Anesthesia for Intracavitary Brachytherapy: a 19-month Experience at the Philippine General Hospital during the COVID-19 Pandemic.

BACKGROUND AND OBJECTIVE: Brachytherapy is the only demonstrated technique of delivering the high radiation dose required to control cervical cancer (>80 Gray [Gy]) without causing unwanted side effects. There is still limited data available in the Philippines regarding the anesthetic management of patients receiving intracavitary brachytherapy for cervical cancer. It is the aim of this study to present the anesthetic management of these procedures performed in a non-operating site remote from the main hospital during the first 1 ½ years of the COVID-19 pandemic. METHODS: A retrospective revi

Previous Lumbar Spine Surgery Decreases the Therapeutic Efficacy of Dorsal Root Ganglion Pulsed Radiofrequency in Patients with Chronic Lumbosacral Radicular Pain.

Chronic lumbosacral radicular pain (CLRP) as a possible adverse consequence of lumbar spine surgery represents a serious medical challenge. Pulsed radiofrequency of dorsal root ganglion (PRF-DRG) treatment is known to be effective in alleviating CLRP. This retrospective study compares the efficacy of a single CT-guided PRF-DRG procedure in the treatment of unilateral CLRP between patients without (non-PSS) and with (PSS) previous lumbar spine surgery. Non-PSS and PSS groups included 30 and 20 patients, respectively. Outcomes (pain intensity and disability) were evaluated by means of the visual

Conversion of First Metatarsal-Phalangeal Joint Arthrodesis to Arthroplasty: A Case Report.

INTRODUCTION: Arthrodesis remains the gold standard for most first metatarsal-phalangeal joint (1MTPJ) pathologic conditions due to its high patient satisfaction, low complication rates, and consistent data. 1MTPJ arthroplasty remains a pursued procedure given the advantages described above, but the literature remains complicated and controversial as a primary surgical treatment. To the authors' knowledge, there is no prior report describing utilization of arthroplasty as an approach to managing clinical failure of a successful fusion in the setting of a technically successful procedure withou

Mesenteric Tear Can Be Caused by Abdominal Counter-Pressure Applied During Colonoscopy.

BACKGROUND We report the case of a patient undergoing a colonoscopy during which pressure applied on the abdomen by a nurse assisting in the procedure caused an extraluminal mesenteric tear. Mesenteric tears can be life-threating and need to be managed appropriately. This is the first case reported in the literature describing abdominal counter-pressure applied in the endoscopic procedure room as the cause of traumatic bowel injury. CASE REPORT A 72-year-old woman presented for a screening colonoscopy. Advancement of the colonoscope became so difficult that the procedure was temporarily aborte

Comparative review of early and intermediate patency rates of polytetrafluoroethylene and autogenous saphenous vein grafts for lower extremity ischemia.

One hundred nine primary bypass operations were performed on 97 patients who had limb ischemia caused by arteriosclerosis obliterans, over a 32-month period starting in January, 1978. The majority of those procedures were done for rest pain or gangrenous changes. The greater saphenous vein was used whenever possible (44 of 109 procedures), and a polytetrafluoroethylene (Gore-Tex) prosthesis was used in 65 of 109 procedures. Average follow-up was 9.3 months. The cumulative patency rate in all vein grafts was 93.1% at 1 month and 73.4% after 31 months, whereas the cumulative patency rate for pol

Investigation of the effectiveness of cooling spray on mammography comfort.

Breast cancer is one of the most common cancer types in recent years. Early diagnosis of breast cancer increases the success of treatment. Breast cancer screening is thus highly critical. Mammography is a screening tool that is still globally valid. I would propose that Mammography can potentially be painful. In our study, we aimed to determine the level of pain experienced by using coolant spray before mammography imaging. Our study is a randomized double-blind prospective study. It was conducted on patients who agreed to participate in the study. In our study, coolant spray was applied to th

Treatment of post-meniscectomy knee symptoms with medial meniscus replacement results in greater pain reduction and functional improvement than non-surgical care.

PURPOSE: Partial meniscectomy is a common orthopedic procedure intended to improve knee pain and function in patients with irreparable meniscal tears. However, 6-25% of partial meniscectomy patients experience persistent knee pain after surgery. In this randomized controlled trial (RCT) involving subjects with knee pain following partial meniscectomy, it was hypothesized that treatment with a synthetic medial meniscus replacement (MMR) implant provides significantly greater improvements in knee pain and function compared to non-surgical care alone. METHODS: In this prospective, multicenter RC

Revision Endoscopic Carpal Tunnel Release: An Update on Technique and Outcomes.

BACKGROUND: Carpal tunnel release is among the most frequently conducted upper extremity procedures. While it typically provides patients with improved sensation, decreased pain, and improved function, a small percentage of cases require revision due to failed initial surgery. Consequently, adoption of endoscopic techniques for carpal tunnel revision procedures is beginning to be explored. METHODS: A retrospective analysis was conducted of postoperative clinical visits and patient-reported outcome measures for four patients who had undergone five endoscopic revision carpal tunnel release proc

Complications of botulinum toxin injections for treatment of esophageal motility disorders†.

In achalasia and spastic esophageal motility disorders, botulinum toxin (botox) injection is considered an effective and low-risk procedure for short-term symptom relief. It is mainly offered to medically high-risk patients. However, no analysis of risks of botox injections has been performed. To determine the incidence and risk factors of procedure-related complications after esophageal botox injections, we analyzed the records of all patients undergoing botox injection therapy for esophageal motility disorders at four university hospitals in Europe and North America between 2008 and 2014.
