Quadriceps tendon vs. patellar tendon autograft for ACL reconstruction using a hardware-free press-fit fixation technique: comparable stability, function and return-to-sport level but less donor site morbidity in athletes after 10 years.

INTRODUCTION: The use of quadriceps tendon-patellar bone (QTB) autograft for anterior cruciate ligament (ACL) reconstruction is gaining momentum. Yet, long-term results that compare this procedure with established methods are lacking. The aim of this study was to report and compare long-term results of ACL reconstruction using QTB autografts versus bone-patellar tendon-bone (BPTB) autografts, both anchored using a hardware-free press-fit fixation technique. MATERIALS AND METHODS: 60 athletes (Tegner score ≥6) with primary ACL rupture were prospectively randomized into two groups. 56 patients

Spontaneous phase separation-driven Janus hydrogel spatiotemporally orchestrates the fibrotic progression for functional tendon regeneration.

Postoperative tendon regeneration hinges on the precise spatiotemporal orchestration of fibrotic progression, aberrant fibrogenesis instead drives pathological adhesion formation and impairs functional recovery. However, current strategies often target isolated aspects of fibrotic progression and rely on complex fabrication procedures. Here, we engineer a Janus hydrogel (SPS-JH) via spontaneous phase separation-driven stratification and photopolymerization of methacrylated hyaluronic acid (HAM) and poly(lactide)-b-poly(propylene glycol)-b-poly(lactide) dimethacrylate (PLD). This innate water/o

Medial patellofemoral ligament reconstruction in osteogenesis imperfecta using quadriceps tendon graft: a rare case report.

Osteogenesis imperfecta (OI) is a rare connective tissue disorder associated with bone fragility and ligamentous laxity, predisposing patients to recurrent patellar instability. Surgical treatment of recurrent patellar instability is challenging because standard medial patellofemoral ligament (MPFL) reconstruction techniques involving patellar tunneling may increase the risk of patellar fracture. We present a 15-year-old male patient with OI type V and recurrent patellar dislocation treated with a minimally invasive MPFL reconstruction using a pedicled quadriceps tendon graft. This patella-spa

Open Traumatic Achilles tendon Rupture in a Pediatric Patient Managed by Modified Kessler Technique: A Case Report.

INTRODUCTION: Open traumatic Achilles tendon rupture is uncommon, particularly in the pediatric population. Unlike closed spontaneous ruptures - which predominantly affect middle-aged adults - sharp lacerating injuries in children pose challenges due to contamination risk and the close relation of neurovascular structures in the posterior ankle. We report a case of complete open Achilles tendon rupture in a 9-year-old boy following a sharp metallic object injury, treated with primary tendon repair using the Modified Kessler technique. This case adds to the limited pediatric literature and may g

Early-life high-fat diet exposure increases Achilles tendon stiffness and induces transcriptomic alterations.

High-fat diet (HFD) exposure is a recognized risk factor for tendinopathy and impaired tendon healing in adults, yet its effects on baseline tendon properties following early-life exposure remain poorly understood. In this study, we investigated the structural, biomechanical, and transcriptomic characteristics of Achilles tendons in 12-week-old rat offspring born to dams fed an HFD or a normal diet during gestation. Compared with controls, HFD-exposed tendons exhibited a significant reduction in anteroposterior diameter on sagittal ultrasound imaging. However, histological assessment revealed …

Isometric points in cerclage for patellar tendon repair: a 3-dimensional, weightbearing computed tomography kinematic simulation.

PURPOSE: The gold standard for the management of ruptured patellar tendon is the surgical repair with nonabsorbable sutures and additional reinforcement e.g. cerclage wires as described by McLaughlin in 1947. Despite the early description, the exact placement of such cerclage wires remains unclear. The aim of the study was (1) to analyze how the cerclage elongates during knee flexion and (2) to define the insertion points with the most isometric behavior at the patella and tibial tuberosity (TT). METHODS: 3D simulations of 10 healthy knees in the range from 0° to 120° of flexion using weightb

Outcomes of Acute Achilles Tendon Repair with Platelet-Rich Plasma Augmentation: A Comprehensive Review of Comparative Studies.

Background/Objectives: Platelet-rich plasma (PRP) has been proposed as a biological augmentation strategy in the surgical repair of acute Achilles tendon rupture (ATR). However, clinical evidence remains heterogeneous, and its impact on structural and functional recovery is unclear. This review synthesizes comparative clinical evidence on PRP augmentation in surgical acute ATR repair. Methods: A comprehensive review of comparative clinical studies was conducted using PubMed, MEDLINE, Scopus, and Cochrane Central databases. Seven studies, including three randomized controlled trials, met the in

Prospective Study of Clinical and Functional Outcomes after Medial Collateral Ligament Reconstruction by Rerouting Hamstring Tendon.

BACKGROUND: Medial collateral ligament (MCL) injuries are among the most frequently encountered ligamentous injuries of the knee, accounting for approximately 35%-40% of all knee ligament injuries in active and athletic populations. High-grade MCL injuries, particularly when associated with multiligament knee trauma, may result in persistent valgus instability and functional impairment, often necessitating surgical reconstruction in chronic or symptomatic cases. MATERIALS AND METHODS: This prospective cohort study was conducted at a tertiary care teaching hospital between November 2022 and No

An Unusual Case of a Mass-Like Hydroxyapatite Deposition Disease Causing Forearm Flexor Tendon Calcific Tendinitis.

Hydroxyapatite deposition disease (HADD) is a relatively common disorder characterised by periarticular deposition of calcium hydroxyapatite, most frequently involving tendons of the glenohumeral joint. Although calcific deposits typically produce a self-limiting inflammatory response, mass-like presentations at atypical sites can mimic trauma, infection or soft tissue neoplasia, posing a significant diagnostic challenge. We report an unusual presentation of mass-like calcific tendinitis within the flexor carpi ulnaris (FCU) tendon of the distal forearm-an atypical location for HADD presenting…

Tendon stem/progenitor cells in heterotopic ossification: functional regulation, molecular mechanisms and targeted therapeutic strategies.

UNLABELLED: Heterotopic ossification (HO) is a pathological process in which ectopic bone tissue forms within soft tissues such as tendons, resulting in pain, limited range of motion, and functional disability that reduces the quality of life. In recent years, tendon stem/progenitor cells (TSPCs) have been considered a key cellular source of HO due to their inherent multilineage differentiation potential. This review aims to systematically elaborate on the advances in the functional regulation and mechanisms of TSPCs in HO. Existing evidence (including lineage tracing and clinical studies) ind…

Successful Healing of a Chronic Distal Toe Ulcer with Tendon Exposure Using the Topical Macrophage Regulator ON101.

Diabetic foot ulcer (DFU) is one of the most challenging complications of diabetes mellitus. Chronic hyperglycemia contributes to impaired immune function, characterized by failure of macrophage polarization from the proinflammatory M1 to the prohealing M2 phenotype, together with increased release of inflammatory mediators. ON101 is a novel macrophage-regulating topical agent derived from botanical extracts, designed to suppress proinflammatory cytokines, restore M1/M2 macrophage balance, and promote collagen synthesis. We report the case of a 77-year-old man with diabetes mellitus who presen

Feasibility and repeatability of MEG-compatible patellar-tendon stimulator for eliciting knee-joint proprioceptive cortical responses.

BACKGROUND: Cortical processing of proprioceptive-dominant afference arising from the knee joint is largely unknown. Magnetoencephalography (MEG) can be used to quantify the cortical processing of the proprioceptive afference, but MEG-compatible and well-controlled stimulation of the knee joint is technically challenging, and thus has received less attention. NEW METHOD: We introduced a novel MEG-compatible stimulator that delivers controlled patellar tendon taps and elicits proprioceptive-dominant afferent input to the cortex in the knee joint. The stimulus intensity is adjustable, allowing

Ceramide-PKCζ signaling axis mediates burn-enhanced ectopic calcification during Achilles tendon healing.

Burn injury frequently leads to heterotopic ossification (HO) during tendon healing, but the underlying molecular mechanisms remain poorly understood. This study investigated the role of the ceramide-protein kinase C zeta (PKCζ) signaling axis in burn-enhanced ectopic calcification during Achilles tendon healing and evaluated the therapeutic potential of PKCζ inhibition. A combined burn injury and Achilles tendon puncture model was established in C57BL/6 mice, with sham control, tendon puncture, and burn combined tendon puncture groups. Micro-computed tomography analysis revealed significantly

A coenzyme-based self-stabilized Janus adhesive patch for spatiotemporal regulation of sutureless diabetic tendon healing.

Chronic inflammation and adhesion formation severely impede the healing process of diabetic tendons, while current surgical sutures and pharmacological interventions often fail to restore microenvironmental homeostasis. Here, we report a coenzyme-based self-stabilized Janus adhesive patch for spatiotemporal regulation of sutureless diabetic tendon healing. Unlike strategies relying on exogenous agents, we utilized the strong hydrogen bonding interactions between endogenous coenzyme α-lipoic acid (LA) and its potassium salt (LAK) to engineer a stable binary supramolecular adhesive layer (P (Lx-…

Timing of Achilles Tendon Revision within 2 Months of Primary Repair Affects Incidence of Acute Complications.

BACKGROUND: Early revision of primary Achilles tendon repair is sometimes necessary, and the Achilles tendon's inherent hypovascularity creates a uniquely challenging biological environment for reoperation. However, the optimal timing for revision and its associated complication risks remain poorly characterized. PURPOSE: To evaluate the relationship between the timing of early Achilles tendon revision, performed within 8 weeks of primary repair, and the incidence of acute postoperative complications. METHODS: Using the PearlDiver M91Ortho data set, patients who underwent Achilles tendon rev
