Phototherapy units in Brazil: are they effective?

The irradiance delivered by a phototherapy unit is an important contributing factor to its efficacy. However, measurement of irradiance during phototherapy requires the use of a photometer which is rarely available in the nurseries of developing countries. We assessed the irradiance of phototherapy units both in the laboratory and in all public hospitals of the City of Rio de Janeiro and found that: a) there is no consensus among companies manufacturing phototherapy units as to the number of fluorescent lights to be used in an equipment since this number varied from 4 to 8, b) over 1/3 of the …

Role of different methods for the detection of diarrhoeal pathogens in HIV-infected patients in Brazil.

To evaluate the ability of different diagnostic methods for the detection of AIDS-related diarrhoeal pathogens in developing countries, we studied 40 HIV-infected patients with diarrhoea. All patients were subjected to stool examinations for parasites, stool culture and peroral jejunal biopsy. Jejunal specimens were processed for histological examination with several stains and for transmission electron microscopy (TEM). Jejunal juice and mucosa were cultured. An aetiologic agent was found in twenty patients. Eleven stool specimens were positive for parasites and stool culture was positive in …

Evidence of previous infection with Mycobacterium avium-Mycobacterium intracellulare complex among healthy subjects: an international study of dominant mycobacterial skin test reactions.

Skin tests with 0.1 mL of intermediate-strength Mycobacterium tuberculosis purified protein derivative (PPD) and 0.1 mL of Mycobacterium avi … ion size showed similar trends. These data suggest that exposure to MAC is similar in developed and developing countries but that broad mycobacterial immunity is greater in developing countries and may contribute to the lower rates of disseminated MAC infections in AIDS in these areas.

Tropical peptic ulcer disease: an endoscopic study from rural Haiti.

We performed upper gastrointestinal endoscopy in 60 rural Haitian patients who complained of chronic upper abdominal pain. Twenty-five of 37 … observations as well as the observations of others indicate that peptic ulcer disease is common in developing countries, particularly among men. We hypothesize that this familiar abnormality reported from unfamiliar places represents the ordinary spectrum of peptic ulcer disease, but that inadequate treatment of recurrent episodes over time leads to obstruction. Epidemiology of travellers' diarrhoea.

Travellers' diarrhoea is the most frequent health problem encountered during a stay in developing countries. A recent study based on interviews with 16,568 charter-flight passengers returning to Europe from 13 destinations in various climatic regions provides epidemiological data on a worldwide scale. Significant differences in diarrhoeal incidence varied not only between individual destinations, but also between hotels in the same area. Travel characteristics, and a record of former tropical journeys influenced the incidence to a minor degree. Persons under 30 were more often affected than ol…

Paediatric fracture complications following traditional bone setting in low- and middle-income countries: a systematic review and meta-analysis protocol.

INTRODUCTION AND AIMS: Traditional bone setters (TBS)-lay practitioners who manage fractures without formal medical qualification-treat an estimated 10%-85% of fracture patients in low- and middle-income countries (LMICs). Children are disproportionately vulnerable due to physeal immaturity and lower diastolic blood pressure thresholds that increase susceptibility to compartment syndrome at lower absolute intracompartmental pressures than in adults. No prior systematic review has restricted analysis exclusively to the paediatric population or generated pooled meta-analytic estimates of complic

Residential greenness and perinatal depression in low- and middle-income countries: A multi-country population-based study.

Perinatal depression (PND) is a major public health concern in low- and middle-income countries (LMICs), yet the role of greenness remains insufficiently understood. This study examined the association between residential greenness and perinatal depression in three LMICs. We conducted a multi-country population-based cross-sectional analysis using nationally representative Demographic and Health Survey (DHS) data from Bangladesh, Mozambique and Nepal for 2022-2023, including 6889 perinatal women. Perinatal depression was assessed using the Patient Health Questionnaire-9. Residential greenness

Global Disparities in Teletherapy Adoption: A Cross-Income Analysis of Mental Health Access.

Mental health disorders affect nearly one billion people worldwide, yet treatment gaps exceed 75% in low- and middle-income countries. Teletherapy has emerged as a scalable solution, but its adoption differs sharply by economic context. This comparative ecological policy analysis used secondary aggregate data from WHO, World Bank, ITU, and national reports to examine teletherapy adoption in low-income (Nigeria, Kenya), middle-income (South Africa, India), and high-income countries (Norway, Canada). Descriptive statistics and simple linear regression were applied, with findings interpreted thro

Outcomes of Pneumonia in ICUs in Low- and Middle-Income Countries - A Systematic Review.

BACKGROUND: Community-acquired pneumonia (CAP) is a leading cause of intensive care unit (ICU) admission and death in low- and middle-income countries (LMICs). We conducted a systematic review and meta-analysis to estimate mortality among patients with CAP admitted to ICUs in LMICs. METHODS: We searched multiple databases using terms related to CAP admitted to the ICU and LMIC. We included both observational studies and clinical trials. The primary outcome was all-cause ICU, 28-day or 30-day mortality. Demographics, comorbidities, clinical characteristics, mechanical ventilation, and length o

Addressing the critical palliative care gaps in ethnic minority regions of low- and middle-income countries: a focus group study.

BACKGROUND: Palliative care is an important approach to relieving suffering and improving quality of life for people living with serious illness. Yet its development remains challenging in many low- and middle-income countries, particularly among vulnerable populations such as ethnic minorities. In ethnic minority regions of China, the palliative care implementation is shaped by distinctive social-cultural and practical realities. This study aims to systematically develop the major barriers and to generate actionable recommendations to policymakers to facilitate the advancement and implementat

Labor support in low- and middle-income countries - transformative impact of birth companions and doulas on maternal and neonatal outcomes: a narrative review.

BACKGROUND: Maternal and neonatal morbidity and mortality remain disproportionately high in low- and middle-income countries (LMICs), partly due to inadequate intrapartum support, psychosocial stress, and disrespectful maternity care. Continuous labor support from birth companions or doulas offers a low-cost, high-impact intervention with demonstrated benefits in high-income settings, yet LMIC-focused evidence remains fragmented. This review synthesizes current evidence on the effects of birth companionship and doula care on maternal, neonatal, psychosocial, and implementation outcomes in LMIC

A multicountry study to establish rates for pregnancy and neonatal outcomes in low- and middle-income regions.

BACKGROUND: Maternal vaccines can reduce the burden of diseases for both mother and neonate. The development and implementation of such vaccines require a thorough interpretation of safety data and standardized disease case definitions. This study aimed to evaluate the current rates for adverse pregnancy outcomes, maternal and neonatal events of interest (EOIs) that will be informative for preparing future phase III clinical trials on maternal vaccines conducted in low- and middle-income countries and further assisting in safety data interpretation (e.g., monitoring potential safety signals in

Association between caesarean birth and childhood acute respiratory infection in low- and middle-income countries: a meta-analysis of population-based data.

BACKGROUND: Caesarean section (CS) rates are rising across low- and middle-income countries (LMICs), frequently exceeding levels considered medically necessary. While CS can be lifesaving, emerging evidence from high-income settings suggests that delivery by CS may increase children's susceptibility to respiratory infections. However, population-based evidence from LMICs remains limited. We investigated the association between mode of delivery and acute respiratory infection (ARI) among children younger than five years across diverse LMIC settings. METHODS: We analysed nationally representati

The double burden of malnutrition: rethinking clinical and policy responses in the era of rising obesity in low- and middle-income countries.

OBJECTIVE: To discuss the growing challenge of the double burden of malnutrition (DBM), the co-existence of undernutrition and obesity, and the associated clinical and policy complexities in low- and middle-income countries (LMIC). DESIGN: This commentary synthesises evidence from recent multi-country and country-specific studies in sub-Saharan Africa and other LMIC. Many LMIC are typified by food insecurity, socio-economic inequalities and fragile health systems which drive DBM patterns, as well as informal community structures such as rotating savings groups which influence access to health

Divergent trajectories in fatal drug harms across high-income settings: a comparative synthesis of public surveillance indicators, 2015-23.

BACKGROUND: Fatal drug-related harms have risen across high-income countries over the past decade, yet national surveillance systems differ substantially in case definitions, reporting timeliness, and indicator coverage. These differences make it difficult to determine whether observed cross-national differences reflect epidemiological divergence or heterogeneous measurement. METHODS: To address this, we compared public surveillance indicators of fatal drug-related harm across the USA, Canada, England and Wales, and European countries reporting to the European Union Drugs Agency (2015-23), us

Effectiveness of Telehealth in HIV Management for People Living with HIV and Key Populations: A Systematic Review of Models, Adherence, Viral Suppression, and Behavioral Outcomes.

Human Immunodeficiency Virus (HIV) is a global health problem that causes many deaths, especially in low- and middle-income countries (LMICs). Most HIV cases occur in sub-Saharan Africa and Asia. In efforts to manage HIV, technologies such as telehealth have begun to be considered as an effective solution, both in high- and low-income countries. This study aims to identify telehealth models used in low-, middle-, and high-income countries and to evaluate their impact on treatment adherence, viral load, and HIV disease management. This study answers the question: "What telehealth models are use

Implementation of point of care HIV viral load monitoring for people living with HIV in low- and middle-income countries: A systematic review on implementation research outcomes.

BACKGROUND: Viral load monitoring has rapidly increased among people living with HIV(PLHIV) in low- and middle-income countries (LMICs), resulting in an increased laboratory workload. The use of innovative Point of Care (PoC) or near Point of Care (n)PoC HIV Viral Load (HIV VL) monitoring has enabled improved patient care, a reduction in laboratory workload, improved clinic retention and reduced turnaround time of results. However, implementation bottlenecks of such methods are uncertain, especially when PoC or (n)PoC is implemented in remote areas in low-volume clinics. OBJECTIVES: The main

Psychological predictors of adherence to antiretroviral therapy among people living with HIV in high-income countries: a systematic review protocol.

INTRODUCTION: People living with HIV (PLHIV) frequently face psychological challenges, including stigma, stress and social isolation, which can negatively affect adherence to antiretroviral therapy (ART). Even in high-income countries where treatment is accessible, poor adherence can lead to drug resistance, reduced immune function and early morbidity. This systematic review aims to synthesise evidence on the relationship between psychological and mental health factors and ART adherence among PLHIV in high-income settings. METHODS AND ANALYSIS: We will include studies published in any languag
