[Application of TightRope system combined with Locking-Loop biplane anatomical reconstruction technique for acute acromioclavicular joint dislocation].

OBJECTIVE: To investigate the effectiveness of TightRope system combined with Locking-Loop biplane anatomical reconstruction technique in the treatment of acute acromioclavicular joint dislocation. METHODS: A clinical data of 28 patients with acute acromioclavicular joint dislocation who met the selection criteria and admitted between June 2018 and December 2021 was retrospectively analyzed. There were 18 males and 10 females, with an average age of 47.7 years (range, 22-72 years). The causes of injury included falling (13 cases) and traffic accidents (15 cases). The acromioclavicular joint d

An Analysis of Hamate Morphology Relevant to Hemi-Hamate Arthroplasty.

PURPOSE: Dorsal fracture-dislocations of the proximal interphalangeal (PIP) joint can lead to joint incongruity from loss of the buttress function of the middle phalanx volar base. Hemi-hamate arthroplasty can reconstruct the volar articular surface of the middle phalangeal base where repair is not possible. We compared the anatomy of the hamate graft with the middle phalanx base. METHODS: Forty unique skeletal specimens (40 hamates, 160 middle phalanges) were sampled. Anatomical features relevant to hemi-hamate reconstruction were measured, including the articular surface areas, the axial ri

Outcome of Surgically Treated Fractures of the Condylar Process by an Endoscopic Assisted Transoral Approach.

PURPOSE: Fractures of the condylar process are frequent. Ideal management of these fractures, as discussed in the literature, is controversial. Some recent meta-analyses have favored open reduction and internal fixation using various approaches. A strictly transoral approach has been described to minimize scarring and risk of facial nerve injury but has restricted visibility. This retrospective study analyzed outcomes of patients with unilateral mandibular condyle fractures who were treated by open reduction and internal fixation through an endoscopic-assisted transoral approach. Da Vinci System: clinical experience with complex proximal humerus fractures.

The purpose of this study is to report the clinical and radiographic outcomes after open reduction and internal fixation of displaced proximal humerus fractures with the "Da Vinci System". It is a triangle-shaped cage whose opposite faces are pierced, and it represents the evolution of a triangle-shaped bone block technique performed in a previous series of 33 patients. The new device is an interesting innovation to treat the difficult problem of fracture fragments reconstruction and stability, metaphyseal bone loss and proximal humerus revascularization. According to the technique, authors po

MR imaging of acetabular fractures: value in detecting femoral head injury, intraarticular fragments, and sciatic nerve injury.

OBJECTIVE: The purpose of this prospective study was to compare the value of MR imaging in the detection of injuries associated with acute acetabular fractures (femoral head fracture, free fragments within the hip joint, and injury to the sciatic nerve) with the value of preoperative CT examinations, intraoperative inspection, intraoperative somatosensory evoked potentials (SEP), and clinical neurologic examinations. SUBJECTS AND METHODS: Coronal fat suppressed long TR/TE and unenhanced and contrast-enhanced T1-weighted MR images were obtained preoperatively in 37 patients with acetabular fra

Special Report: The Moore Pediatric Surgery Center: An Evolving Model for Pediatric Orthopaedic Surgical Care in a Limited Resource Environment.

ABSTRACT: The authors describe their 11-year experience with 1 model for providing short-term (about 1 wk/y in country) pediatric orthopaedic surgical care in a limited resource environment. This paper provides a detailed narrative of 1 team's pediatric orthopaedic work at the Moore Pediatric Surgery Center in Guatemala City, how it has evolved over these 11 years, financial aspects of the model, and examines patient follow-up data for a consecutive 8-year period. The authors have reviewed financial records, case lists, patient charts from 2014 to 2022, and patient photographic records from Th

Return to sport following Lisfranc injuries: A systematic review and meta-analysis.

BACKGROUND: Information regarding return rates (RR) and mean return times (RT) to sport following Lisfranc injuries remains limited. METHODS: A systematic search of nine major databases was performed to identify all studies which recorded RR or RT to sport following lisfranc injuries. RESULTS: Seventeen studies were included (n=366). For undisplaced (Stage 1) injuries managed nonoperatively (n=35), RR was 100% and RT was 4.0 (0-15) wks. For stable minimally-displaced (Stage 2) injuries managed nonoperatively (n=16), RR was 100% and RT was 9.1 (4-14) wks. For the operatively-managed injuries,

Arthroscopically Assisted Reduction and Internal Fixation of a Displaced Posterior Glenoid Fracture: A Case Report.

CASE: We report the successful use of arthroscopy for reduction and internal fixation of a posterior glenoid fracture in a 62-year-old man 5 days following a skiing accident. Additionally, we review the technique utilized for successful fixation of this rare fracture, and we report the postoperative return of function. CONCLUSION: Arthroscopically facilitated fixation of glenoid fractures has been reported in the literature with excellent clinical outcomes. However, arthroscopic reduction and screw fixation of a posterior intra-articular glenoid fracture (Ideberg type Ib), to our knowledge, h

Multiple Concomitant Injuries in One Upper Extremity: A Case Report.

BACKGROUND: Surgical correction of hallux valgus deformities often results in decreased first metatarsophalangeal joint (MTPJ) range of motion. Loss of motion has been shown to affect patient satisfaction. The purpose of this study was to evaluate the immediate change in MTPJ range of motion that occurs after a distal soft-tissue reconstruction (DSTR) and proximal metatarsal osteotomy (PMO). METHODS: DSTR and PMO were done on 16 below-knee cadaver specimens with clinically apparent hallux valgus deformities. Two examiners assessed preoperative and postoperative dorsiflexion (DF), plantarflexi

Terrible triad of the elbow.

The terrible triad of the elbow is a difficult injury with historically poor outcomes. Improved experience, techniques, and implants have advanced to the point where restoration of elbow stability can be expected. Careful attention to each destabilizing element of the injury pattern is essential and places high demands on the surgeon's mastery of the anatomic complexity of the elbow. Technically, the surgeon must bring every skill to bear, as soft tissue techniques, fracture repair, and joint arthroplasty are routinely required to adequately treat these complex constellations of injury.

The forearm complex: anatomy, biomechanics and clinical considerations.

The forearm complex is comprised of the proximal radioulnar joint, middle radioulnar joint/interosseous membrane, and the distal radioulnar joint. These three areas function in a coordinated manner to rotate the hand in space and allow performance of functional tasks. If a structure or structures in one of these three areas is disrupted, this can adversely affect the function at any of the other two remaining areas. Surgical intervention focuses on restoring anatomical alignment to preserve the function of the forearm complex. Rehabilitation is guided by the relationships between the three are

Undisplaced femoral neck fractures in children have a high risk of secondary displacement.

Femoral neck fractures in children are rare and known to have a high complication rate (e.g. femoral head necrosis, persistent deformities, and pseudarthrosis). While open reduction and internal fixation is the treatment of choice for displaced fractures, non-operative treatment methods have been proposed if the fracture is undisplaced. Three consecutive patients aged 11, 14 and 16 years with undisplaced femoral neck fractures were seen at our institutions and primarily treated with the recommendation of non-weightbearing and minimal flexion until consolidation. All three cases showed secondar

Le Fort fractures (I). A study of frequency, etiology and treatment.

The cause and frequency of maxillary fractures as well as type of treatment performed has been reviewed in this study. During the time period between 1969 and 1982, a total of 301 maxillary fractures have been treated at our Department of Oral and Maxillofacial Surgery in Gothenburg. The period has been subdivided in 3 parts to evaluate whether differences in the amount of severe fractures could be observed. Since a law was instituted in Sweden in 1975 calling for a mandatory use of front seat belts, the frequency of fractures related to the time periods before and after 1975 could be of inter
