Grown-up congenital heart (GUCH) disease: current needs and provision of service for adolescents and adults with congenital heart disease in the UK.

This report addresses the needs and problems of grown-up congenital heart (GUCH) patients and makes recommendations on organisation of national medical care, training of specialists, and education of the profession. The size of the national population of patients with grown-up congenital heart disease (GUCH) is uncertain, but since 80-85% of patients born with congenital heart disease now survive to adulthood (age 16 years), an annual increase of 2500 can be anticipated according to birth rate. Organisation of medical care is haphazard with only three of 18 cardiac surgical centres operating o

Interventions to Improve Outpatient Follow-up for Hospitalized Patients With Alcohol Use Disorder : A Systematic Review.

BACKGROUND: Hospitalizations for alcohol use disorder (AUD) are common and morbid. Outpatient follow-up after discharge is critical for reducing alcohol use and subsequent readmissions, but effective interventions to support this transition are unknown. PURPOSE: To summarize evidence on the effectiveness of hospital-based interventions to improve outpatient follow-up for AUD. DATA SOURCES: Ovid MEDLINE, EMBASE, Cochrane Central Register of Controlled Trials, PsycInfo, and CINAHL (January 2000 to February 2025). STUDY SELECTION: Two authors independently reviewed abstracts and full-text arti

Practical Management of Adverse Events Associated with Bispecific Antibodies for the Treatment of Multiple Myeloma: A Qualitative Interview Study

ABSTRACT PURPOSE Bispecific antibodies (BsAbs) represent a major advancement in the management of relapsed/refractory multiple myeloma (RRMM), offering high response rates even in heavily pretreated patients. However, their use presents operational, safety, and supportive care complexities that require coordinated care teams, and evolving infrastructure. This manuscript summarizes best practice recommendations for adverse event (AE) management, outpatient operational models, referral pathways, and emerging strategies to optimize long-term tolerability. METHODS Medlive—A PlatformQ Health Brand

Virtual Preoperative Education in Bariatric Surgery: Patient Acceptance and Travel-Related Carbon Savings.

BACKGROUND: Comprehensive preoperative patient education is critical for optimizing postoperative outcomes and long-term weight management after bariatric surgery. Traditional in-person educational sessions, however, generate considerable greenhouse gas emissions, primarily due to participant travel. Telemedicine offers an environmentally sustainable alternative with the potential to reduce the carbon footprint within obesity care. This study aimed to quantify savings in greenhouse gas emissions and assess patient acceptance of virtual preoperative obesity educational sessions compared to conv

A simple intervention to improve day 1 mobility following fragility hip fracture: the Cardiac Chair Position : a single-centre cohort study of 270 patients.

AIMS: Fragility hip fractures are a growing concern, increasing year on year by just over 1%. These injuries carry high mortality rates and often result in poor functional outcomes, especially in frail patients. Early mobilization after surgery has been linked to better survival and shorter hospital stays. While Enhanced Recovery After Surgery (ERAS) protocols are well established in elective orthopaedics, their application in hip fracture care remains limited due to the frailty of this patient group. METHODS: The Cardiac Chair Position (CCP), originally used in cardiac surgery, may support e

Risk factors associated with moderate and severe traumatic brain injury mortality in admitted older adults: a retrospective, single-center study at a level II trauma center.

PURPOSE: Approximately one in four adults aged ≥65 years experiences a fall each year, and over one-third of these events require medical care. Older adults are at higher risk of complications because of comorbidities. Most prior studies have focused on incidence rather than outcomes, and few have evaluated the association between age and traumatic brain injury (TBI) mortality. This study assessed risk factors and risk-adjusted mortality and morbidity among older adults with moderate or severe TBI. METHODS: We conducted a retrospective review of data from a single-center trauma registry. Incl

Mindfulness-Based Stress Reduction adapted to pregnant women with psychosocial vulnerabilities - a randomized feasibility trial in a Danish hospital-based outpatient setting.

BACKGROUND: Supporting prenatal mental health in women with a preconception history of mental disorders or psychosocial vulnerability is important for a positive transition to parenthood. There is a need for more evidence-based and non-pharmacological mental health interventions in antenatal care. The primary objective of this trial was to evaluate the feasibility of prenatal MBSR, which is an adaption of Mindfulness-based stress reduction (MBSR). This to inform a randomized controlled trial. METHODS/DESIGN: The trial was conducted at an obstetric outpatient clinic at Copenhagen University Ho

Long-Term Healthcare Utilization and Outcomes in Patients Hospitalized for Heart Failure With and Without Atrial Fibrillation.

The long-term association between AF and clinical outcomes, healthcare resource utilization, and healthcare costs among patients with HF remains underexplored. We conducted an exploratory analysis of 5-year outcomes among patients enrolled in the patient-centered care transitions in HF (PACT-HF) stepped-wedge cluster randomized trial who were hospitalized for HF and discharged alive between February 2015 and March 2016. Patients were stratified by baseline AF status. Administrative health databases were linked to assess mortality, rehospitalizations, emergency department visits, healthcare uti

Implementation of a lisocabtagene maraleucel chimeric antigen receptor T-cell therapy outpatient monitoring program: Multicenter nursing/advanced practice provider perspectives.

OBJECTIVES: Lisocabtagene maraleucel (liso-cel; Breyanzi), an autologous chimeric antigen receptor T-cell therapy, has demonstrated efficacy and safety in relapsed/refractory large B-cell lymphoma, chronic lymphocytic leukemia/small lymphocytic lymphoma, follicular lymphoma, and mantle cell lymphoma. Outpatient monitoring after liso-cel administration has shown lower health care resource utilization and cost and high patient satisfaction. However, outpatient monitoring programs require well-designed processes with collaborations from multiple stakeholders across several facilities. The study o

Addressing Hypertension and Diabetes through Community-Engaged Systems (ANDES) in Puno, Peru: rationale and study protocol for a hybrid type 2 effectiveness and implementation randomized controlled trial.

BACKGROUND: Hypertension is the most common cardiovascular disease in Peru despite the availability of cost-effective, evidence-based treatment. Here we describe the rationale and study design for a hybrid type 2 randomized controlled trial to test the implementation and effectiveness of a community health worker (CHW)-led hypertension control program within the national primary care system in Puno, Peru. METHODS: We will recruit 1068 adult participants with hypertension aged ≥ 18 years in Puno, Peru, via facility-based enrollment and community health fairs. Participants will be individually

Prevention of psychosocial distress consequences in somatic hospital inpatients via a stepped and collaborative care model: protocol of SomPsyNet, a stepped wedge cluster randomised trial.

INTRODUCTION: Approximately 30% of somatic hospital inpatients experience psychosocial distress, contributing to increased (re-)hospitalisation rates, treatment resistance, morbidity, and direct and indirect costs. However, such distress often remains unrecognised and unaddressed. We established 'SomPsyNet', a 'stepped and collaborative care model' (SCCM) for somatic hospital inpatients, aiming at alleviating this issue through early identification of distress and provision of appropriate care, providing problem-focused pathways and strengthening collaborative care. We report the protocol of t

Technology-enabled activation of skin cancer screening for hematopoietic cell transplantation survivors and their primary care providers (TEACH).

BACKGROUND: Hematopoietic cell transplantation (HCT) is a curative option for a growing number of patients with hematologic diseases and malignancies. However, HCT-related factors, such as total body irradiation used for conditioning, graft-versus-host disease, and prolonged exposure to immunosuppressive therapy, result in very high risk for melanoma and non-melanoma skin cancer (NMSC). In fact, skin cancer is the most common subsequent neoplasm in HCT survivors, tending to develop at a time when survivors' follow-up care has largely transitioned to the primary care setting. The goal of this s

A Cost-Effectiveness Analysis Of Pregabalin For The Treatment Of Patients With Chronic Cervical Pain With A Neuropathic Component In Japan.

PURPOSE: To evaluate the cost-effectiveness of pregabalin versus other analgesics among patients with chronic cervical pain with neuropathic components during routine clinical practice in Japan. PATIENTS AND METHODS: The analysis considered patients with chronic cervical pain with a neuropathic pain component (radiating pain to the upper limb) and who were treated with pregabalin with or without other analgesics (pregabalin-containing treatments) or other analgesics alone (usual care) for 8 weeks. Other analgesics included non-steroidal anti-inflammatory drugs (NSAIDs), weak opioids, antidepr

Effect on birthweight of an antenatal multiple micronutrient supplementation programme compared with iron-folic acid supplementation: a cluster-randomised, controlled trial in 85 698 Ethiopian women.

BACKGROUND: Prenatal multiple micronutrient supplementation (MMS) including iron, folic acid, and other essential micronutrients, has shown potential to improve birth outcomes in controlled studies. We did an effectiveness study in Ethiopia to determine the effect on mean birthweight of MMS provided as part of routine antenatal care, relative to iron-folic acid supplementation (IFA). METHODS: A pragmatic, two-arm, facility-based, cluster-randomised controlled trial was conducted across 42 districts in five regions of Ethiopia to determine the effect on birthweight of MMS relative to IFA. Do free healthcare policies play a role in expanding national health insurance enrollment among informal sector workers? The case of the Afya Care pilot program in Kenya.

BACKGROUND: Several Sub-Saharan African countries have transitioned from fiscally unsustainable free healthcare policies to national health insurance schemes. These national schemes often expect informal sector workers to contribute directly to the scheme. Although enrollment in the national scheme is critical to achieving universal health coverage, informal sector workers’ inclusion remains challenging. In this paper, using the case of the Afya-Care free healthcare policy pilot in Kenya, we examine an issue that has thus far received limited attention in the literature: the extent to which ea

Extremity fractures, attempted suicide, blood transfusion and thromboembolic events are independent risk factors for a prolonged hospital stay in severely injured elderly.

METHODS: Due to demographic change, the number of polytraumatized geriatric patients (> 64 years) is expected to further increase in the coming years. In addition to the particularities of the accident and the associated injury patterns, prolonged inpatient stays are regularly observed in this group. The aim of the evaluation is to identify further factors that cause prolonged inpatient stays. A study of the data from the TraumaRegister DGU® from 2016-2020 was performed. Inclusion criteria were an age of over 64 years, intensive care treatment in the GAS-region, and an Injury Severity Score (I

The midlife transition and the risk of cardiovascular disease and cancer Part I: magnitude and mechanisms.

Heart disease and cancer are the leading causes of death in the United States. In women, the clinical appearance of both entities-coronary heart disease and cancer (breast, endometrium, and ovary)-escalate during the decades of the midlife transition encompassing the menopause. In addition to the impact of aging, during the interval between the age of 40 and 65 years, the pathophysiologic components of metabolic syndrome also emerge and accelerate. These include visceral adiposity (measured as waist circumference), hypertension, diabetes, and dyslipidemia. Osteoporosis, osteoarthritis, sarcope

Homelessness, Unsheltered Status, and Risk Factors for Mortality: Findings From the 100    000 Homes Campaign.

OBJECTIVES: People who live in unsheltered situations, such as the streets, often have poorer health, less access to health care, and an increased risk of premature mortality as compared with their sheltered counterparts. The objectives of this study were to (1) compare the characteristics of people experiencing homelessness who were sleeping primarily in unsheltered situations with those who were accessing homeless shelters and other sheltered situations, (2) identify correlates of unsheltered status, and (3) assess the relationship between unsheltered status and increased risk of mortality.
