Treatment of Peripheral Talus Fractures.

Peripheral talus fractures include injuries to the lateral process, posteromedial talar body, and talar head. These injuries are rare and are often missed. Nonunion with conservative treatment is high and excision can lead to joint instability, rapid arthrosis, and earlier need for arthrodesis. Open reduction internal fixation of most peripheral talus fractures is critical to achieving a good outcome. Open reduction leads to more rapid union and ability to mobilize the ankle and subtalar joints, quicker revascularization of the talus, and lower rates of arthrosis. Surgical treatment can lead t

Cephalomedullary fixation for femoral neck/intertrochanteric and ipsilateral shaft fractures: surgical tips and pitfalls.

OBJECTIVE: Surgical management options for femoral shaft fracture and ipsilateral proximal femur fracture vary from single-implant to double-implant fixation. Cephalomedullary fixation in such fractures has relative advantages over other techniques especially because of less soft tissue dissection and immediate postoperative weight bearing with accelerated rehabilitation. However, the surgery is technically demanding and there is a paucity of literature describing the surgical techniques for this fixation. The aim of the study was to describe the surgical technique of cephalomedullary fixation

Ulnar nerve palsy following closed fracture of the distal radius: a report of 2 cases.

Ulnar nerve palsy subsequent to a fracture of the distal radius is extremely rare compared to a median nerve injury. The lesion tends to occur in younger patents with a high-energy mechanism of injury and a severe injury pattern consisting of wide displacement, comminution, combined distal ulnar fracture and open fracture. The mechanism of injury can contribute to a direct contusion and traction, compression secondary to prolonged edema and tissue fibrosis, intraneural fibrosis and laceration. We report 2 cases of progressive ulnar nerve palsy subsequent to closed fractures of the distal radiu

Flexor carpi radialis tendon rupture as a complication of a closed distal radius fracture: a case report.

Although extensor pollicis longus tendon ruptures have been noted as a complication of distal radius fractures, flexor tendon ruptures in association with acute fractures of the distal radius are rare. We report a rupture of the flexor carpi radialis tendon as a complication of an acute distal radius fracture that was discovered during operative management of the fracture.

Treatment of femoral fracture through the sliding screw portal following fixation of the femoral neck fracture.

The authors report a case of secondary femoral fracture through the sliding hip screw portal following fixation of the femoral neck fracture. The femoral neck and subtrochanteric fracture was successfully treated by superimposing a trochanteric stabilization plate (TSP) onto the regular sliding hip screw.

Forearm bone non-union and its management.

A total of 125 patients with forearm (radio-ulnar) bone compound fractures resulting from missile injuries were examined 8 to 16 weeks after the time of injury with the objective of finding out the rate and causes of non-union. Initial management in each case was circular POP with windows for wound care or posterior slabs made of POP, Cramer-wire, or wood at random. Out of these, 22 (17.5%) patients had non-union. All were male, mean age 26 years, in good nutritional state and no underlying chronic illnesses found on routine examination. Fourteen had ulnar bone and 8 radial bone non-union. Fou

[Osteosynthesis and radiotherapy of the proximal femur: report of 6 cases].

PURPOSE OF THE STUDY: Post-radiation shaft fractures of long bones are uncommon. Late or partial healing increases the risk of stress fracture. Different fixation techniques have been proposed. Treatment of radiated bone is difficult. We report a series of 6 nonunions of the proximal femur after radiotherapy. MATERIAL AND METHODS: Three patients had true fractures of irradiated bones several years after radiotherapy for bone or soft tissue tumors. Three other patients had pathological fractures or malignant osteolysis related to a myeloma and treated by osteosynthesis and post-operative radio

[Biomechanical studies on plate prebending and pretensioning in compression osteosynthesis (author's transl)].

By tensioning plates which are contoured exactly to the bone shaft a torque is produced leading to gapping of the cortex opposite to the plate. The gapping depends on the amount of tension force and plate stiffness. In osteosyntheses performed on transversely osteotomized tibial and femoral shafts with plates adapted to the bone surface the gapping angles of tibia osteotomies with narrow plates are twice as large as those of femur osteotomies with broad plates. In order to eliminate the torque the plates must be prebent before application. Three phases have been defined to describe the tension

Morbidity associated with ORIF of intra-articular calcaneus fractures using a lateral approach.

This is a review of 183 patients with 218 displaced intraarticular fractures of the calcaneus treated by open reduction and internal fixation. One hundred and ninety four (89%) wounds underwent primary uneventful healing. Twenty-four wounds (11%) required local wound care. One deep infection occurred in a neuropathic foot that required below-knee amputation. No free-tissue transfers, local tissue flaps or skin grafts were needed in patients who presented initially with a closed fracture. Ninety-five fractures (43.5%) required subsequent surgical procedures (hardware removal-88/95 [93% of secon

Restoring Volar Tilt in AO Type C2 Fractures of the Distal Radius With Unilateral External Fixation.

PURPOSE: To determine whether unilateral external fixation using a staged multiplanar reduction technique restores anatomic volar tilt in a distal radius fracture model. METHODS: We used radiographic images to obtain baseline measurements in 20 fresh-frozen cadaveric wrists. Through a standard dorsal approach to the radius, we performed osteotomies to simulate displaced AO/ASIF type C2 fractures. After placement of a unilateral external fixator, a stepwise technique of applying longitudinal traction followed by a volar translational maneuver was performed. Radiographic imaging was obtained af

Unilateral hinged external fixator of the elbow in complex elbow injuries.

We present the outcome of treating complex injuries of the elbow with the "Orthofix" external fixator in patients where the condition of the soft tissues did not permit extensile surgical approach or where the internal fixation would not be stable enough to permit safe early joint mobilization postoperatively. Eight patients were treated, three with supra-intercondylar fracture in the presence of marked osteoporosis and five with fracture-dislocation of the elbow and excessive soft tissue impairment. The fixator was applied for a mean of 8.5 weeks (5-13 weeks). Great care was taken to use the

Fixation of Displaced Medial Epicondyle Fractures in Adolescents.

BACKGROUND: Open reduction and internal fixation of displaced medial epicondyle fracture dislocations in adolescents is recommended for incarceration of the epicondyle in the joint and for athletes who need a stable elbow for their sport. A screw placed across the epicondyle into the medial column avoiding the olecranon fossa is a common fixation method. One author has recommended adding a metal washer to the screw fixation because of the perceived risk of epicondyle fragmentation or penetration when using a screw alone. The purpose of this study was to determine whether the use of a screw and

Four part fractures of the proximal humerus.

BACKGROUND: Four part fractures of the proximal humerus represent approximately 5% of all proximal humeral fractures. Treatment options for these displaced fractures include nonoperative treatment, open reduction and internal fixation, and arthroplasty. Presently, there is a lack of consensus on the optimal management strategy for this complex injury. OBJECTIVE: To determine the effect of alternate devices in the management of four part proximal humerus fractures in patients on the risks of reoperation, need for implant removal at 1 year, and pain.

Fractures of the femur in relation to cemented hip prostheses.

We have reviewed the results of treating 75 fractures of the proximal femoral shaft in the presence of a cemented femoral prosthesis. A simple radiographic classification into four types is proposed, and suggestions are made on the appropriate management of each. Comminuted fractures around the implant need early revision, whilst spiral fractures in this region may be treated conservatively or by operation. Transverse fractures at the level of the tip of the prosthesis are difficult to manage, and may require open reduction and internal fixation.

Stable rates of operative treatment of distal radius fractures in Ontario, Canada: a population-based retrospective cohort study (2004–2013).

BACKGROUND: Rates of surgical management of distal radius fractures are increasing internationally despite the higher cost and limited outcome evidence to support this shift. This study examines the epidemiology of distal radius fractures and asks if the same shift has occurred in Ontario, Canada (population 13.9 million). METHODS: This population-based, retrospective cohort study examined distal radius fractures in people aged 18 years and older over a 10-year period (2004–2013). The incidence analyses were based on the first occurrence of a fracture within a 2-year time period. The number o

Transnasal endoscopic ultrasound-guided reduction of maxillary sinus wall fracture.

Surgical morbidity from open reduction and internal fixation (ORIF) of maxillary sinus wall fracture often surpasses the benefits of ORIF. Hence, the authors devised transnasal endoscopic-assisted reduction of maxillary sinus wall fracture (TERM) without internal fixation as a minimally invasive surgery for maxillary sinus wall fracture. The purpose of this study was to investigate the feasibility of TERM in cadavers and patients. Six cadavers were dissected to evaluate the feasibility of TERM. In addition, 20 patients with maxillary sinus wall fractures who underwent TERM in a tertiary hospit

Periosteal osteosarcoma and Marfan's syndrome: A case report and literature review.

Periosteal osteosarcoma (POS) is a rare primary malignant bone tumor arising from the surface of long bones. In addition, Marfan's syndrome (MFS) is an infrequent hereditary autosomal dominant connective tissue disorder with high penetrance and variable phenotypes, which primarily affects the ocular, skeletal and cardiovascular systems. The present study reported a case of POS and MFS co-occurring in a child. A 6-year-old girl with MFS presented with pain, swelling and deformity in the right thigh following a fall. The patient was diagnosed with a right femoral shaft fracture and underwent ope
