Correction of Excyclotropia by Surgery on the Inferior Rectus Muscle in Patients with Thyroid Eye Disease: A Retrospective, Observational Study.

PURPOSE: To examine the characteristics of excyclotropia correction through surgery on the inferior rectus muscle in patients with thyroid eye disease. METHODS: This was a retrospective, observational study at a single institution. We reviewed 36 patients who had undergone unilateral inferior rectus muscle recession, with or without nasal inferior rectus muscle transposition. The following factors were investigated as possibly influencing excyclotropia correction: inferior rectus muscle thickness, degree of adipose change in the inferior rectus muscle, smoking status, history of orbital radio

The horizontal dark oculomotor rest position.

BACKGROUND: This study sought to investigate whether eye dominance and age are related to the stimulus-free oculomotor resting state described via the dark disconjugate position (near or far), the dark conjugate position (left to right), and the near dissociated phoria. METHODS: Nineteen non-presbyopes and 25 presbyopes with normal binocular vision participated in two identical sessions. The left-eye and the right-eye positions were recorded using a video-based infrared eye tracker while the subjects were in total darkness. Dark disconjugate responses and dark conjugate responses were calcula

Surgical management of vertical ocular misalignment in thyroid eye disease using an adjustable suture technique.

PURPOSE: To report a surgical approach to managing hypotropia associated with thyroid eye disease (TED) that uses adjustable sutures. METHODS: The medical records of consecutive patients with TED and hypotropia who underwent adjustable suture inferior rectus muscle recession with or without superior rectus muscle recession of the hypertropic eye were reviewed retrospectively. The initial goal was undercorrection in primary gaze and no hyperdeviation in downgaze. Outcomes were rated according to the presence of diplopia in primary and reading positions as excellent (none), good (no diplopia wi

Management and outcomes of cataract in children: the Toronto experience.

PURPOSE: To review the management and outcomes of pediatric patients with cataract who were managed by one of two surgeons at a single institution. METHODS: An observational series of consecutive cases identified from the hospital's outpatient billing records and surgical data program were used. Visual acuity was measured with the Snellen or Allen charts. Glaucoma was defined as IOP >20 mm Hg with clinical signs of glaucoma or visual field loss. Procedures for aphakic/pseudophakic glaucoma were excluded from analysis of additional surgeries performed subsequent to cataract extraction. RESULT

Skew deviation: a precursor to basilar artery thrombosis.

The aim of this study was to describe skew deviation and vertical nystagmus as the initial signs for basilar artery thrombosis, a life-threatening disease. A 51-year-old woman complained of vertical diplopia for more than 20 h. A computed tomography of the brain was normal, but subsequently the patient developed additional symptoms including nausea, ventilation problems (dyspnoea) and somnolence. Neuro-ophthalmological evaluation revealed a skew deviation and a vertical nystagmus. Magnetic resonance imaging allowed the diagnosis of basilar artery occlusion. An emergency intervention with cereb

Magnetic resonance imaging of the functional anatomy of the inferior oblique muscle in superior oblique palsy.

PURPOSE: To study size and contractility of the normal inferior oblique (IO) muscle using high-resolution magnetic resonance imaging (MRI) and to evaluate abnormalities of the superior oblique (SO) and IO muscles in chronic SO palsy. DESIGN: Prospective, case control study. PARTICIPANTS: Thirteen patients with SO palsy and 17 orthotropic subjects. METHODS: High-resolution, surface coil MRI was used to obtain sets of contiguous, 2-mm thick coronal and sagittal images repeated in multiple gaze directions. Digital image analysis was used to measure IO and SO muscle cross-sectional areas for ev

The incidence of diplopia following coronal and translid orbital decompression in Graves' orbitopathy.

PURPOSE: Firstly, to assess the incidence of induced diplopia following orbital decompression in patients with Graves' orbitopathy. Secondly … pre-operative motility impairment developed diplopia in all directions. Twenty-five per cent needed strabismus surgery (9% multiple times). High satisfaction scores were noted after both types of orbital decompression. Through a review of the literature, several factors that may add to heterogeneous results were identified, including definition of diplopia, …

[Ophthalmologic evaluation at the age of 2 years of newborn infants hospitalized in the neonatology center at Rennes].

The eyes of 1 466 children (88.3% of the total) who were admitted to the Centre for premature babies and the neonatal intensive care unit at Rennes between 1973-1975 were re-examined at the age of 2 years. There were 839 premature and 226 term babies whose weight was appropriate for gestational age, and 74 premature and 327 term babies who were small for dates. 95 (6.5%) had eye disorders at the age of 2. Abnormalities were more common in infants with a Birth weight of less than 1 500 g and in those with a gestional age of less than 28-30 weeks. Boys were more commonly affected than girls. 38%

Ophthalmoplegia and Ondine's curse.

Ocular abnormalities and psychomotor difficulties were prominent in two unrelated children; in addition, the older child had respiratory irregularity during sleep. The pathologic findings included lesions of the optic nerve in the case with available material and established the diagnosis of Leigh's subacute necrotizing encephalopathy. This disorder is thought to result from inhibition of a thiamine-dependent enzymatic process and may be modified by greatly increased thiamine intake. Suspicion of the diagnosis in a child with ophthalmoplegia or other ocular abnormalities may lead to earlier re

Additional findings included a nasal-root dermoid cyst excised in early childhood, strabismus, conductive hearing loss, kyphoscoliosis and a kinked aorta. Performance of Automated Vision Screening and Refraction in Young Children in Belize.

PURPOSE: To determine how automated instrument-based screening with the Spot Vision Screener (SVS) (Welch Allyn, Inc/Baxter) compares with standard visual acuity testing in Belizean schoolchildren, and to compare refractive results of the SVS and QuickSee hand-held autorefractor (PlenOptika). METHODS: During a school-based vision screening program in Orange Walk, Belize, SVS performance was evaluated against visual acuity testing as the reference standard. Children failing visual acuity per American Association for Pediatric Ophthalmology and Strabismus (AAPOS)/National Center for Children's …
