Efficacy of acute intravenous verapamil, 10 mg, and chronic oral verapamil, 320 mg, daily were studied electrophysiologically in 15 patients with paroxysmal supraventricular tachycardia (PSVT). Toxic effects of 14 esters with chemical properties similar to those of pyrrolic pyrrolizidine alkaloid metabolites, were investigated in rats. Disopyramide phosphate (DF) was administered intravenously to 50 patients during 55 episodes of arrhythmias. Diazoxide was injected into the pulmonary artery in nine patients with primary pulmonary hypertension. Pharmacokinetic data were used to calculate an i.v. bolus dose and infusion regimen for tubocurarine (dtc). This produced continuous paralysis in 12 anaesthetized patients. Three patients are described who either died suddenly or had severe, life-threatening arrhythmias during or immediately after doxorubicin administration. Since doxorubicin and daunorubicin administration is known to be associated with acute EKG abnormalities, the acute decompensation in these patients appeared to be caused by the administration of these agents. The importance of careful observation of patients receiving doxorubicin, because of the possibility of acute cardiac arrhythmias, is stressed.
