Distal radius fractures are one of the most prevalent fracture types. Dorsal tilt of the articular surface of the distal radius as assessed on the lateral radiograph is often used in determining the correct treatment method. This report seeks to highlight the importance of correct forearm positioning when obtaining distal radius radiographs, and the effects of pronation and supination on dorsal tilt measurements of the radius. It presents a case of a 35-year-old male with a malaligned left-sided distal radius fracture sustained during a sports-related fall.

The distal radioulnar joint (DRUJ) is vital to the stability and function of the wrist and forearm. The osseous morphology is variable and provides little stability. A complex of confluent soft tissues is the primary stabilizer; however, the contribution of each component has yet to be elucidated. It has become increasingly clear that the anatomic fixation of distal radius fractures restores DRUJ stability, obviating the need for additional DRUJ stabilization. This review will describe the anatomy and biomechanics of the DRUJ and discuss injury patterns, treatments, and clinical results.

We describe a 36-year-old man with a long-standing diagnosis of ulnar fibrous dysplasia with associated fracture of the ulna. He presented with a growing and increasingly tender forearm mass and was diagnosed with adamantinoma of the ulna, for which he underwent wide resection of the ulnar diaphysis followed by reconstruction with a vascularized fibula autograft. This case serves to emphasize the importance of performing a stepwise workup for the diagnosis of osseous neoplasms even in cases with long-standing diagnoses. [Orthopedics. 2024;47(2):e102-e105.].

Acquired brachial cutaneous dyschromatosis (ABCD) is a relatively newly described, acquired disorder of pigmentation characterized by geographic-shaped, gray-brown, hyperpigmented patches and interspersed with hypopigmented macules, involving the dorsal aspects of the forearms in postmenopausal women. There is a suggested relationship with hypertension and antihypertensive medication intake, specifically angiotensin-converting enzyme inhibitors, or a cumulative effect of sun damage, as possible triggers. ABCD is benign, asymptomatic, and more of an esthetic concern. Topical depigmenting agents…

Anterior interosseous nerve syndrome (AINS) represents a form of neuralgic amyotrophy (Parsonage-Turner syndrome). AINS does not originate from external compression of the AIN in the forearm. Fascicular constrictions (FCs) of the median nerve are identified within the anterior interosseous fascicular group at or above the medial epicondyle. Spontaneous recovery is not ensured, leaving up to 30% of patients with permanent weakness or palsy. Fascicular microneurolysis of the median nerve, performed at or above the elbow, is a treatment option for patients who do not recover spontaneously.

The length of tracheal defect or stenosis exceeded 5 cm could not be treated by simple resection and end-to-end anastomosis of the remaining trachea. Various ways of tracheal replacement had appeared sequentially, such as radial forearm free flap with cartilage grafts, tracheal tissue-engineering and tracheal allotransplantation. Among these methods, tracheal allotransplantation displayed a better long-term result. In this review, we are focused on recent advances in tracheal allotransplantation, particularly on revascularization and reepithelialization of graft, as well as on the application …

Brachioradial pruritus (BRP) is a rare type of chronic pruritus that usually localized at the dorsolateral part of the forearms. Itching, burning, or pain are common symptoms at the involved areas. The etiological factors are still unknown but sun exposure and/or cervical spine lesions seem to be trigerring or precipiting factors. Neuropathogenic mechanism plays role in etiopathogenesis of BRP, therefore, antiepileptic drugs such as gabapentin, oxcarbazepine. and pregabalin are suggested medications for BRP. Herein, we report three cases with BRP successfully treated with pregabaline.

Focal hand dystonia (FHD) is characterised clinically by a task-specific symptom and typical electromyography findings of a characteristic pattern of cocontraction of the agonist and antagonist muscles of the hand and forearm. The aetiopathogenesis of this condition is still not clear. We present a case of a patient with an unusual aetiology for this condition, in the form of retained hardware in the distal radius. This patient had complete resolution of symptoms after removal of a retained radial plate. Thorough history-taking, clinical examination and necessary investigations are the corners…

Limited cutaneous scleroderma is a subtype of scleroderma limited to the skin of the face, hands, feet and forearms. We present a case of a 45-year-old woman affected by limited cutaneous systemic scleroderma involving the orofacial region and causing restricted mouth opening. The patient showed noteworthy improvement of the skin lesion by use of a combination of intralesional corticosteroid with hyaluronidase and various multiantioxidants, resulting in amelioration of her mouth opening problem. The patient gave her full informed written consent to this report being published.

Adoption of the transradial approach for coronary procedures is rapidly growing in the US. A decrease in vascular complications and improved patient comfort are the primary benefits of this technique compared to femoral artery access. However, as this is still an invasive procedure, it is important for operators who utilize this technique to be aware of the known complications that are associated with radial access. Radial artery occlusion and spasm are the most common issues that occur with transradial procedures. Vascular complications such as perforations, forearm hematomas and pseudoaneury…

Erythema nodosum (EN) is the most frequent clinicopathologic variant of panniculitis with painful red or violaceous nodules on the anterior surfaces of the legs. The condition is a cutaneous reaction that might be associated with a wide variety of disorders or might be caused by medications that produce painful nodules on the shins, and less commonly on the thighs and forearms. In this paper, we describe, for the first time in the world, erythema nodosum as the simultaneous presenting complaint of monozygotic twin sisters after streptococcal pharyngitis. This paper might support the effect of …

The decision to apply spinal motion restriction should be based upon assessment findings. Mechanism of injury is a consideration, but ultimately the decision should be made on objective physical exam findings. The patient in the scenario has a significant mechanism of injury and a potentially distracting forearm fracture. However, was the pain from the fracture enough to distract him from participating in the exam and giving reliable answers? Did he need to be collared and boarded? When in doubt, or when local protocols are ambiguous, err on the side of caution.

This case report documents a rare cause of block to reduction in association with an anterior traumatic dislocation of the radial head in children. The radial head dislocations described were not reducible with a closed reduction secondary to "button holing" of the radial head through the anterior joint capsule. Open reduction was required to release the capsule and reduce the radial head. After reduction, the elbow was immobilized in flexion and the forearm in mild supination for 3 weeks. Postoperatively, all patients demonstrated full elbow function and range of function.

Long duration of intravenous drug abuse brings the risk of infectious and surgical complications. The case report of the forearm soft tissue and bone infection followed by rapid progress of compartment syndrome is described. Primary bone infection is unusual. It is necessary to think about it if the treatment of soft tissues comes short of the inflammation suppression. Especially if an intravenous drug abuser is concerned. The case stresses the necessity of thinking of all the diagnostic possibilities and bone affliction with its risks and of the well-timed treatment urgency.

The management of acute trauma to the upper extremity includes the urgent treatment of injuries and the timing and choice of surgical stabilization and reconstruction. To evaluate and treat severe upper extremity trauma, the orthopaedic surgeon should understand the principles of emergency department and operating theater management of commonly seen traumatic injuries to the distal humerus, elbow, forearm, wrist, and hand. A review of the principles for treating these complex injuries, including principles of soft-tissue coverage, will aid surgeons in achieving the goal of providing optimal tr…

We present the case of a 29-year-old black female with an initial clinical and histopathologic diagnosis of actinic lichen nitidus. Three years later, she presented with scattered hyperpigmented macules with oval pink/violaceous plaques bilaterally on her forearms and on her neck, clinically consistent with actinic lichen planus. She was treated with topical steroids at each visit, with subsequent resolution of her lesions. In this report, we discuss the spectrum of actinic lichenoid dermatoses and of disease that presents even in the same patient.

We report the case of a young man who presented with atrophy of the musculature of the right forearm and hand. The clinician suspected cervical myelopathy in Hirayama's disease on the basis of the findings at electromyography and recommended an MRI examination in both flexed and neutral position. The typical imaging findings for cervical myelopathy in Hirayama's disease were observed and the diagnosis was confirmed. The key findings for Hirayama's disease consist of asymmetrical medullary atrophy with involvement of the anterior horns, dorsal detachment of the dura mater, and dilatation of the…

We report the results of a new form of therapy for unilateral spatial neglect. Functional electrical stimulation (FES) applied to the left forearm extensor muscles reduced the symptoms of severe left unilateral visual neglect in three patients, with the benefits being measurable at 6 months post-treatment. We suggest that FES activates a proprioceptive map within the right parietal lobe whose level of activation is otherwise diminished by the lesion. This both increases awareness of the contralesional side and stimulates functional interactions with the environment.
