Dedifferentiated liposarcoma can arise de novo or as a complication of a preexisting well-differentiated liposarcoma. We describe the radiologic and pathologic features of a long-standing liposarcoma with multiple recurrences in a 59-year-old male. Imaging demonstrated a heterogeneous fat-containing mass in the anterior thigh. Myxoid liposarcoma usually develops in the retroperitoneum or deep soft tissues of extremities. Synchronous cardiac and abdominal metastases are extremely rare. We describe the case of a 63-year-old man, with a history of a thigh liposarcoma, treated by resection, chemotherapy and postoperative radiotherapy 13 years ago. Patient now presented with an intracardial, a pericardial and an abdominal mass, identified as metastatic myxoid liposarcomas. He eventually underwent successful emergency sternotomy and subsequent laparotomy for tumor resection.

CT offers specific advantages in the evaluation of complex spine trauma and pelvic injuries, particularly to evaluate fractures of the acetabulum. Spinal stenosis and lumbar disc herniation are conclusively demonstrated by CT and lumbar myelography thus becomes superfluous. Femoral and tibial torsion may be estimated with high accuracy by CT. CT is of definite value in the evaluation of bone neoplasms, and chronic inflammatory diseases of the spine and pelvic bones, especially for the determination of the tumor extent and soft-tissue pathology.

Soft tissue sarcomas (STS) are rare tumors of mesenchymal origin. About 50% of patients with STS experience relapse and more than 30% will die within 10 years after diagnosis. Malignant peritoneal mesothelioma is usually confined to the abdomen and pelvis. Extra-abdominal and extrapelvic dissemination is very uncommon mostly at advanced stages of the disease. This rare clinical event may be related to contiguous spread through connecting pathways linking the abdomen and pelvis to the thorax and proximal lower extremity.
