Effect of caudal block injection speed on optic nerve sheath diameter in pediatric surgeries: A prospective observational cohort study.

Perioperative pain management in children is challenging due to limited communication and the risk of long-term psychological effects. Regional anesthesia, particularly the caudal block (CB), is frequently used in pediatric surgery for lower abdominal and urogenital procedures. However, the influence of injection speed during CB on intracranial and hemodynamic responses remains unclear. This prospective observational cohort study included 64 pediatric patients (American Society of Anesthesiologists I-II, aged 1-7 years) undergoing elective lower abdominal or urogenital surgery under general an

Ultrasound-Guided Erector Spinae Plane Block for Refractory Chest and Epigastric Pain Control in Sickle Cell Crisis: A Case Report.

This report aims to demonstrate the potential role of ultrasound-guided erector spinae plane block (ESPB) in managing refractory epigastric and chest pain due to vaso-occlusive crises (VOCs). An 18-year-old girl with sickle cell disease (SCD) presented to the emergency department with refractory epigastric and chest pain due to VOC. An ultrasound-guided left-sided erector spinae plane block (ESPB) using 30 mL of 0.2% ropivacaine provided rapid and sustained analgesia, reducing her Numerical Rating Scale (NRS) pain score from 8/10 at baseline to 0/10 at 2 hours postblock. She remained pain-free

Efficacy of 810 nm low-level laser therapy on muscle regeneration after bupivacaine-induced myonecrosis in a rabbit model.

Skeletal muscle tissue has considerable regenerative capacity following injury; yet, this process may be gradual and constrained. Bupivacaine, a local anaesthetic, serves as a model for inducing muscle damage in experimental research. Low-level laser treatment (LLLT), also referred to as photobiomodulation (PBM) has demonstrated considerable efficacy in promoting the healing of skeletal muscle tissue. The aim of this study was to evaluate the effect of LLLT (810 nm) on skeletal muscle regeneration following bupivacaine-induced myonecrosis in a rabbit model. This study was structured as a contr

Intravenous Lidocaine Decreases Pain Scores 24 Hours Post Cardiac Surgery: Substudy of a Randomized Controlled Trial.

BACKGROUND: Post cardiac surgery pain remains a problem for a significant number of patients. While opioids have long been used for cardiac surgery pain control and hemodynamic stability, methods to improve pain control while also reducing reliance on opioids are desired. Intravenous lidocaine has shown promise for pain and opioid reduction in multiple operative settings, yet its role in cardiac surgery lacks conclusive data. OBJECTIVE: To determine the effect of intravenous lidocaine on pain scores and opioid consumption in the first 48 hours post cardiac surgery. STUDY DESIGN: Preplanned s

Efficacy and Safety of Adding 1 μg/Kg Dexmedetomidine to 20 ml Bupivacaine in Supraclavicular Brachial Plexus Block: A Randomized Trial.

BACKGROUND: For surgical procedures of the upper limbs, ultrasound-guided supraclavicular brachial plexus block (SCBPB) represents a safe substitute for general anesthesia. The present study evaluated the effectiveness and safety of incorporating 1μg/kg dexmedetomidine (DEX) into 20 ml bupivacaine, as opposed to using 20 ml and 30 ml bupivacaine without additives, in SCBPB. METHODS: This randomized, controlled, double-blind study included 75 patients assigned to elective upper-limb surgery under the mid-humerus level. Patients were randomized into three equal groups to receive US-guided SCBPB

Maximum effective volume of 0.25% ropivacaine for preventing hemidiaphragmatic paralysis in 90% of patients undergoing shoulder arthroscopy: a biased coin up-and-down sequential allocation trial for ultrasound-guided superior trunk block.

BACKGROUND: Compared to interscalene block (ISB), the superior trunk block (STB) lowers the incidence of hemidiaphragmatic paralysis (HDP) without compromising analgesic efficacy, making it a valuable alternative. However, the lack of consensus on the optimal local anesthetic (LA) dose for STB limits its standardized clinical application. This study employed a dose-finding protocol to determine the maximum effective volume of 0.25% ropivacaine required to prevent HDP in 90% of patients (MEV90) undergoing shoulder arthroscopy with ultrasound-guided STB. METHODS: A biased coin design was used f

Dural puncture epidural technique with 23-, 25- or 27-gauge pencil point spinal needles versus standard epidural technique for labor analgesia: a randomized trial.

BACKGROUND: Dural puncture epidural (DPE) improves labor analgesia by reducing numeric rating scale (NRS) score, accelerating onset, and enhancing sacral blockade. Although larger-gauge needles may potentiate local anesthetic diffusion and improve analgesic quality, current clinical practice favors smaller-gauge needles (25-gauge to 27-gauge) to mitigate post-dural puncture headache (PDPH) risks. This randomized trial specifically evaluates the differential impacts of 23-gauge, 25-gauge, and 27-gauge needles on both analgesic efficacy and procedure-related complications. METHODS: In this rand

A Pilot Study to Investigate the Effects of Bupivacaine Nasal Treatment in Palliative Care Patients with Swallowing Difficulty.

Dysphagia among end-of-life patients in palliative care is debilitating and requires symptomatic management. Current methods involving usage of IV fluids, total parenteral nutrition, intermittent oral liquid feeds, use of naso-gastric tubes or administration of nasal saline drops are inadequate in providing relief to the patient, warranting exploration of newer methods. At SSCHRC, a novel intervention of intranasal administration of Bupivacaine, a widely used anesthetic has been used successfully to treat end-of-life cancer patients, providing temporary relief for about half an hour with each

The Interfascial Plane Block: An Adjunctive Ultrasound-Guided Block for Proximal Humerus Fracture and Fracture Dislocation Anesthesia and Analgesia in the Emergency Department.

BACKGROUND: Proximal humerus fractures (PHFs) and fracture-dislocations (PHFDs) are frequently encountered in the emergency department (ED). Adequate pain control for these injuries can be challenging, and traditional methods, including opioid analgesics and other blocks may pose significant risks such as medication side-effects and hemidiaphragmatic paresis. The interfascial plane block (IPB) is an emerging, ultrasound-guided technique that targets the fascial plane between the deltoid and subscapularis muscles, potentially offering safer and more focused analgesia for acute shoulder injuries
