Pedicled vascularized bone grafts for disorders of the carpus: scaphoid nonunion and Kienbock's disease.

The use of reverse-flow pedicled vascularized bone grafts from the dorsal distal radius makes it possible to transfer bone with a preserved circulation and viable osteoclasts and osteoblasts. The resultant primary bone healing without creeping substitution within the dead bone is an alternative to conventional bone grafting in aiding or accelerating healing, replacing deficient bone, and/or revascularizing ischemic bone. Recent advances in understanding the anatomy and physiology of vascularized pedicled bone grafts have increased their use in treating a variety of carpal maladies. A basic und

Fractures of the proximal radial head and neck in children with emphasis on those that involve the articular cartilage.

This is a review of 116 children who had a fracture of the proximal radial head or neck over a 15-year period. Of 33 teenagers with closed physes, 17 (52%) had intraarticular involvement. Of 83 younger children with an open proximal radial physis, six (7%) had an intraarticular fracture (Salter-Harris type III or IV). Of the 17 patients with closed physes and intraarticular fracture, 13 had adequate follow-up. There were eight excellent, three good, one fair, and one poor results. Of the six children with open physes and intraarticular fracture, there were one good and five poor results. This

Effects of intra-articular distal radius depression on wrist joint contact characteristics.

Twelve cadaver extremities were used to study the effect of scaphoid and lunate facet depressions on the contact characteristics of the radiocarpal joint. Pressure-sensitive film was inserted into radiocarpal joints with varying degrees of depression, and the specimens were loaded statically in neutral position, radial deviation, and ulnar deviation. The film was removed and analyzed for contact area and pressure. The only statistically significant effect of a lunate fossa depression was an increase in scaphoid fossa pressure with a 3-mm step-off and the hand in neutral position. Scaphoid foss

Solitary bone cyst of the radius containing Liesegang's rings.

The authors describe the case of a 33-year-old man with a solitary cystic lesion of the left distal radius with central radiodensities that radiographically suggested a chondroid matrix. Curettage of the lesion revealed numerous calcospherites with a lamellar structure. This lamellar pattern is thought to be a result of alternating precipitation and diffusion of supersaturated solutions. This biochemical process has been called the Liesegang's phenomenon, and the structures have been termed Liesegang's rings. The presence of these structures in this benign lesion contributed to atypical roentg

Volar intercalary carpal instability following a seemingly innocent wrist fracture.

A case of volar of intercalary carpal instability following a radial fracture is described and the mechanics of injury examined. Such an injury is rare and the type of radial fracture which produced it unusual. Continued instability of the carpus produces late degenerative changes. In contrast to the many reports of early and persistent pain and functional limitation this patient's wrist settled rapidly.

Soft-tissue changes accompanying recent scaphoid injuries.

In 200 consecutive cases of suspected scaphoid fracture, 27 such fractures were demonstrated. Radiologically, when a fracture was present, the soft tissues on the dorsum of the wrist were definitely abnormal in 24 cases and equivocal in only three cases, whereas the scaphoid fat pad was normal in two cases and equivocal in 13 cases. In addition, in 88 cases when the soft tissues on the dorsum of the wrist were definitely normal, no fracture was ever demonstrated.

Fracture and dislocation of snowboarder's elbow.

BACKGROUND: This study focuses on the analysis of snowboarding versus skiing injuries, especially fracture, dislocation, or both, of the elbow, based on 7 years of medical records and roentgenograms of patients injured at a ski-snowboard area, Mt. Zao National Park, and demonstrates the precise characteristics of snowboard injury in the elbow region. METHODS: A retrospective study of 1,445 injured snowboarders and 10,152 injured skiers was undertaken to assess both snowboarding and skiing injuries. Sixty-four cases of snowboarding injuries and 152 cases of skiing injuries were available for p

Acute Forearm and Hand Compartment Syndrome in a Child Following Delayed Presentation of Forearm Trauma: A Case Report and Literature Review.

Background and Clinical Significance: Acute compartment syndrome is a rare but limb-threatening emergency in pediatric patients. While most cases follow high-energy trauma or displaced fractures, acute compartment syndrome precipitated by initially underestimated forearm injuries is uncommon and may create a significant diagnostic challenge, particularly in young children who exhibit atypical clinical presentations, such as escalating anxiety and analgesic requirements, rather than classic ischemic signs. Case Presentation: We report the case of a 4-year-old girl who developed severe forearm a

Type C distal radial fractures treated with conventional AO plates: an easy and cost-saving solution in a locking plate era.

PURPOSE: C-type distal radial fractures remain challenging fractures. Currently locking plates are very popular because of their length preserving, stability. A considerable drawback is the high cost. Since 2003 we have been using mini AO plates (2.7 mm) as an alternative. We analysed our results and performed a cost analysis. METHODS: Retrospective analysis was performed of all patients operated upon between 2003 and 2008 for C type distal radius fractures. Reduction was achieved with mini AO plates, applied in a buttress fashion, with ligamentotaxis. Rehabilitation consisted of immediate mo…

Palmar plating system for Colles' fractures--a preliminary report.

PURPOSE: To present our results of the palmar plating system that we developed for the treatment of Colles' fractures. METHODS: By using the palmar plating system that we developed a consecutive series of 40 acute Colles' fractures were treated surgically. There were 12 men and 28 women with a mean age of 57 years at the time of the injury (range, 25-90 y). All patients had internal fixation using the trans-flexor carpi radialis tendon approach. The system has 3 main features. First, the plate is small in size, being 1.1 mm in thickness and 47 mm in length. Only a 3- to 4-cm skin incision is

Assessing cortical bone changes with Romosozumab after one year: Insights from an exploratory HR-pQCT study.

PURPOSE: To describe changes in bone microarchitecture after 6 and 12 months of romosozumab (ROMO), identify responders based on cortical thickness (Ct.Th) increase at the distal radius (≥3% at 12 months), and explore factors associated with treatment response. METHODS: We conducted a prospective cohort study of postmenopausal women with osteoporosis at very high fracture risk who started ROMO between August 2021 and February 2023. HR-pQCT, BMD, and bone turnover markers were assessed at baseline, 6, and 12 months. Patients were classified as responders or non-responders based on 12-month Ct.

Vitamin K supplementation and bone mineral density in dialysis: results of the double-blind, randomized, placebo-controlled RenaKvit trial.

BACKGROUND: Vitamin K deficiency is highly prevalent in patients on dialysis and may contribute to their low bone mineral density (BMD) and increased risk of fracture. This study investigated the effect of menaquinone-7 (MK-7) supplementation on BMD in patients on chronic dialysis. METHODS: In a multicentre, double-blind, placebo-controlled intervention trial, 123 patients on chronic dialysis were randomised to a daily oral supplement of either MK-7 360 µg or placebo for 2 years. BMD of the distal radius (1/3, mid, ultradistal and total), femoral neck, lumbar spine (L1-L4) and whole body was

Serum FGF-21 levels are associated with worsened radial trabecular bone microarchitecture and decreased radial bone strength in women with anorexia nervosa.

BACKGROUND: Anorexia nervosa (AN) is a psychiatric disorder characterized by self-induced starvation and low body weight. Women with AN have impaired bone formation, low bone mass and an increased risk of fracture. FGF-21 is a hormone secreted by the liver in starvation and FGF-21 transgenic mice have significant bone loss due to an uncoupling of bone resorption and bone formation. We hypothesized that FGF-21 may contribute to the low bone mass state of AN. SUBJECTS AND METHODS: We studied 46 women: 20 with AN (median age [interquartile range]: 27.5 [25, 30.75] years) and 26 normal-weight con

Computer algorithms for three-dimensional measurement of humeral anatomy: analysis of 140 paired humeri.

BACKGROUND: In the presence of severe osteoarthritis, osteonecrosis, or proximal humeral fracture, the contralateral humerus may serve as a template for the 3-dimensional (3D) preoperative planning of reconstructive surgery. The purpose of this study was to develop algorithms for performing 3D measurements of the humeral anatomy and further to assess side-to-side (bilateral) differences in humeral head retrotorsion, humeral head inclination, humeral length, and humeral head radius and height. METHODS: The 3D models of 140 paired humeri (70 cadavers) were extracted from computed tomographic da

Trabecular bone score (TBS)--a novel method to evaluate bone microarchitectural texture in patients with primary hyperparathyroidism.

CONTEXT: In the milder form of primary hyperparathyroidism (PHPT), cancellous bone, represented by areal bone mineral density at the lumbar spine by dual-energy x-ray absorptiometry (DXA), is preserved. This finding is in contrast to high-resolution peripheral quantitative computed tomography (HRpQCT) results of abnormal trabecular microstructure and epidemiological evidence for increased overall fracture risk in PHPT. Because DXA does not directly measure trabecular bone and HRpQCT is not widely available, we used trabecular bone score (TBS), a novel gray-level textural analysis applied to sp

Médecins sans frontières experience in orthopedic surgery in postearthquake Haiti in 2010.

INTRODUCTION: During January 2010, a 7.0 magnitude earthquake struck Haiti, resulting in death and destruction for hundreds of thousands of people. This study describes the types of orthopedic procedures performed, the options for patient follow-up, and limitations in obtaining outcomes data in an emergency setting. PROBLEM: There is not a large body of data that describes larger orthopedic cohorts, especially those focusing on internal fixation surgeries in resource-poor settings in postdisaster regions. This article describes 248 injuries and over 300 procedures carried out in the Médecins
