Caudal clonidine-bupivicaine block with bladder hydrodistension: a novel combined treatment for the painful bladder.

The authors describe a new combination procedure consisting of bladder hydrodistension with clonidine-bupivicaine caudal block for the symptomatic relief of bladder pain. They report this new technique whereby patients who had tried multiple forms of therapy with little response, including bladder hydrodistension under general anaesthesia for their chronic pelvic bladder pain, responded to this novel combination therapy.

Multifactorial causes of irritating bladder symptoms in patients with Sjögren's syndrome.

UNLABELLED: AIMS Patients with Sjögren's syndrome (SS) are reported to have an increased severity of irritating bladder symptoms, including … ice water test (IWT) to detect the presence of a C-fiber mediated micturition pathway. Interstitial cystitis (IC) was diagnosed with post-hydrodilatation cystoscopic findings of glomerulations and a KCl test. RESULTS: These patients complained predominantly of overactive bladder symptoms (OAB), including frequency (n=20, 87%), nocturia (n=16, 66%), and ur …

Molecular epidemiology of uropathogenic Escherichia coli.

Escherichia coli is the most common cause of complicated as well as uncomplicated urinary tract infections (UTIs). Most of these uropathogenic E. coli (UPEC) strains exhibit certain virulence factors (VFs), including adhesins, iron uptake systems, synthesis of cytotoxins, and specific O:K:H serotypes. Molecular epidemiological studies of UPEC have contributed to the discovery of uropathogenic VFs, to an understanding of the pathogenesis of UTIs as ascending infections, and to the clarification of genetic linkages between different virulence genes such as pathogenicity islands (PAIs), which are

Gene therapy strategies for urological dysfunction.

Novel molecular techniques such as conventional and ex vivo gene therapy, and tissue engineering have only recently been introduced to the field of urology. The lower urinary tract is ideally suited for minimally invasive therapy, and also ex vivo approaches would limit the risk of systemic side effects. Muscle-derived stem cells have been used successfully to treat stress incontinence, and rats with diabetic bladder dysfunction benefited from nerve growth factor (NGF)-based gene therapy. Nitric oxide synthase and capase-7 might provide suitable gene therapy targets for erectile dysfunction an

Mixed effects of 2,6-dithiopurine against cyclophosphamide mediated bladder and lung toxicity in mice.

2,6-Dithiopurine (DTP) has been proposed as a possible chemopreventive agent because of its ability to react with electrophiles. Acrolein, an electrophilic metabolite of cyclophosphamide (CP) involved in the toxicities of this anticancer drug, can be scavenged by DTP. The present study examined the effect of DTP treatment on CP-mediated bladder and lung toxicity in male ICR mice. Mice fed a diet containing 4% DTP that were treated intraperitoneally (i.p.) with 350 mg/kg CP showed no significant bladder damage (measured as bladder blood content at 48 h) with respect to the group fed a control d

Coecocystoplasty: an evaluation of operative results.

Coecocystoplasty for substituting or augmenting the bladder is a relatively common operation in the United States, but not in Scandinavia. In order to focus the attention on this voiding-preserving operation we evaluated the late postoperative results of 8 patients. Median follow-up was 5 years. Seven of the patients had symptoms from a contracted bladder and 1 patient with epispadias was undiverted from a ureteroileocutaneostomy. There were some early postoperative complications. Two patients had a revision of the coecocystoplasty, 1 patient ended up with a ureteroileocutaneostomy and 1 patie

[Pharmacokinetic and clinical evaluation of cefoxitin].

An investigation conducted on healthy volunteers showed that cefoxitin quickly reaches high plasma concentrations, and is almost completely excreted via the urine within 6 hours. In a series of 21 cases treated with 2 g i.v. in 100 ml of a 5% glucose solution two or three times a day, a clinical cure was achieved in 20, and marked improvement in the remaining patient.

The prognostic significance of fever in childhood urinary infections: observations in 350 consecutive patients.

All infants and children with proven bacteriuria require urologic investigation and surveillance, but their prospects for developing chronic renal disease are variable. These differences are related to whether or not the bacteria have access to the kidneys. Such access correlates closely with the presence of an underlying congenital anomaly, particularly vesicoureteral reflux. Fever during an episode of active urinary infection is a strong indication that the patient has vesicoureteral reflux or some other significant uropathy and probably pyelonephritis.

Emerging roles of combined curcumin and berberine in disease modulation: a comprehensive review of mechanisms and therapeutic relevance.

In-Situ Self-Assembling Multifunctional Peptide-Hydrogel Enables Efficient Bladder Cancer Therapy via Sustained Release of Bacillus Calmette-Guérin and Enhanced Immune Response.

Intravesical instillation of bacillus Calmette-Guérin (BCG) remains the standard-of-care immunotherapy for high-risk nonmuscle-invasive bladder cancer (NMIBC). However, its clinical efficacy is frequently compromised by rapid drug washout resulting from intermittent bladder voiding, whereas intensified dosing regimens are associated with severe adverse events, including tuberculous cystitis and hematuria. Herein, we report an enzyme-responsive peptide, D-Nap-GFFYp, that undergoes alkaline phosphatase (ALP)-triggered self-assembly into a hydrogel selectively at the NMIBC tumor site. Using tumor…

Acute kidney injury during ruxolitinib therapy in pediatric patients with graft-versus-host disease: incidence, associated factors, and histopathological findings in a retrospective cohort study.

Ruxolitinib is a cornerstone treatment for steroid-refractory graft-versus-host disease (GVHD) following allogeneic hematopoietic stem cell … ney disease; biopsies excluded BK virus nephropathy. BK virus reactivation, BKV-related hemorrhagic cystitis, CMV reactivation, older age at transplantation, and higher baseline creatinine were significantly associated with AKI, whereas ruxolitinib dose was not. Post-transplant AKI in this setting appears multifactorial; controlled studies including a ruxo …

More than an infection: the ecological puzzle of recurrent urinary tract infections.

Recurrent urinary tract infections (rUTIs) represent one of the most common infectious conditions worldwide, yet their pathophysiology extends far beyond repeated episodes of acute bacterial cystitis. Increasing evidence indicates that recurrence may arise through overlapping mechanisms including reinfection from intestinal or periurethral reservoirs, intracellular bacterial persistence, microbial dysbiosis, impaired mucosal immunity and chronic inflammatory remodelling of the bladder microenvironment. Current diagnostic frameworks remain largely based on symptom-based definitions and standard…
