intramedullary nailing (RIMN) and lateral locking plate (LP) fixation are commonly used surgical methods for these fractures. This study aimed to compare the clinical and radiological outcomes of these two techniques. METHODS: In this retrospective study, a total of 72 patients with distal femur fractures classified according to the AO/OTA system and treated surgically were included. Patients were divided into two groups: RIMN (n=36) and LP (n=36). Demographic data, fracture cla

Postoperative limping following proximal femoral nailing due to superior gluteal nerve injury: A retrospective observational study.

BACKGROUND: Postoperative limping is a common yet under-investigated complication following proximal femoral nailing (PFN). One overlooked etiology is iatrogenic superior gluteal nerve (SGN) injury. This retrospective study determines the incidence of SGN injury in patients with postoperative limping following PFN and evaluate associated risk factors and functional outcomes. METHODS: A retrospective cohort study was conducted over five years (2017-2022) involving 213 patients who underwent PFN for intertrochanteric and subtrochanteric femoral fractures. Patients presenting with postoperative

Combined pudendal and sciatic nerve palsy following intramedullary nailing in fracture shaft femur: a rare case report.

Traction tables have been widely used by orthopaedic surgeons for the management of femoral shaft fractures. Although complications associated with their use are uncommon, they can still occur. Among neurological complications, sciatic and pudendal nerve palsies are the most frequently reported. Recognizing the potential causes and analyzing these complications are crucial for raising awareness among surgeons and implementing preventive strategies to enhance patient safety during surgical procedures. A 22-year-old male sustained a comminuted fracture of the left femoral shaft following a road

Treatment of subtrochantric femur fracture with intra-medullary nailing (IMN) in a patient with osteopetrosis: A case report and literature review.

INTRODUCTION AND IMPORTANCE: Osteopetrosis is a rare hereditary bone disorder characterized by defective osteoclastic bone resorption, leading to dense but brittle bones. These structural abnormalities predispose patients to fractures and complicate orthopedic surgeries due to obliterated medullary canals and increased cortical density. Subtrochanteric femoral fractures in such patients are rare and challenging, with limited consensus on optimal surgical management. CASE PRESENTATION: We present a 45-year-old male with adult-onset autosomal dominant osteopetrosis who sustained bilateral subtr

Outcomes in Treatment of Ankle and Pilon Fractures with Retrograde Tibiotalocalcaneal Nailing Without Articular Preparation in the Setting of Diabetes Mellitus.

BACKGROUND: Treatment of ankle and pilon fractures in the setting of diabetes mellitus (DM) is challenging due to a propensity for postoperative complications. Limb salvage is a primary concern following these injuries, as below knee amputation (BKA) occurs at an unacceptably high rate. Primary retrograde tibiotalocalcaneal (TTC) joint nailing without articular preparation has emerged as a solution to treat diabetics with ankle and pilon fractures to mitigate surgical complications and prevent BKA. The technique minimizes surgical dissection and has previously demonstrated utility in fragility

Comparison of the efficacy of Philos plate and Multiloc intramedullary nail in the treatment of complex proximal humeral fractures with osteoporosis in the elderly.

INTRODUCTION: Proximal humeral fractures are common in elderly patients with osteoporosis. Complex three- or four-part fractures often require surgical intervention. Philos locking plates and Multiloc intramedullary nails are widely used, but their comparative effectiveness in osteoporotic elderly patients remains uncertain. METHODS: A retrospective study was performed on 90 elderly patients (aged 70-95 years) with Neer three- or four-part proximal humeral fractures treated between January 2021 and December 2023. Patients received either Philos plate fixation (n = 50) or Multiloc intramedulla…

Risk of Distal Femur Fracture in Antegrade Intramedullary Nail Fixation of Proximal Femur Fractures: A Finite Element Study.

Surgeons often use long antegrade intramedullary nails (aIMNs) when fixing fractures of the proximal femur in geriatric patients. However, risk of peri-implant fracture in relation to aIMN termination level has not yet been studied. We therefore studied the correlation between aIMN nail tip termination level and extent of distal femur fracture in three different loading scenarios using finite element models of seven different length aIMNs. These were loaded into 3D models of synthetic osteoporotic femurs, which were loaded in single-leg stance (SLS) with body weight load at the femoral head, t…

Does Stem Design for Conversion Total Hip Arthroplasty From Previous Short Intramedullary Nail Predispose to Fracture?

INTRODUCTION: Conversion to total hip arthroplasty (THA) from prior short intramedullary nail (IMN) is an increasingly common procedure. Currently, there are no studies investigating if distal interlock screws serve as stress risers contributing to perioperative fractures, or whether primary stems can be used in previously reamed and fractured femurs. We hypothesized that conversion would be successful regardless of stem design. METHODS: Nineteen patients who underwent short IMN to THA conversion were retrospectively identified from 2013 to 2022, with a mean 13 months between the procedures.

Cephalomedullary Nailing vs. Dynamic Hip Screw for the Treatment of Pertrochanteric Fractures: The Role of Cytokines in the Prediction of Surgical Invasiveness.

Background/Objectives: Limited surgical invasiveness is desirable in elderly patients with femur fracture. Serum cytokines have been considered as a possible marker but with inconclusive evidence. The present study aimed to assess the systemic inflammatory response to surgical trauma through the serum levels of several cytokines (IL1β, IL6, IL8, and IL-10), inflammatory markers (c reactive protein-CRP), and muscular damage markers (creatinkinase-CK) at different time intervals in a consecutive series of patients affected by pertrochanteric fractures (PFs) and treated by two different surgical

Reconstruction after wide excision of the nail apparatus for in situ or minimally invasive subungual melanoma: A retrospective case series.

INTRODUCTION: Historically, subungual melanoma (SUM) was treated by amputation of the affected digit. Wide local excision (WLE) of the nail apparatus has since become a conservative alternative for in situ or minimally invasive lesions. While several reconstructive techniques have been described after WLE, few centers have reported their outcomes objectively. This study presents our series of reconstructions following WLE of the nail apparatus. METHODS: We conducted a retrospective study at a university hospital, including patients referred by dermatologists for WLE and nail apparatus reconst

A novel nail-plate construct for the treatment of AO/OTA 31-A3.3 intertrochanteric fractures: a finite element analysis.

BACKGROUND: The AO/OTA 31-A3.3 is the most unstable type with a lesser trochanteric fragment and a broken lateral femoral wall (LFW), which constitute a four-part unstable intertrochanteric fracture. Implant failure remains one of the catastrophic consequences after surgical treatment. A novel nail-plate construct, called proximal femoral universal nail system (PFUN), is proposed by our team to fix comminuted LFW fracture fragment and lesser trochanteric fragment. The aim of this study is to evaluate the biomechanical properties of PFUN compared with proximal femoral nails anti-rotation (PFNA)…

Intramedullary Nailing vs. Plate Fixation for Trochanteric Femoral Fractures: A Systematic Review and Meta-Analysis of Randomized Trials.

Background/Objectives: Trochanteric femoral fractures pose significant surgical challenges, particularly in elderly patients. Intramedullary nailing (IMN) and plate fixation (PF) are the primary operative strategies, yet their comparative efficacy and safety remain debated. This meta-analysis synthesizes randomized controlled trials (RCTs) to evaluate clinical, functional, perioperative, and biomechanical outcomes of IMN versus PF specifically in trochanteric fractures. Methods: A systematic search of six databases was conducted up to 20 May 2024, to identify RCTs comparing IMN and PF in adult

Tips and Tricks for Installation of the SLIM Nail in Osteogenesis Imperfecta with Narrow Medullary Canals: A Surgical Guide with Case Insights.

INTRODUCTION: Osteogenesis imperfecta (OI) presents significant surgical challenges due to bone fragility, narrow medullary canals, and complex deformities. While telescoping rods like the Fassier-Duval (FD) system are commonly used in growing patients, they are unsuitable when the canal diameter is too small or when patients approach skeletal maturity. The Simple Locking Intramedullary (SLIM) nail offers a solid, non-telescoping alternative in these cases. METHODS: We describe the surgical technique for SLIM nail implantation and highlight key technical pearls developed through institutional

Static versus dynamic intramedullary nailing for femoral and tibial shaft fractures: a double-blind, randomized clinical trial.

BACKGROUND: The optimal fixation method for femur and tibia shaft fractures remains debated. Both static intramedullary nailing (SIMN) and dynamic intramedullary nailing (DIMN) are commonly used. This study aims to compare the efficacy of SIMN versus DIMN in promoting fracture healing. METHODS: This double-blind, randomized clinical trial was conducted from June 2022 to March 2024. Fifty-two people with single transverse or short oblique femur or tibia fractures were randomly put into two groups, SIMN and DIMN. Each group had 26 people. The primary outcome was time to union, and secondary out
