Stress avulsion of the tibial tuberosity after tension band wiring of a patellar fracture: a case report.

INTRODUCTION: To the best of our knowledge there is no other report of an elderly patient who was surgically treated for a patellar fracture with tension band wiring and who subsequently suffered from an avulsion fracture of the tibial tuberosity. The combination of a patellar fracture and avulsion of the patellar ligament has only been described as complication after bone-patellar tendon-bone anterior cruciate ligament reconstructions. However, due to demographic changes and more elderly patients treated this injury may become more frequent in future. CASE PRESENTATION: We present the case o

Endoscopic-assisted anterior transarticular screw fixation and arthrodesis for atlantoaxial instability via a retropharyngeal approach: technical note and preliminary clinical outcomes.

BACKGROUND: Atlantoaxial instability (AAI) is complex and challenging to manage due to the biomechanical and neurovascular significance of the craniocervical junction. Posterior fixation remains the gold standard but may be contraindicated or infeasible in select patients. Although anterior transoral/transnasal approaches provide adequate access to the atlantoaxial joints, their deep, narrow, and contaminated surgical corridor may reduce visualization and increase postoperative infection risks. Full-endoscopic anterior retropharyngeal procedures also have blind zones that limit visualization,

Biomechanical comparison of five fixation methods in minimally invasive hallux valgus osteotomy: a three-dimensional finite element analysis.

OBJECTIVE: Although minimally invasive osteotomy for hallux valgus employs a variety of internal fixation methods, systematic biomechanical evidence evaluating the stability and strength of different fixation configurations is lacking. This study aimed to quantitatively compare the biomechanical properties of five internal fixation techniques using three-dimensional finite element analysis. METHODS: Based on CT data of the foot of an adult female patient with moderate hallux valgus (HVA 27.6°, IMA 12.4°), a finite element model of the post-osteotomy state was constructed. The following fixati

Single pararectus approach combined with three-dimensional guidance for the treatment of acetabular fracture.

BACKGROUND: Surgery for acetabular fractures involving both columns is difficult and traumatic, making it necessary to explore a minimally invasive and accurate surgical method. METHODS: This retrospective case-control study analyzed the clinical data of 34 patients and divided them into two groups: a control group (9 males and 8 females) and a research group (11 males and 6 females) with acetabular fractures involving the anterior and posterior columns. All patients were placed in the supine position via the pararectus approach. A three-dimensional (3D) guide was placed at the position where

Comparison between Pin Fixation and Combined Screw Fixation in Proximal Chevron Metatarsal Osteotomy for Hallux Valgus Deformity Correction.

BACKGROUND: Problems associated with hallux valgus deformity correction using Kirschner-wire (K-wire) fixation include pin pullout and loss of stability. These complications are pronounced in the osteopenic bone, and few reports have focused on pin versus screw fixation. We examined the use of additional screw fixation to avoid these problems. The aim of this study was to compare outcomes of K-wire fixation (KW) and a combined K-wire and screw fixation (KWS). METHODS: Two groups with hallux valgus deformity, who were treated with a proximal chevron metatarsal osteotomy (PCMO), were compared b

Stability of Lisfranc injury fixation in Thiel Cadavers: Is routine fixation of the 1st and 3rd tarsometatarsal joint necessary?

BACKGROUND: There is debate as to whether a home run screw (medial cuneiform to 2nd metatarsal base) combined with k-wire fixation of the 4th & 5th tarsometatarsal joints is sufficient to stabilise Lisfranc injuries or if fixation of the 1st and 3rd tarsometatarsal joints is also required. Unlike the 2nd, 4th and 5th tarsometatarsal joints, stabilisation of the 1st and 3rd requires either intra-articular screw or an extra-articular plate which risk causing chondrolysis and/or osteoarthritis. The aims of this cadaveric study were to determine if routine fixation of the 1st and 3rd tarsometatars

Evaluating the Effect of Bone Morphogenetic Protein on Dental Implant Stability and Marginal Bone Level Following Immediate Implant Placement (Randomized Controlled Clinical Study).

BACKGROUND: Immediate implant placement has become a widely accepted treatment modality because of reduced treatment time and preservation of alveolar bone architecture. Recombinant human bone morphogenetic protein-2 (rhBMP-2) has been proposed to enhance peri-implant bone regeneration and implant stability. This study evaluated the effect of recombinant human bone morphogenetic protein-2 (rhBMP-2) combined with biphasic calcium phosphate graft on implant stability and peri-implant bone preservation following immediate implant placement. METHODS: Twenty-four patients who received 32 immediate

Treatment outcomes of pathological fractures in patients with benign bone tumors.

Pathological fractures of benign bone tumors can be difficult to treat, and the underlying pathogenesis remains unclear. Herein, we aimed to determine preventive measures for pathological fractures in patients with benign bone tumors based on fracture outcomes. Between April 2015 and July 2023, we enrolled 18 consecutive patients with oncological pathological fractures treated at our department. Age, sex, histopathological diagnosis, site of origin, whether incisional or pathological fracture, treatment, operative time, blood loss, recurrence, and characteristics of impending and pathological

Comparative analysis of posterior column screw trajectories: pararectus vs. lateral window of the pelvis.

OBJECTIVE: The pararectus approach (PRA) offers good visualization of the entry point for posterior column screw insertion with an adequate screw length. However, conventional practice typically employs the lateral window (LW) of the pelvic approach. This study aimed to compare the radiographic trajectory of posterior column screws inserted via the PRA with that of screws inserted via the LW approach. METHODS: We retrospectively analyzed 68 cases of posterior column screw placement during acetabular fracture surgery, beginning in March 2020, when metal-reformatted images could be generated fr

Percutaneous anterior to posterior screw fixation via lateral malleolus incision window for Bartoníček-Rammelt type 4 posterior malleolus fractures.

INTRODUCTION: The presence of a posterior malleolar (PM) fragment has a negative prognostic impact in ankle fractures. Percutaneous anterior to posterior screw fixation has been advocated for the treatment of ankle fractures involving the posterior malleolus and satisfactory results were demonstrated in several studies. The aim of this retrospective study was to evaluate the clinical effect of percutaneous anterior to posterior screw fixation via lateral malleolus incision window approach (APSF-LMA) for the treatment of posterior malleolus fracture (PMF) and present outcomes of patients with t

Innovative Technique of Anchoring the Upper End of the Cervical Plate With C1 Anterior Arch Screw for Upper Cervical Tumors.

BACKGROUND AND OBJECTIVES: Upper cervical spine bony tumors pose a challenge to surgeons because of their extensiveness and proximity to vital structures. Reconstruction following resection is necessary to prevent instability. Our objective is to describe a technical note on the C1 anterior arch screw with mesh cage reconstruction for anchoring the upper end of the cervical plate after resection of the upper cervical tumor. METHODS: A retrospective review of patients with primary bony tumors of the upper cervical spine after surgical resection between 2018 and 2022 was included in the study.
